I had a decade of several bowel obstructions a year. GI could not diagnose so I was given a bunch of medications. My last obstruction was a solid month of vomiting, excruciating pain, no food. Hubby took me to GI again who wrote a script for a hundred percocet and told me to go home. Hubby handed him the script back and insisted he start doing some tests. Hospital did a CT scan, admitted me and scheduled a resection as soon as I was stable. Surgeon said it was a "bad u-turn" and had to come out. Surgery is big, 11-day hospital stay, long recovery but worth it. Wish it could have happened sooner.
I’m sorry to hear that and thank you for sharing, sounds like a “kink” in the bowel from adhesive disease, some thing just can’t be fixed with a pill, a knife is needed. Glad to hear that you’re feeling better!
I almost died last year from 70cm of dead septic intestine removal with a large obstruction I spent 6 months of vomiting prior and was in the hospital already for ammonia in the brain which put me in a coma but didn't get rushed in for emergency surgery for obstruction for almost 4 months later, this was after 6 months of vomiting with no notice of problem... I'm lucky to be alive
I hope you talked to an attorney. There is no excuse in medicine for your outcome when protocol is followed. Someone dropped the ball based on your short comment.
Hospitalized 2x for SBO and I did have surgery when I was 2 days old. My problem was a hernia. After surgery to repair hernia in 2020, I no longer suffered from SBO.

@@citizensurgeon Thank you. I am enjoying a rainy weekend which is cause for celebration and hopefully a rainy week to come. We have bad wildfires here - one that has taken out 200 homes a few miles away. It has been a stressful week with one evacuation that was just five minutes from my place. That has thankfully now been lifted.
Hello, for the last 5 months I have on and off episodes (lasting 48 hrs) of abdominal pain (Pain level 10) and vomiting (NO FEVER). Last year I went to the emergency room and the SCAN confirmed that I had bowel obstruction. I was immediately rush to surgery ( Laparoscopy), the surgeon told us that there was no obstruction observed while in surgery or it resolved itself. After this episode, 6 months later, i was in the ER again same symptoms and the SCAN confirmed bowel obstruction, i was admitted to the hospital (no surgery), the bowel obstruction resolved itself. Last week, i was again in the ER again same symptoms and the SCAN confirmed bowel obstruction. I refused to go to the hospital and it seems that the obstruction resolved itself (no pain, no vomiting, eating and drinking) bowel movement is mixed. THE QUESTION IS... how can i prevent future obstructions, this is VERY STRESSFUL . Please let me know if you have advise.
I’m having same issue too. Just went thru 2 days of severe pain and vomiting before it resolved but didn’t go to the hospital this time. I’ve been there too many times over the years. I wish I knew how to make it stop happening. I now have a “do not eat” list of items I think cause the issue but wonder if it really is the food or the adhesions from previous surgeries. (First sbo started from adhesions from a c-section scar 9 years ago)
@@simonbryant3828 That’s interesting. I haven’t notice a correlation with black pepper yet. First, I have been taking 2 beano every night before dinner (I set an alarm) for a few years now and it’s helped me tremendously. Something about breaking the gas bubbles down to little tiny bubbles helps them pass through my intestines so much better. As far as the “do not eat” list it’s things that produce lots of gas or don’t breakdown easy so no cauliflower, broccoli, cabbage, mushrooms, corn on the cobb, etc. Garlic and yellow onions are ok only if they are cooked really well and part of a bigger dish like spaghetti sauce. I also try to chew my food a lot but don’t always remember to. I’m awful with my water intake too so that doesn’t help.
I have all the sumptomps of sbo but my doctor cant understand. Can you kindly check my ct scans please? I lost 26 kg of weight and still loosing. Thank you
I just hit my 1 year “ alive date”. I cannot believe it’s been a year and I’ve made it this far. Recovery from surgery is very brutal and trying even for the strongest. So my symptoms arrived out of the blue one day. Pain. Period. That’s all I had. No vomiting , no nausea. Straight pain near the belly button. Went from a 5-6 to a 10 within a few hours. Went to ER and was not treated with urgency. Scans revealed an illius and blockage. They attempted to evacuate the fluid and air in my abdomen which didn’t help. I remember at one point using glycerin of some type through the anus to relieve the blockage. Ultimately my intestine ruptured. Thank my lord I was at the hospital when it happened or I would be dead. They took 6.5 feet of small intestine and large bowel. Recovery is just plain living hell. I don’t know how I’ve made it a year. I will only say my surgeon was a miracle. Post op report showed an incredible amount of necrotic tissue. I was dying from the inside out. We never found the cause. Scary as hell. I wish we knew so I could avoid whatever it was. Not knowing scares me because is it going to happen again and if it does, I’ll end up on bags for sure. If by chance the doctor that created this video reads this, I am inspired by your passion. It’s isn’t just med students who end up here. Patients who have been through this tend to research for themselves. They are their own best advocates! Anyway, I hope you read this because I want to thank you. Doctors like you save patients like me by being the master of their crafts
I’ve had 23 small bowel obstructions since 1995 my last one two weeks ago. I’ve had small bowel removal resection times 3 secondary to a nicked bowel during an emergent appendectomy.
Thank you sir the video was extremely informative indeed. I just wanted to inquire about pain control from the moment the patient presents to the ER till the surgery is done.
Thank you for the question and thoughtful feedback. After the diagnosis is made I feel that nasogastric decompression can provide significant relief and intravenous or per rectal acetaminophen is helpful followed by intravenous opioids if needed. I usually avoid NSAIDS initially as these patients are usually hypovolemic and may be at risk of renal injury.
Wow Doc, what a comprehensive video on small balance obstruction. It was even not that difficult for the layman to understand. Last two months I've never had a decent bowel movement. And it's nothing in my rectum I already checked that. But every time I take miralax and Ducosate. I have a loose movement very watery and like sand particles at the bottom of the toilet . And a decent amount almost as much as whatever I ate that day so I've been doing this for a few weeks including enemas but now I'm starting to think that I have a fecal impaction maybe or some sort of blockage and when I take the laxatives and the enemas I'm just driving the product that's on top of the mass around it sort of like sneaking down between the intestinal wall and the blockage maybe. I have made a decision to go get checked at the ER. The nearest gastro appointment is 2 weeks I don't want to wait that long but I have waited after watching videos especially yours I'm very concerned. Anybody out there have the same or similar situation and were they able to take care of it at home??
Dr. I went to emergency room, they did a CT scan but the doctor sent me home with 2 prescribed meds. One for IBS and the other an antiacid. I had been in pain and concerned. Is this normal to be sent home instead of the hospital?
I was diagnosed with prominent bowl. I am having no symptoms except my side nerve, pain in back and pain where my overt is on the left side. Am I gonna have to have surgery?

When my sister died of a brain aneurysm in 2019 due to a poor diet & smoking, I stopped eating meat, fast food and caffeine and lo & behold, I've HAD NO GASTROINTESTINAL DISTRESS SINCE. Now with an ileostomy most people can't handle raw cashews (I eat those to get SOME protein) and the fruit and spices that I can tolerate. I am on a low protein, high carb, high potassium diet. And yes, with only 3 feet & 9 inches of bowel, I only weigh 100 pounds BUT .I box, swim, go biking and run the treadmill 6 days a week & my quality of life is beautiful! My PCP says I am his first & only short gut patient and he wonders if I've hit on something. Best wishes for you & your wee one 🙏✌️
So sorry for your baby (and for you.) 😪Fiber is the worst, yet many a dr will say eat fiber, as they don't understand this condition. If the bowel is constricted by adhesions, the softest of foods only and plenty of fluids. No one with adhesions should be taking in fibrous foods.
Over the last 5 years, I've had over 30 Bowel obstructions, I'm 71 and at one stage it was happening nearly every 10 weeks..Firstly I had a 360 Spinal fusion which was the start of my bowel blockages. It left me heavily scared Front & Back They first found that I had bowel cancer so after a resection of the bowel some years ago I was put on a colostomy bag for six months. Then more operations,& more adhesions then 5 years later liver cancer and so more adhesion and more bowel blockages..So the more operations I had made it worse. Only 2 weeks ago was my last stay with more B/O but this time I was told if I had to have the opp I would be a lot worse off and if I didn't have the opp they said they would make me comfortable until I pass away.
I’ve had complete sbo twice and multiple partial sbos, due to removal of my left ovary in 2005. This caused adhesions. The first time they treated it conservatively. NPO, NG tube, pain meds and iv fluids. They did this for 4 days. They finally decided to go in laparoscopicly to see what was going on. Turns out the adhesion had surrounded the small bowel and cinched it off. Once that was released I made a full recovery. Since then I’ve had partials , which I treat at home, npo with liquid only. It can take from a day to a couple of days to resolve. I know that is I begin to vomit or the pain is to much to handle, I go to the hospital. Can you do a video on partial obstruction. Great video, thanks!
Had first flare of SBO 12 yrs ago. Thought I ate bad food. Severe pain for hrs, followed by extreme vomiting 🤢. 10 went to a 2 . About 1 yr later identical repeat. Thought bad food. About 3 yrs later , repeat.. wow. About 7 yrs after the first flare,I decided to see my doc. Began having flares 3 to 4 times a yr. Finally went to the ER Jan 2024..Diagnosis, SBO. Ng tube and rest. 3 more flares later, went back to ER. NG and rest. Treatment was to schedule surgery. Had surgery September 2024. Diagnosis, SBO due to a twisted and folded bowel. Repairs were done without resection..😊 9 weeks after surgery,,had a severe flare. Went to ER again. Treatment was NG tube and rest. All and many test and cultures were negative and doctors dont recommend surgery,, only modification of ingested foods. Small portions and add a laxative often. Very healthy and energetic , but reserved on what and how much to eat. Does this all sound familiar to anyone, and wil my SBO ever heal or cease ??
Wife throwing up thick dark green, major pain in abdomen, went to ER and has sbo… started passing gas but no#2.. released after 5 days but still no # 2 .. what should I do ?
Definitely contact the medical and surgical team without hesitation if you’re worried, they know the specifics of your wife’s case and can advise. Good luck I hope she recovers quickly!
Great as usual 👏🏻 Just in case of intussusception specially in pediatrics as it mostly idiopathic and there is no leading point but surely could be , in such cases what is the role of gastrograffin challenge? If it relieves the obstruction, is there need for further imaging to look for (secondary cause) ? as we usually depend only on US to dx intussusception. And if it fails , do we repeat it? Or go for surgery ? Specially if patient condition stable and no complications developed yet. * the most valuable part to me is the scientific approach to the subject with the mention of the recent updated articles about it 👌✨
Intussusception in children is a bit different, if a child is hemodynamically stable without peritonitis a contrast enema is indicated for reduction. If the intussusception is reduced on the first attempt often that child can be discharged from the emergency room following a protocolized recovery. No additional tests necessary unless symptoms return.
I'm 71 and I have intussusception. I guess it's unusual for adults. No lead point. Had to have an ambulance twice in the past 2 years.I was told that there's really nothing to be done for it because surgery would cause scar tissue. I just tell myself that it'll never happen again.
1 day my mom, girlfriend & i were cleaning, plus we had a house full of ants, we spray bleach everywhere. I ingested something, probably bleach on food, was in the most PAIN ever had in my LIFE, 4 hours later, my small Bowel Twisted. I had a EMERGENCY OPERATION 2HOURS LATER 😮. Surgeon Cut 2ft of small intestine. I was so healthy before then, ate fruit vegetables, even jogged. Now I can't do as Much, it changed my Life. I digested food too quick now. If I drink coffee or eat sweetes like Donuts 🍩 or 🍰 cake, Im in the bathroom in 5 minutes. Are any of these new complications happening to any of you guys? How do you handle it? And how can we reverse it or make it better?
Man, I worked out 7 days a week (two workouts 6 days a week) and drank a minimum of half a gallon a day... I still ended up jacked up. I'm 9 days into an ostomy surgery. This gas shit is ass, bro. I haven't had a good night's sleep since this operation. I learned I had intussusception. At friggin 34. I'm trying to get as much advice as possible. The gas is way worse than all the staples in my abdomen. Sometimes I still feel like it's going to come out the back end. I need this to get better fast.
my life changed after surgeon nicked my pancreas by mistake ended up hospitalized several months and two major surgeries after witch left me with sbo for the past twelve years happening several times a year always ending up in excruciating pain for hours rolling on my living room floor crying out for help from my dedicated husband tapping and rubbing my back to help release gas pockets and help me to vomit which is very hard because i dont have a reflexx to do so easily like most people many times end up in er. until it revolves on its own. then go on a liquid diet for several days and lose ten pounds easy and become weak because of no nutrition going in me. then eating baby food just to get by these adhesions strangle my small bowel and they collapsed all because of a surgeon nicked my pancreas by mistake and my life will never be normal again. can't eat raw vegetables or nuts or desserts. I'm damned if I do and damned if I don't. lost sooo much weight in the past ten years 😢and struggle every day. food is my enemy now
Gastrograffin challenge is traumatic, it was put down my ngt and I then vomited for the next 3 hours around my ngt and then had diarrhea that was out of control. Never again, and my surgeon says I may have another SBO. I had surgery 2 months ago to remove adhesions and it took 4.5 hours and then had 30 feet of small bowel, my entire small bowel internally herniated which was not expected.
I was admitted with sbo, had the ng tube and i had the g challege. I started to pass gas and have some diarrhea, was able eat. So they realeased me. Well im the second day out and im starting to have trouble passing gas again and only having small bowel movements. No pain or bloating. Should i go back to the hospital to get checked out or are my bowels just slow to move again?
My stomach recently felt like it was gonna EXPLODE. I was going to the bathroom the whole time but it was small amounts & I had gas tho not a lot the entire time. I started vomiting which is something I rarely do so I immediately went to the hosp. I was almost sepsis when I got there. NG tube was HUGE for my body frame & caused MEGA issues & after suffering 9hrs I made em remove it. They wanted 2 do surgery the following morning but I asked em to wait. I’m glad they waited but I’m still struggling to heal some but thx to new meds I’m getting there. I have scar tissue but I nvr had surgery so it doesn’t make sense to me. Something must’ve happened over the yrs of my doc’s not listening to me telling em tho I go a small amounts throughout the day I knew I wasn’t going nearly what I was eating. Now they r listening 😂🤣😂🤣

What is npo?
I had a sbo in Jan 2023, and today March 2024 have had 6 surgeries with many complications
Awesome content… very helpful

Repairs were done without resection..😊
9 weeks after surgery,,had a severe flare.
I have not gone in 7 days I have wind feeling sick and pain In right lower and it feels like my belly feels blotted but scared to take anything
Maybe a colonic irrigation , gravity fed water only .
Please why don’t we add analgesics in conservative management? With NG decompression etc
Great as usual 👏🏻
Just in case of intussusception specially in pediatrics as it mostly idiopathic and there is no leading point but surely could be , in such cases what is the role of gastrograffin challenge?
Much Thanks for the clear explanation as usual 🙏🏻
I'm 71 and I have intussusception. I guess it's unusual for adults. No lead point. Had to have an ambulance twice in the past 2 years.I was told that there's really nothing to be done for it because surgery would cause scar tissue. I just tell myself that it'll never happen again.
Do you answer a few questions for people. I’m not asking for diagnosis. Just a few questions? After treatment questions.
Happy to answer “general” questions, just can’t answer questions about care specific to a person that’s for their doctor or healthcare professional
Awesome video, keep it up I’ll be looking forward to more surgical cases 👏🏼
Excellent, pumped you enjoyed it!
I'm damned if I do and damned if I don't. lost sooo much weight in the past ten years 😢and struggle every day. food is my enemy now
Good job
Thanks so much Jimmi! Glad you enjoyed it!
Does laproscopic inguinal hernia repair give you a big risk for sbo?
I have the same question
@@lunarlake1 Both open and laproscopic have the sbo & twisted intestinal risk What I want to know is the laproscopic a greater risk?
@@bluewave7120i had laparoscopically appendectomy 7 years ago.. do u think that will cause me obstruction after 7 years???
Thanks Doc.🙏
Absolutely thank you!
Same here
Do all sob need sugery
Absolutely not, many small bowel obstructions will resolve with bowel rest or a gastrograffin challenge
Sir how to do it.
I dont poop for 3 weeks .till it goes to critical mass then repeat....docs are no help.
Sorry to hear that, I’ll put out a video on constiptation and help explain it a bit.
