As a victim of child abuse, the line "I have found some kind of temporary sanity this shit piss and cum on my hands" fucking breaks me. This is about a person that is in a much deeper rabbit hole than I am, I've never had the urge to hurt anyone. But being capable of having enough parallels to relate allows me to see a horrifying image, and the knowledge that the only thing that has avoided me, or you for that matter, from ending up like that is cause we've been priveliged by our surroundings is terrifying to say the least. Holy shit these lyrics are so well written.

Adam is playing a 6 string guitar, so what does he do to get that low B note? Does he string his guitar with a low B string on the low E slot? What does he do there?

That guitar, specifically, is setup for the low E (which he typically has in D anyway) to be tuned to a low B. I believe he uses 10-52 Skinny Top/Heavy Bottom gauge strings.
