With today's technology and the rising demand for CPAP there is no reason we shouldn't have custom fitted nasal pillows. Everyone has different size and shape nostrils. Most people have different right and left nostrils. You can buy custom fit hearing aids and contact lenses. I can't ever have a comfortable fit on my nasal pillows so I put up with the leaks, frustration and irritation. I would pay good money for a perfect fit nasal pillow.
You can get custom but why do you need it? It's not like hot nostrils are super different sizes like a bra to the point where it's a huge difference. Why not switch to the n30i or similar?

I’ve been trying for I don’t know how many months to make CPAP work for me. They sent me so many masks to try 😔 I was giving up and thinking it was all BS I was ripping all the other masks off and having nightmares. I kept watching your videos trying to figure out what I was doing wrong. They even suggested the implant. Until a new girl sent me the Philips dream wear nasal mask, my life has completely changed! Even though everyone INSISTED I needed a full face, this one worked perfectly for me! I don’t get it, but, I sure am happy 😊 Thank you for all you helpful videos 🙏🏻

Oh gosh…. i’m on month six and still don’t have the right mask. I feel like I’ve tried everything but I really haven’t. I’m still searching. I wish there was an easier way instead of paying for something and knowing right away that it’s not gonna work sticking it out and trying it for a week or two and then realizing it’s just not gonna work and then having to buy something else. I’m just so tired of it. I’m ready to give up six months I mean that’s a long time.
I used the Resmed AirSense 10 CPAP for 6 years and now the AirSense 11 for 1 year. I used the small P-10 pillow mask with both devices. Tried the medium pillow but didn't like it. Sometimes, the outer lower part of my nose would get a little tender but I continued using it. Then I saw your video a while back about the Brevida pillow mask. In the video, you mentioned you used the M/L pillow but thought it was too big for you and could probably wear the XS/S mask. I read many reviews and also took your advice and got the Brevida pillow mask in the XS/S size. It's a game changer for me. Been wearing it for 8 months now and I freaking love it! It's way more comfortable in my nose, no irritation whatsoever, and my leak rate is almost nonexistent. My apnea count is a little lower too. Thanks for sharing info about the Brevida mask. Greatly appreciate your videos and your honest opinion about CPAP info.
Thanks so much for this tip. I'm new to CPAP and was given an N30 "hybrid" mask with a medium cushion. The N30 was a challenge in terms of a proper fit and I was never that comfortable with it. I did determine that I breathe mostly through my nose (unless my sinuses act up) so I bought a P10 nasal pillow mask. Using the medium insert I experienced frequent leaking. After watching your video, I put the the large insert and over the past two nights, I slept much better and almost no leaking. My event numbers went way down as well. Maybe that won't last but, on its first try, large appears to be the better way to go.
You so much. I think us newbies think the pillow has to go into the nostrils, but the seal to the outside is more important. Excited to use larger pillow and see if it solves my problem.
I was using a small nasal pillow and the outside bottom of my nose was getting red and sore, dah I changed to a medium and wola no more irritation. Lol 😮 love your channel.😊
I got my first CPAP machine in the late nineties. (You know the reverse vacuum cleaner with the Y2K compliant sticker.) It was almost five years before I realized they had nasal pillows. I turn a lot in my sleep and like to sleep on my stomach. The P-10 with large pillows was a godsend! I so hated the huge headgear on face and nose masks. I recently bought the F&P Brevida to test new options. It is a good mask, but doesn’t let me sleep on stomach as readily as the P-10. Thanks for the video!
Hi there Jason, love the videos as always. Could you please do a video on UARS and how to treat it using bilevel / more advanced forms of therapy? I've been having a really hard time as of late, as I've been trying to navigate college whilst not getting any sleep. It feels like I keep on going in circles and getting my hopes up, only to be disappointed yet again. At first, I got a regular AirSense 10, but after 3 months of trial and error, I concluded that it wasn't gonna get the job done. I got an AirCurve VAuto about a week ago, and, despite using OSCAR and trying out tons of different settings, whilst keeping my leak rates minimal, I still feel terrible. I don't really know what to do at this point and am kind of losing hope that I'll ever be free of this thing. Thanks again for the videos; they keep me going when I feel like giving up.
What about your pillows etc? Are you using an app like sleep as android or sleepscore, etc. to see what else might be the issue? Is your rdi bad or is it good and you are still having problems? Also is your leak rate really good or are you having grey lines on Oscar? (Did you set Oscar all you can see flags 1 and 2?)
I was "fitted" to a medium P10, and had the results you said, feeling like I was breathing through a straw, and needing to bend it to fit into my nose. I upsize to a large per your advice and that's what I use all the time.
I'd suggest adding a link to the Lansinoh lanolin cream you had mentioned in your description as it might help others with the nasal soreness using the pillows. Thanks for the info here, helps a lot.
Vanicream face moisturizer, it's much cheaper and works amazing. Doesn't have lanolin in it as lanolin is an allergen I guess and people can be allergic to it 🤷🏼 (vanicream is like the ultimate "if you're allergic/sensitive to everything" use this brand). I tried it because I like their face wash so much as a dupe for a pricey luxury line I was using -even if the name sounds like a 1950s medical product) and was shocked at how well it worked for cpap nose and mouth taping redness/irritation. (And I'm ginger so it was really really red...)
P10. Problem isn't the inside of the nares in general, but more on the columella, at the very bottom, and on the inside, left . Working on resolving it.
@@screamtoasigh9984 Nares: the holes in the nose. Columella - the skin covered part of nose at the bottom that separates the Left from the Right nare, running from the tip of the nose back towards the head. DME sleep tech just dropped off an N30i to try tonight. Does that help?
I took my P10 off last night at 12:30, as the nose pain was really bad. I have a scab on the inside of my nose, despite using vaseline for two nights. I’m taking tonight off from my CPAP to give my nose a break. I’ll go from a medium pillow to the large to see if it helps.
I enjoyed watching your video at 2:30 am in my London hotel room. Obviously, some jet lag going on here. Traveling with a machine and masks is new, but no problems getting through security. I'm three weeks into using my BiPap vAuto ResMed machine. I feel I'm months ahead of other new papers. I really do appreciate your informative and entertaining videos. I love the Bleep Eclipse and use it most nights, but tonight I tried the F&P Evora nasal pillows, and some Cover Roll stretch tape, too. I like the Evora, but after watching the video, I think I'll order the Brevida from CPAPSupplies, with discount, of course.
Well Jason.. it was a bust for me. I've been using small and cramming my pillows inside nostril. Just tried the large and LEAK city. I'll try to get a medium and try it. I have appt with sleep center Jan 28.
I have a P10 and a Brevida. I like the P10, but you explained what I feel some nights. I feel air starved and I have to breath through my nose to "catch up". I have a large, but I feel like I need an extra large which doesn't exist. For some reason with the Brevida I don't have this issue even if the opening seems to be about the same in the M/L cushion I use. I can't explain it, but I feel so much better with the Brevida than the P10 that I've switched away from the P10. I'm hopeful that the Solo will be the happy middle ground.
My problem is keeping the nasal pillow (p30i) against my nose all night. I often wake up and the pillow is completely away from my nose (even pulled under my chin one night). I'm considering giving the n30 a shot to see if the smaller headgear works better...
P30i has a smaller headgear option. It's the same as the n30i, so just get the small 30I headgear and you can try both the p30i and n30i with the same headgear.
Great video. I have the best overall results with the Dreamwear nasal pillows with the N30i as a back-up. I do have a large nose and will switch from the med. to large to see which works better. I have never had great results with the Brevida. I really want to like it but for me it leaks. One older nasal pillow mask that gets over looked is the Opus 360.
Almost exact same results as you, use n30i (and after consultation with Jason/lanky lefty - a full face mask f30i cause I was leaking a lot, I do use the mouth tape he recommends now as well). I switch between those and occasionally the evora. The brevida small was too big from my nostrils and was irritating. I accidentally also used the medium n30i and f30i and realised it was too big so switched to small. The p30i doesn't come in x small and left me air starved. I said we have similar issues even though I don't have a large nose (cough rhinoplasty cough) because I experienced the same issues and use the same masks.
It took me a month to get the pressure right after going up in pillow size.😬 Apap to start with ... too much pressure... blew the tape off my face. Went from 10 down to 9 with a stop at 9.5 . Switched to cpap @ 9 Oscar agrees this is a good pressure. Hard to believe that switching pillow size would make that much difference.. Thank you... ahh Jahsahn 😁

Hi Jason, great information on the size. I've actually seen very little videos on how to properly size different masks. I've tried the nasal pillow masks, I'm not really a fan of those, but I do really like the N 20 nasal air touch mask. Med and Lg both seem to fit OK so I just choose one. Any thoughts on properer sizing / fit? Thanks.

Tried a nasal pillow mask and it fit well, but even just tightening up the headgear *juuuuust* enough to stop it from leaking, I would wake up feeling not chafed or raw, but rather like I had been punched in the nose hard enough that I expected to see bruising or some kind of mark. I never was bruised or marked, but it just felt that way. I quit nasal pillows after about 5 days of that mess.
You mentioned setting your machine to nasal for Bleep -- I'm curious what changes when you pick different mask settings (pillows vs full face vs nasal). Do you have a video on that by chance?
Omg... I've always used the small and stick it up my nostril. One that is deviated, i use a crochet hook to pull my nostril open to stick it in. This is eye opening. I need to try the large. Thanks. Also, is it true to pick full face for the setting with pillows because it will give a bit more air. I've been doing that.
Very serious question that for whatever reason i cant find the answer for, how the hell am i supposed to breathe with a cpap i feel overwhelmed with pressure no matter what. Just got the nasal pillows and if i open my mouth its very uncomfortable
Hi Jason, I tried both P30i Medium and Large, the M seems fits better, whereas the L feels more comfortable. Except the L sometimes, in the middle of the night, slightly and partially sticks out of my nasal, which causes leakage. But, I can't really tell the difference. Can we tell which is better from the flow rate, like by using which one, I get more air? Cheers
Oh no don't do that, why would you use expensive glycerine? use vanicream face moisturizer, cheap and great for sensitive skin. If you have a huge huge issue you can use a medical skin liquid gel or spray they use to apply electrodes, it's called barrier film, or skin protective dressing. It's to stop irritation from friction or moisture. They use it in things like before people wear electrodes for TENS, to stop irritation from medical tape and stuff like that. But if it's really bad long term it's probably your gear. I have super sensitive skin and only had irritation from two masks, the swift fx has a weird nasal angle that was rubbing my skin and the brevida which only came in small andI needed xs so it was too big around my nose.

Jason, I'm new to cpap and have watched many of your videos and you say a lot to use OSCAR to monitor and evaluate sleep. My issue is my cpap machine is not supported by OSCAR. I have adjusted my machine from apap to cpap and by the way I feel after waking now it was the right decision. I guess my question is, if my machine data can't be analyzed using OSCAR how can I adjust to get the best sleep possible? I'm pretty much on my own, haven't heard from the Dr that diagnosed me with OSA since the sleep study( over a month ago).

I'm guessing the other masks made you breath through your mouth....which is not ideal for the majority of cpap users! 😉

I use the ResMed P10 Medium and Oscar is showing a lot of mask leaking. If I wear the small, I struggle breathing air in.

N30 is a leaker. N30i is much more stable.
P30i has a smaller headgear option. It's the same as the n30i, so just get the small 30I headgear and you can try both the p30i and n30i with the same headgear.

@kerrib1474 Don't give up... I've been using oscar for a few years now. It took me awhile to get it figured out, but it's worth it.

Sounds like they were too small

Hi Jason, I tried both P30i Medium and Large, the M seems fits better, whereas the L feels more comfortable. Except the L sometimes, in the middle of the night, slightly and partially sticks out of my nasal, which causes leakage. But, I can't really tell the difference. Can we tell which is better from the flow rate, like by using which one, I get more air? Cheers

Oh no don't do that, why would you use expensive glycerine? use vanicream face moisturizer, cheap and great for sensitive skin. If you have a huge huge issue you can use a medical skin liquid gel or spray they use to apply electrodes, it's called barrier film, or skin protective dressing. It's to stop irritation from friction or moisture. They use it in things like before people wear electrodes for TENS, to stop irritation from medical tape and stuff like that. But if it's really bad long term it's probably your gear. I have super sensitive skin and only had irritation from two masks, the swift fx has a weird nasal angle that was rubbing my skin and the brevida which only came in small andI needed xs so it was too big around my nose.
Only use lube without silicone 😉

Jason, I'm new to cpap and have watched many of your videos and you say a lot to use OSCAR to monitor and evaluate sleep. My issue is my cpap machine is not supported by OSCAR. I have adjusted my machine from apap to cpap and by the way I feel after waking now it was the right decision. I guess my question is, if my machine data can't be analyzed using OSCAR how can I adjust to get the best sleep possible? I'm pretty much on my own, haven't heard from the Dr that diagnosed me with OSA since the sleep study( over a month ago).
