    Thank you for these! I will pass them along. About the rising heels one. There's a Pilates exercise on the Reformer during Footwork when we raise and lower the heels, so they go from plantar flexion to dorsiflexion usually ten times. I've had clients in the past rush to the bathroom right after. And most of us feel some sensation in our bladder during doing it. Keep in mind there's resistance from the springs of course, not identical to what you're showing here, plus we're laying down. So I found it counter intuitive that it would be suggested as a trick to stop/control it, especially to someone who already has issues. Would you mind explaining it a bit more? Also, do you think that the exercise I described would be beneficial or the opposite for someone with bladder or prolapse issues? I'm a bit concerned now. Thank you!

      I had typed a big long response to this one on my phone and then lost it as I was getting on the train!
      Here goes again… (and I’m on the train again so wish me luck 😂)
      The studies into plantar flexion specifically relate to detrusor overactivity. They have shown that the exercise reduces bladder pressure. The mechanism of action isn’t fully understood but it’s believed to interrupt the signal that is triggering the premature contractions of the bladder.
      What’s important to note is that the exercise is performed during the urgency episode, as opposed to when the bladder is relaxed. This exercise is less likely to work if the urgency episode is not being caused by the bladder contracting.
      When it comes to performing exercises on the reformer with dorsiflexion/plantar flexion and that producing urgency, I would have a few questions. Is the exercise performed at the start of the session, or closer to the end? Do the people who experience urgency in that exercise have issues with urgency normally?
      It is normal for the bladder filling to slow during intensive exercise and to ramp up quickly when the intensity has subsided, which can result in a very full bladder after working out.
      To your next question, when it comes to bladder issues and prolapse, it’s really important that the person performing the exercises can control intraabdominal pressure and produce sufficient tone in the pelvic floor to manage any increases in pressure. That’s what makes the difference between an exercise being safe or not with prolapse. It’s also sometimes the case that very strong fit people struggle to balance pressures, so work with subtle activations and making sure everything is going it’s job before adding resistance.

      @@TheFlowerEmpowered sorry I made you type again! And such a thorough reply, thank you! I will relay the info on plantar flexion during an urgency episode and keep it in mind too. As far as the exercise, we normally do it in the beginning of a session. It doesn't happen often, but in my twenty years of working it has surprised me a lot whenever it has happened. And this whole looking into this matter right now has opened up many questions as you can imagine. We teach people from day 1 about lateral breathing and how to use their abs during Pilates but I am a bit worried not everyone shares info that personal with us sometimes so as to know what to avoid. In any case, again thank you, and I'll be studying your videos and blog for more clarity! 🙏

    Very good video. I also read recently that a stool softener can help in some instances.

      Thanks! I hadn't heard stool softener for bladder urgency but just looked it up and there is some research linking constipation to bladder urgency, so I suppose it makes sense that treating the constipation can be helpful.

    How long should one hold or stay in Supta gummy bear?

      I would relax in supta gummy bear for a minute or so. The key thing, regardless of which of the tricks you use, is that you take a relaxed bladder to the bathroom afterwards. This helps to communicate safety to the nervous system around bathroom breaks which is an important aspect of returning to normal function.

    Whenever i do reverse kegels and diaphragm breathing it helps my frequent urination, however my sinus is badly blocked so its very difficult for me to perform both. Do you think that unless i manage to get my sinus clear it will be impossible to be successful in doing diaphragm breathing and reverse kegels. Also do you have any tips of clearing sinus i have already tried netti pot helps a little but not enough to clear sinus

      Netti pot would be my suggestion, but you need to do some for of inversion after netti pot and should do the netti pot daily. If you do netti pot and then a gentle yoga routine that includes downward facing dog (very simple inversion), that helps with the movement of lightly salted water through the nasal passage. The nasal passage is a strange chamber that can be hard to empty, which is why the netti pot should be followed by moving yoga. You will likely find that you are half way through your yoga practice when it starts dripping again. Some twisting poses added can also help. Maybe try this one: ua-cam.com/video/XHHjJr-2JgE/v-deo.html It's gentle and there isn't a lot of inversion but it might be enough. Add an extra downward facing dog at the end if it doesn't help. Try first thing in the morning after your nights sleep and do it consistently for about a week to see if it helps.

    Is overactive bladder just feeling like you have to go before your bladder is full, like when you have been irritated by scented toilet paper, or is it actually having a full bladder all the time, like when you take diuretics?

      It’s more that you are getting signals that you need to go frequently when the bladder isn’t actually full - so full signals when the bladder doesn’t have much urine in it. It’s typically diagnosed when all other conditions have been ruled out. Make sense?

    My lower abdominal muscles feel cramped and it pulls down there especially towards the end of voiding what does it mean, what can I do against it?

      Where do you live? It would be good to see someone why specializes in visceral mobilization. It’s a technique that releases connective tissues adhesions which can give symptoms like you have described. It’s usually physiotherapists and osteopaths that do visceral mobilization.     I'm only 24 but my bladder has gotten so bad just to function I legit have to wear a full on diaper 😭 as helpful as they are it's so humiliating and It sucks so much🥺❤️

      Whatever you do never ever have a vaginal birth. There's nothing "natural" about it and you will pay with guaranteed permanent bladder damage. Don't let doctors fool you into thinking putting even slight stress on your pelvic floor is fine.

    Would it be bad to take deep belly breaths on a full bladder as it applies pressure and relaxes the pelvic floor?

      If you have bladder issues and you and bearing down when trying to breath deeply, this can cause some additional pressure on the bladder which can exacerbate the bladder issues. Are you having bladder irritation?

      @@TheFlowerEmpowered yes I feel like it pushes onto the bladder, which is why I then breathe more shallow. But I also have a very tense abdomen from trying to hold it all in and when I try to relax I find it hard to tell wether I’m relaxed or bearing down.. also I sometimes get kind of muscle ache in the lower abdomen even though I haven’t performed any exercises. But how can I relax that region when breathing doesn’t seem to work?

    I get abdomen pain when I try to hold urgency because I have primary bladder neck obstruction so urine frequency and urgency is common so how to prevent pelvic pain because it's sometime impossible to find washroom outside

      Primary bladder neck obstruction is tricky because it’s the internal sphincter that doesn’t relax, which is not under voluntary control. Has your doctor given you anything to help relax the bladder or are they doing watchful waiting?
      Pelvic floor relaxation exercises may help as they will reduce pelvic tension. The cause of PBNO isn’t fully understood, but like all bladder conditions, it is good to avoid bladder irritants and reduce stress as much as possible.

    I get this awful pressure feeling and suffer from urgency and frequency I have even completely cut out alcohol and caffeine literally all I drink is water and sometimes milk yet I still suffer plz help pelvic flower lady 🦋🌺

      Have you done the bladder diary? It’s really good to see the pattern of the bladder. Do it for 3 to 7 days to see what it reveals about your specific pattern. Also, make sure that you get checked by doc and urologist if possible

    Hi. I have two questions if you could kindly respond.
    1) Do any of these urge suppression techniques work inside the washroom? My urgency goes from barely noticeable outside the washroom door to almost 10 by the time I reach the pot or commode. Is there anyway I can calm the bladder so I can pee in a relaxed manner? My holding capacity has thankfully increased by a lot and I feel I could even reach the maximum capacity now if I keep training but the problem is that the more full the bladder is, the stronger the sudden urge increase inside the washroom which literally has me dancing inside to barely make it.
    2) I have a technique that I use to trick my bladder mechanism where I imagine I’m just going to the washroom without the intention to urinate. I can easily reach the pot. If then I decide that I have to pee, the urge gets stronger suddenly again but that’s barely a few seconds because I’m already over the pot. However I feel this technique is not workable because I can’t always trick my bladder like this and there are circumstances where we have no choice but to go to the bathroom only with the intention to pee. Can you kindly advise?

      What you are experiencing is similar to the “key in the door” urgency. Your nervous system knows that going in to the bathroom is associated with urination and due to your OAB, the bathroom acts as a trigger. The tricks in the video can be used regardless of where you are, so they should also work in the bathroom. Some advice from my personal experience:
      1. Go in to the bathroom and wash your hands when you don’t need the bathroom. If you get urgency, use the tricks and leave the bathroom. This helps to teach the nervous system that bathroom doesn’t always mean toilet.
      2. Do not pee in the shower (if the bathroom is the one with your shower). Use them in the bathroom only for showering at shower time and an opportunity to practice continence. You can leave the bathroom for a few minutes and then come back in to pee if you had the urge.
      Lastly - the most important part of bladder training - stop when you have achieved your goal. Then let your bladder function return to normal without measuring and monitoring. This is a critical step to returning to normal when all is done.

      Just to elaborate, I face this sudden increase in urge in two circumstances:
      1) When my bladder has reached its current limit, or is close to it, then I face this sudden increase in urge as soon as I decide to go and it becomes unbearable the moment I reach the door or sometimes even before. But in such cases its already very uncomfortable because the bladder is nearing capacity. But the sudden increase in urge is still palpable.
      2) If my bladder is half full, then this sudden strong urge inside the washroom doesn’t hit until I decide to pee. For example sometimes I go into the washroom with a half filled bladder with no intention to pee and there is no urgency increase at all. I would sometimes decide that I am not going to pee and literally stand over the toilet and nothing happens. But as soon as I decide the sudden strong urge comes crashing through. But I can’t trick my mind without really meaning it. So sometimes I would say to myself that I’m going to trick my mind but as soon as I am over the toilet comfortably, I’m going to pee, then in that case the urge starts as soon as I enter. So if I don’t have to pee, then my subconscious knows and when i have to, it knows it as well.
      Can you kindly guide how to manage this?

      Not sure there is much more I can add to be honest. What you think impacts what manifests in your body. You will need to use reassuring thoughts to on help communicate with your nervous system to o help calm the urgency. I would suggest you watch some of Lorimer Molesly’s videos on pain as it has a similar pathway to the urgency signals.

      @@TheFlowerEmpowered Sorry for bothering you again but I need your kind advice in relation to two things related to bladder training.
      First question:
      I can manage the urgency inside the washroom through two mechanisms. The first one (which is harder) involved the combination of contractions and breathing. It gives me time to sit and pee before the urge comes again. The second one (which is easier) is that I walk out immediately from the bathroom whenever I get a strong urge before sitting on the toilet side. Once I go out, the urge subsides. But it gives me around only 30 seconds before the urge comes again. But the time is enough for me to reach the toilet seat safely. As soon as I sit, I start peeing. Which of these two techniques do you think is more appropriate for managing the latchkey urgency near/ inside washroom?
      Second question:
      I feel that the higher my bladder is filled, the greater is the latchkey effect. For example the latchkey urgency at 250 ml is harder to suppress than lets say at 200 ml. I'm trying to train the bladder to be calm until I reach the toilet size. So is it necessary that I do this at volumes where I genuinly have the urge to pee or will it have the same effect if I do this training at first urge (200 ml)?
      (I did capacity training a month ago by just holding on and then running towards the bathroom when it became unbearable. I started touching 520 ml but it was a useless exercise because it didnt solve the latchkey problem. I would get the first urge at around 250 ml and things started becoming uncomfortable at 400. However, the basic problem of unwarranted detrusor contractions before reaching the toilet seat remained then even when I wemt at 350 ml. So lomg story short, if my latchkey urgency associated with my decision to go pee can be solved, the capacity issues can be resolved later)

    Hello, i have symptoms of overactive bladder after i got an anal fissure do chronic anal fissures and constipation affect the muscles of the pelvic floor?

      Yes, they can. Best to get an assessment by a pelvic floor physiotherapist if you are having pelvic floor symptoms. Pelvic floor tension can contribute to constipation.

    Any tips for the other waste product we have? Yes, it is very uncomfortable talking about the other waste product as it happens much less frequent than urine. While the vast majority of fluid output is through urine, and your pee is basically just an excess output for the stuff you drink plus water soluble substances such as vitamins, minerals and other water soluables nutrients in addition to excess urea exit in your pee when your body has enough of these. Your pee is a very mild waste product compared to your poo. The vast majority of the content of urine is not waste unlike your faeces. Your faeces is basically the waste product from what you eat. Undigested food, billions of bacteria and fibre head out that way. Water is also good softening your bowels and a small percentage of water exits in your poo. My question is, does these exercises help rid the system of your poo especially if you are constipated? Do you plan on making bowel control videos?

      The tricks in this video do not directly help constipation, however, movement helps a lot. There are many potential causes of constipation, and there can be issues with peristalsis (the movement of the smooth muscles in the intestines etc). Yoga or other exercises that bend and flex the spine help due to the flexion, extension and twisting that compress and decompress the contents of the intraabdominal space (which is a vacuum chamber). Both active and passive yoga forms can help.
      If pelvic floor hypertonicity is contributing to constipation then pelvic floor relaxation can help a lot as it improves the ability to relax the pelvic floor.       You are correct when you highlight that there is a difference between urine waste and fecal waste.
      One of the key differences between the urinary system and the digestive system is that the digestive system is a single pipe. What you put in your mouth and swallow travels through the pipe coming out at the other end, albeit in a waste form as the micro biome had broken it down allowing the nutrients to be pulled in to the body for use.
      The urinary system in the other hand has no direct connection to fluid coming in through the mouth. Rather, the blood being is filtered through the kidneys which create the urine. Urine excreted by the kidneys includes nitrogenous waste products (ammonia, uric acid, urea, creatine, creatinine, and amino acids) as well as excess salt and water. This action of excretion of urine by the kidneys helps to maintain blood pressure.
