I guess to me when I hear "Spaghetti Fantasy" my mind goes more to Spaghetti Western so I would expect something more built off the tropes in the western genre, but with a coat of fantasy paint over it. So basically the Sword Coast.

Well, since spaghetti western is western infused with an Italian flair, spaghetti fantasy is fantasy infused with an Italian flair. No western here.

I think the inspiration for the material comes from a popular Italian movie of the 70s, L'Armata Brancaleone.
this is one of the sources but far from the only one. Pinocchio, the Tale of Tales, and Italo Calvin's books are prominent, too.
