The one error, IMO: Not learning the lesson of the Sunshine Skyway Bridge by building robust dolphins to protect the pilings.
I said that the day it happened. But boy did they protect those electric poles. But none for the bridge.
This bridge was built three years before the Skyway Bridge accident. Lessons were learned and now all new Bridges need better protection but they never went back and retrofitted old bridges.

There was no error and there was no fault. It was just an accident. Accidents happen.
That doesn't justify the punishment of innocent people simply so people can feel good by being immortal with an unjust call for vengeance against people that made a mistake.
There are no amount of rules, laws, or regulations that will prevent an accident from happening. That's why its an accident.

My question was why was that in the title of the video, but then nowhere does he actually say what he thought the one error was, so the title did not seem aligned to the video. But overall, yes an accident. And like with airline crashes they will no doubt find multiple things that contributed, rarely just one thing.

That answer is what I was waiting for as well. I'm not an engineer by no means, but as a Navy Seabee, I have built bridges. My overall question, is what caused the bridge to collapse by the ship hitting that one section.

@@SSK1977FEB turn of ship was caused by "propeller effect", the term for a propeller's tendency to rotate about a vertical axis (also known as yaw motion).
After power eas restored they started the main engine (used for ship propulsion) on astern. The MV Dali engine is reversible, so when they put the ship's engine on astern the propeller start to turn clockwise pushing ship's hull to starboard. When ship is on astern, the turning lever is too small for the rudder to actually turn the vessel, therefore the rudder effect on ship is virtually ineffective. This behavior of the ship had to be known by the captain.
That's why I always said is HUMAN ERROR caused by incompetency onboard.
Onboard MV Dali the mistakes and lavk of professionalism piled up with this result.
"Bad fuel" is a lame excuse from them.
Delay in decision, poor crisis management, low professionalism, lack of training, lack of knowledge and lack of seamanship.

So you are going to have massive swirling water currents that will affect the direction a ship is heading. On the ship they would be making constant steering corrections to compensate but they lost all that ability with the power failure

Been watching many videos about this incident. Some are very good and together they give a good clear overall view about all aspects of this sad disaster.

When building make each section independent of the other, not dependent.
If you look at the footage being aired you will see the front the ship you see about 20 ft section missing this was taken out by the bridge its self so you can figure the bridge shifted half that also keep in mind this was a toll bridge so it might not be paid for yet

Tolls are probably siphoned off and put into the general fund, not used for bridge maintenance and improvements.

Even a junior officer learn in maritime school that ship's anchor is designed to hold that ship and even to stop it at a speed of 8-10 knts.
If both anchor would have been dropped at first black-out, the collision would have beed avoided.

That's been pointed out back in 2001 by a Canadian. Nobody will help the US under any circumstances, but we are REQUIRED to offer help in ALL circumstances or else. Granted, that this may be an overstatement, but not much over.
American administrations are reluctant to allow foreign support in US disaster situations since this would suggest weakness or incompetence. Habitually repulsed, foreign governments tend to wait to be invited.
FEMA's response to Hurricane Katrina is unfortunately still seen as fairly typical of the USA's response to natural disasters and deteriorating infrastructure makes man-made disasters increasingly frequent, due to tax-cutting attitudes. Cost-cutting decisions on the safety construction aspects of the bridge in question here reveal the attitude to be "penny-wise and pound-foolish".

I don't think anyhone so far including me (who made the first comments), has said what caused the accident. However, on TV shorthly after the disaster, I recall it being said that the ship had its electronics serviced just prior to departure.

So there are two "causes" of note: 1) the cause of the ship's loss of power, and 2) the cause of the bridge collapse. The investigation will focus mainly on the first one (the "root" cause), and the video did not speculate on that aspect. The cause of the bridge collapse was obviously the allision of the MV Dali against a main structural support; there is no investigation needed. No bridge is designed to withstand that type of impact on the main structure, hence the deployment of protective dolphins etc. on other bridges.

It took 10 years for CALTRANS in CA to remove and replace the east span of the San Francisco Bay Bridge which was damaged in an earthquake and by a cargo ship colliding with the bridge. The west span of the bridge was also supposed to be replaced but hasn’t been.
So it’s reasonable to expect that removing the wreckage of the Key bridge and constructing a new Key bridge will also take at least 10 years.

Sounds like consideration of another tunnel should be made. I believe the current tunnel further inland was made by sinking prefab components. This sounds like a job for The Boring Company.
Must have been avoided the collision. Those Tugboats are so essential for Manuevering especially passing to the bridge. A different body that are responsible for the Safety routes,before and after passing of the bridge. The Ship was going in a wrong direction route to collided towards the Leg posts concrete foundation of the bridge. Four minutes,before the collision is not enough to correct the right lane routes,as even though whether there is or no engine failure,due to super long of the ship,big loaded,continuous drifting of the ship (8 knots) Different to a Corvettes that can easily change its course. I heard a U.S. Aircraft Carrier do passing the bridge for only 5 knots. In the first place from the start of navigation,when departed from the port of origin,if not use radar,the compass must set to desired degree course. Where in the Center of the span,arc shape of the structural,in that blinkering light. If not there must be two(2) pilots Fwd & Aft,handheld radio directing what’s happening,the position of the ship,towards the blinkering light,Center of the span. Especially in the Fwd pilot. There is something happened engine failure or drifting,Ship is still in safe position. But already 4 minutes Ship is still,not in a proper location,heading towards the Leg posts concrete foundation. The Ist time i heard they were 33crews & 2 pilots,but the 2nd time 22crews & 2 pilots. Is there an undocumented passengers?
Your math doesn’t add up. According to you, there were more then 8 and how was someone rescued immediately if all the emergency services to several minutes to get there

There is no question why the bridge fell. The real question is why it collapsed so rapidly like a deck of cards, especially the section beyond the second pylon whilst the two ramps remained unaffected.
The lattice truss is one continuous structure. Removal of any one support exceeds the strength of the other, interlocked, members. You can see the lattice snapping at each support leg as their tensile strength is rapidly exceeded.
The ramps that remained are not part of the truss section but can be considered single span bridges in their own right. Although I haven’t seen any shots of end of the remaining ramps, I expect virtually no damage with the truss section parting away at the expansion joints.

Very few ports will allow this poorly maintained tin can in their ports. Have a look at who owns her, who leases her, where she is registered and who has the right insurance on her.
There were a vastly insufficient number of dolphins/bollards to protect the bridge. The fact the Key Bridge was "grandfathered" in after the Skyway bridge disaster in 1980 was a huge mistake. The act of grandfathering in buildings, bridges, etc. when it comes to safety is insane. And thus the insanity we observed when the Dali struct the Key.
Retrofits are expensive. It ain't broke, don't fix it.

Is nobody noticing the incendiary and smoke where the bridge separates as it falls? Not the flashing lights. Watch fullscreen, they are easy to see.
@didgeridoo Likely cause was the severing of the power cables which run on the bridge for lights, etc. Also, rending metal can release a lot of heat when violently severed/ No, there were no bombs or other destructive devices on the bridge. Think about it... what you saw happened AFTER the ship hit the support.

I wouldn't be surprised if the ship lost power because of neglect. Maintenance and repairs delayed or skipped entirely due to cost. Management decisions that resulted managers getting bonuses and promotions for cost savings, for keeping the ship in service when it should have been taken out of service for repairs and maintenance. As an industrial mechanic, I've had to watch the machinery destroying itself because the production manager demanded production on scheduled maintenance days, As lead mechanic, excessive downtime due to major breakdowns were blamed on me. Fortunately, the production plant I worked for didn't crash into anything or kill anyone. Of course, the managers that made the critical decisions resulting in this accident hold themselves blameless. Technicians have probably been disciplined or fired.

Well, the guarding around the bridge was built years ago when the ships were not as wide. If you notice the front of the ship is sloped and very wide at the top and narrow near the water.
The problems continue with the steel for a new bridge needing to be specially manufactured, and maybe a lot of the skilled bridge builders are already in work.
Wasn't the DALI in that port before? Was this the first time that ship ever entered that port and was unfamiliar?
I would like to know if the ship deployed the starboard anchor or the port anchor. The course line of the ship was naturally to the starboard side of the bridge channel (like staying in your right lane on the highway) and thus if any effort to veer the ship course was to be made with the anchor, it should have been the port side anchor that was deployed. Having seen some ostensibly accurate AIS course information, the ship apparently began to veer to starboard towards the bridge pylon not too long before the collision. I wonder if the starboard anchor was deployed causing the starboard veer, whereas a port anchor would have tended to steer the ship more towards the port side and perhaps cleared the abutments. Hindsight is always 20/20. I feel badly for all concerned, especially the families of the workers killed.
The question was asked in the video: "Who is going to pay for all the damage?" - The answer, as always, is the taxpayer!
On each end of the bridge were found they have rain guards. It goes across the street on each side of the bridge so the water can flow off the main metal structures into the semen down to a drain pipe yeah that’s why the bridge fell so quickly go on Google maps you will see what I’m talking about.

If no one did anything when they lost power the freighter would have moved out without hitting anything. It was when they got power that all hell started the freighter toward the bridge pilling. This was not a mistake. If everyone was doing their job as they should this should not of happened. Just as it has not happen for over 50 years. This was not a bridge problem.
They dropped the portside anchor. Your video shows them dropping the starboard anchor. Attention to detail clarifies, rather than confusing you viewers.

They should have some kind of " Hard or Soft Rubber bumpers" whatever they think would be better in the middle of these "Support Truss" Base from the outside where these Ships go thru mainly in the middle since this is the deepest part & where the Biggest Ships pass thru. The nxt Key Bridge they build, hopefully will have some. If they would have had some from the beginning maybe this could've been avoided.

Why did the Pilots not ordering both anchors out, at the first black out? (6 minuts left before collision with the Bridge) Both anchors at the bottom of the channel with 4 - 6 lengths of heavy chain in the water might have stopped the ship before ramming the bridge. A black out is always a warning saying, something isn`t working correct and it´s better trying to stop the vessel before it´s ramming a bridge etc. When they had power again, for some seconds the ME should have been sat to going astern.
Dropping anchors does not stop a ship any faster than dumping the air on a train or locking up the brakes in a land vehicle. Dropping the port anchor was a desperate attempt to steer the ship to port. Alas, even if it had been effective, it was attempted too late.
the square ring around the bridge support survived. But, the bow of the dali reached over and past that ring and struck the support. A bridge can be effectively protected from a strike and collapse by ANY SHIP!!!
I drilled thousands of holes for rebar on the Knight Street Bridge in Vancouver. The columns were connected with sheer walls and the bases were fortified with plinths. I thought it was overkill and could it survive a magnitude 7 earth quake? I guess it's better to be safe than sorry.

The comments about more protection for supports is sound. Preventing impact is the issue. You can't prevent a collapse like that as that would be expected. Protect the support system. The recovery operation is amazing. And thank goodness for the early warning that saved lives.

Yeah, I heard a rumble in at like 130 almost 120 so I’m in the morning live in Curtis Bay Cypress St., Avenue. The reason it fell so quickly is they got rain guards up on the bridge and it’s like 2 to 3 feet down with no semen for rain coming off the metal until the bridge. If you go on Google maps you will see what I’m talking about, the metal beans on the ground where the rain goes into goes across the road that’s where the bridge claps from if you go on the map, you will see that rain guard going cross it that’s why it fell so easily
So what happened on the ship to cause the loss of power?
Dropped one anchor says it all! Caused the hard right hand turn, if it dragged the ground. Easily to turn a slow moving vessel than to stop it 🧐
Yeah, its super easy to manually turn a rudder that weighs several tons by hand even though that's literally impossible and utterly reliant on a powered system to turn such a heavy component.
Not to mention that without the propellers moving water past the rudder the rudder can't stear the ship anyway, you know, the proppellers without power.
But your correct, they should have just turned the rudder and spun the 10 ton proppellers by hand because of how easy and feasible that is in imagination land.
There are only three questions that matter, and this video did not mention or address any of them. 1) Exactly why did the ship lose power three times? 2) Did the ship have rudder control even when the power was down? 3) What made the ship take a half-right turn into the bridge support, just before the ship reached the bridge?

Sure are a lot of experts here. Accidents or tragedy's like this are rarely if ever caused by a single error. Usually a string of much smaller errors that have piled up
I heard the police communication there was no one on the other side after the bridge fell only one side they requested local police after the fact .

WHY should the TAX PAYER pay for it?
Because any litigation is going to be wrapped up in court for years at minimum, and the need of people who use that route for a replacement bridge as quickly as possible VASTLY outweighs your need to whine and shit and bitch and piss your panties on the internet about it. It's literally the federal government's job to build and maintain infrastructure like this (and repair or replace it when necessary).

The error was operator telling captain to leave Baltimore port with major safety violations. This ship was experiencing constant blackouts for two days prior. No accident but criminal negligence.
The 1851 law orders a fine of USD 1000,-- . How much is that today ? USD 40.000,-- ?
Loosing power is the cause of the problems
Are you saying the power was not tight enough? "Lose" and "Loose" are two different words, two different meanings.
MOSTLY FALSE INFORMATION. Here's why. The ship lost electrical power, NOT engine power and not rudder control. There are 3 diesel powered electrical generators to provide the vital electrical needs of a 100,000 ton vessel. These generators power bow thrusters which assist in docking, but not while underway. With only 4000 feet remaining, the ship went to full reverse. This is proven because the speed went from 8k to 7k and a large smoke plume can be seen. No captain would go to reverse because full reverse CAUSES a swing to starboard. The only possible answer is that a crewmember in the engine control room made the engine go to full reverse at the exact worst time. The rudder on a large vessel does not swing out of control with the loss of either electrical or engine power. The rudder has nothing to do with the turn.
I do not understand. The video shows the ship going straight by the bridge abutment. Then it goes dark, and it makes a hard right. Directly towards the abutment. If people cannot SEE this, they are blind.

I'm having trouble buying this explanation. I've seen at least two videos showing the impact of the ship and the resultant collapse of the bridge. At least one of these videos has been doctored. One video shows an 18-wheeler traveling across the bridge and it gets about 80% of the way across before the ship strikes the bridge. At the moment of impact, the 18-wheeler ceases to exist. It doesn't fall into the water. It doesn't continue on its way across the bridge. It simply ceases to exist. The other video doesn't show a single vehicle still remaining on the bridge at the time of impact. Both of these videos cannot be full, actual accounts of what took place. So, why the doctored video(s)?
