      Like many have said before, I think the Cyborg Santos caved in head was the worst injury but... Yang Jian Bing in One Championship did die from something to do with his weight cut so if we count that as an "injury" I'll go with that.

    I like his calm voice and it just makes normally gruesome stuff a lot more watchable. It's a weirdly calming and scientific approach to some of the worse injuries I've seen in sports.

      I'm honestly seriously surprised he could even see that in real time. It's such a subtle thing when viewed in real time. Obviously slowed down it's a bit more obvious, but mad props for being able to see it as it happens (and being right about what he saw as well!)

    Joana's hematoma after fighting Weili was pretty gnarly. Robbie Lawler's lip in the second fight against Rory and Overeem's lip after getting KO'd by Jairzinho were also pretty bad.

    I still find that Tim Sylvia one incredible. I've continued to do things after some pretty bad injuries and understand how potent adrenaline can be but to not only continue (and _want_ to continue) but also have the presence of mind to effectively deceive the ringside physician after that injury and knowing what that injury means in terms of his performance is astounding. You have to admire his tenacity/ bravery/ stupidity/ stoicism after that!

    I watched and own the DVD of UFC 48 and, regardless of what Tim Silvia said after the fight was stopped, Frank Mir did in fact break his arm. Frank Mir's response to Silvia's remarks was epic

    I was at UFC 48 at Mandalay Bay sitting 8 rows from the ring when that arm snapped. It sound like someone breaking a mop handle. Even with all the cheering I could hear it Loud and clear..

    The injuries are always shocking but what always stuns me is how tough these fighters are. They seem impervious to pain.

      High pain threshold is pretty much just practise. Heat of the moment too like getting punched cleanly one or two times the damage wont even register instantly. Happened to me when i got into a fight, i got whipped with two straight jabs and i didnt even flinch but a couple of seconds more then boom i just got knocked out for a couple of seconds. When my senses got back i was already on the ground getting my arse whooped by 5. Yup was an idiot charging 5 men lmao.
      If you watch boxing and see those boxers take those scary jabs directly multiple times is no joke. Without practise those jabs WILL knock you out. So its all in the training minimizing damage by predicting such shots and responding accurately. Boxing is scary, MMA is scarier when you ask some but boxing is like brutal.
      Imagine facing Manny Pacquiao and taking clean hits like bruh god dayum.

      @@m0n4rch911 I am a huge sports fan including MMA. I do completely understand pain thresholds. A childhood illness prevented me from participating in sports but my family always says I had the heart of an athlete and with my high pain tolerance due to 25 surgeries before turning 18, all my years of physical therapy, and just getting through life with braces, on crutches, using a wheelchair, wherever I was at in any particular time in my life, I would have been great at sports.
      So I never got to play but have always been a dedicated very appreciate fan. I have loved football since a young girl watching with my dad who taught me the game-I’m a Seahawk fan. I’m a fan of college football & basketball, baseball-Cubs fan-hockey, tennis, track events-since I can’t run I’m fascinated by all those athletes and events-all the skiing competitions, love those kids on snowboards, and the MMA. I will say I feel MMA injuries are unique and can be especially vicious as the men and women are incredible athletes never wanting to give up. As you said being in the heat of the moment, they are focused on the fight not what is hurting at that moment.
      Thanks for your reply. I was laughing at your fight and that I do understand. My dad was a Marine, 30 years in the service, and he taught me to fight in spite of my limitations. Being a military brat moving almost every year most often going to schools with civilian kids, my siblings and I had to know how to defend ourselves. Going home crying or whining about someone pushing us around or picking on us WAS NOT AN OPTION. Love your moniker...I mean who doesn’t love bacon. Lol

    In the slow motion of the arm break you can almost see both bones break one right after the other. If you watch Frank's glove you can see the snap through his gloves due to the lights reflecting off the glove.

    There was one from way back. I can’t remember who it was but they tore multiple ligaments in their knee and when the stood up off the stool their knee was visibly hyperextended and they couldn’t walk! Ref called it off immediately

    Mir was wearing a steel muay thai style cup. Tim was defending the arm bar exactly how your taught, clear the elbow above the croch, Mir is just a beast, very strong + that cup creating a perfect pivot point to break the forearm. Props to Herb smoking Dean for seeing that and stopping the fight.

    Dr.bryan my knee moves to the side instead of up and down when I put too much pressure the kneecap dislocates what do you think is the cause

      I had a dislocation of the kneecap. I was told. The tendons get stretched that hold the cap in place.

    Hey doc, can you do a video or a discussion on the health considerations of weight cutting in MMA? The vid of a female fighter passing out during the weigh ins a few weeks back really makes you wonder what kind of pain and trauma they are putting themselves in to make a certain weight

      And since you replied, I will take the opportunity to answer your question on which injury I thought was the worst. At least visually.
      #1 - UFC 168: Anderson Silva's Spaghetti Fiesta Leg-Break.
      #2 - UFC 261: Pretty recently, Chris Weidman, unfortunately, suffered the same outcome as Anderson Silva suffered back in UFC 168. (This is the very same man that checked the kick Anderson threw, resulting in my '#1')
      #3 - I wiggle between any orbital fracture (Only because I can only imagine the pain is probably beyond 'pain' lol) and Curtis 'Razor' Blaydes elbow against Sjamil Abdurachimov's nose. Urgghhhhhh 😩.

    I would know about the radius and olna as I fractured them both and needed surgery, I can't properly twist my wrist anymore but that looked like a greenstick fracture on the radius and maybe a transversal fracture on the olna bone.

    Can you explain why when you have a broken nose and you blow it a fluid goes to your eye and closes it completely. We've seen many fighters make that mistake and I was wondering why is it happening.

    9:07 Tim is still having complications from that injury today, the metal pins they put in got infected and moved around and he racked up some big medical debt. It’s fucked the ufc doesn’t give their fighters Health insurance, especially from injuries that happened in the octagon. Tim’s wife set up a go fund me if anyone wants to help out an old champ, it’s close to the goal.

    hmmm i think during that fight at some point when they were up against the fence his toe was caught in the cage and thats what created the initial fracture and then when he put body weight on it later thats where we saw the toe almost completely break off

    I was playin basketball with a friend in gym and I checked up the ball to him and as it hit the floor some kid ran between us and stepped on the ball he ate the ground and when he turned around and we saw his bone stickin out his arm. He got surgery but even after it his arm is still bent real bad
