Action Man HQ has a caption on one of the pictures that the canteen and belt were added for effect. I think this was to explain that it was not part of the original set.

In the US the first issue Medic set was issued in the GI Joe Action Marine range as a A Marine Medic, hence the camo fatigues. In the UK we didn't have the Action Marine range for Action Man so the Medic set was issued in the Action Soldier range where the fatigues were the standard olive green. The stretcher and the flag you have come from the second issue Action Man Medic and despite what the photo your book shows, that is the incorrect flag for the first issue Medic.
As your insert mentions, the flag should match the red cross on the armbands and helmet, i.e. with a white square in the middle of the cross. Your medic bag is the correct one for the first issue. The belt and the first aid pouch were not included in any first issue Medic sets/accessory cards. The standard khaki web belt and the first aid pouch appeared in other GI Joe/Action Man sets and small accessory cards as these belts and first aid pouches were standard kit for all army personnel.
I have the same NG Taylor book and it's a wonderful picture book with great illustrations, but it is riddled with inaccuracies so do not take everything in it as gospel. As a reference guide it's definitely lacking. The three volume Alan Hall set of books and 'Action Man The Real Story' by Kevin King are far more accurate reference sources.
