Unless I missed it, you never mentioned that PCP & Ketamine are both dopamine re-uptake inhibitors as well as being NMDA antagonists... And, why no mention of dextromethorphan when talking about NMDA antagonists?
i know that you have a core audience but this is my first time watching and i like the video style and content but i need a breakdown of the textbook words pls!
I don't know what ketamine your talking but all ketamine settings are very calming and a very nice experience you must be getting it off the street and not from a professional in a clinic and a dirty drug the k hole is very nice and a great experience
Any good videos explaining how Methadone works on the brain ? I heard it’s an antagonist for the nmda receptors as well and fully synthetic unlike the other painkillers. I’ve been on methadone for about two years and close to being off it now by tapering 1mg per week. Methadone does not cause a euphoric high compared to Norco or oxycodone not even close to the same feeling but yet it’s a lot stronger and way more powerful overall in my opinion and a lot more sedating in the beginning of using it to get off other types of opioids. The two things good about methadone for me atleast is that it 100% fully took away my desires to ever use another opiate ever again by just making me not have even the slightest cravings at all thank god haha and also it lasts me a good 18-24 hours of relief and gives me the clearest mind state ever and not cloudy headed or poor judgement compared to norco for example…. Other pain meds give me extreme energy and make me very hyper and extremely euphoric but methadone doesn’t or ever has given me euphoria compared to other opioids at all. The bad part about methadone is that it’s very hard to “Kick” and last so long that your withdrawals will last a lot longer than all the other pain killers out there and it’s worse than kicking anything else in my opinion including alcohol because the intense hallucinations that come with it and severe dehydration and projectile vomiting and diarrhea are no joke and for me I get restless arms and legs so bad that I’ve had to have people hold my arms behind my back just to try and get some rest which is impossible while kicking methadone cold turkey even at low doses you will suffer greatly ! The other bad part of getting off methadone is the intensity of “Hot and cold” flashes and shivering cold sweats while your entire body is burning up worse than any fever I’ve ever felt before all while having delusions and extreme hallucinations that are not like lsd or shrooms whatsoever. Methadone is a whole different type of “Monster” when compared to traditional painkillers especially the faster acting strong ones. It’s so unique and I’m thinking it’s due to being fully synthetic and has antagonist NMDA properties unlike norco and OxyContin etc…. I’ve heard many addicts say they’d rather quit heroin cold turkey than to quit methadone cold turkey and that’s pretty damn scary to hear coming from the horses mouth haha the reason I had to join the methadone treatment clinic is due to having a severe knee injury many years ago that no doctors were able to “Figure out” how to fix with surgery so instead of fixing the issue in the very beginning they just kept writing me scripts like Candy and I got very addicted to the tramadol and norco/Percocets and than started taking xanax and ativan as well to help ease myself whenever I ran out of my pain meds early because of how much pain I was in 24/7 and it was to this day the most intense pain I have ever felt in my life compared to all my other injuries over the years which were all pretty bad as well but nothing like having a fully torn in half ACL and fat pad being crushed under my knee cap pinched tightly as well. Such a unique knee injury is what they said and I wasn’t able to walk the entire time without a straight knee brace from my hip to my ankle it had to be locked in place for about 7 years straight with no medical help and suffering. So now being in the methadone program for about two years has helped me reach my peak dose and now over halfway done weaning down. My knee was finally fixed about two months before I signed up for the clinic because one my knee was healed finally I didn’t need the pain meds ever again but needed help getting off them so I joined the clinic and now I’m very close to being fully sober and will be done between January-March of next year depending on how fast I wean down which I’m gonna go from 1mg a week to 2mg per week to ween down a lot faster due to the way the world has been going lately lol
I don't understand where the "violent" rep of PCP comes from. You explained the insensitivity to pain in a way that I am able to understand but the PCP violent response you mentioned isn't explained at all. Or why ketamine doesn't have this effect.
LyleStyle Not all people on PCP are violent. I’d probably say that most are not violent. Ketamine can have this effect. Particularly , as people come out of the effects of ketamine. This is known as an emergence reaction. Something like 10-15% of patients can develop this reaction where they will be confused, agitated and potentially violent. This is why it’s important to have a person in a quiet, calm environment as they emerge and even consider benzos to attenuate the response.
while i do experience a somewhat knowing (omniscient) sort of feeling that my mind is gone . what is that? it's preceded by a feeling of myself being selected specifically by some greater force . i feel like i get amnesia then come back to my own life . i can look at a person and forget what language is . its like i forget what people are and what everything is and i can see it from the perspective of someone who sees human life for the 1st time but just from the pov of the inside of the shell ive grown to know as myself. does any of this makes sense. i feel like i die and come back to life or have died and came back to life millions of times and remember myselves . i feel reborn and a prophecy is fulfilled to my ancestors and im proud of who i am to an infinite amount and then in as flash i feel normal and like it was all an illusion . then i feel stuck sometimes like i can't remember what to do then worried I'll stay like that but can't do anything mentally ro counteract it . then i snap out of it then coming back to myself makes me feel euphoric. I'd really like to research what I'm going through it's such a weird trip. microdosing is possible but I'd like what's going on in my mind to be broken down to me in layman's terms. i can be around people ive known forever then in a a flash they look unfamiliar and i don't understand what that are saying. i see the world like fragmenting and the earth dividing and breaking apart just for me like whatever is controlling the universe does it for me making me feel like all my ancestors and people i know are witnessing it Happen to me like a spotlight on me and im like screaming wtffff !!!! it's crazy like it's really happening and it is . then next thing i know I'll be in custody in the middle of the street and it's not happening it's just me thinking it. but its still like happening in my mind but it's dangerous because it's then involving cops and people think you're crazy . what's the purpose of an agent like this ? to feel so good and intense and narcissisticly rewarded only to be so crazy looking that i get killed by a cop one day im lucky i haven't been killed because to them im looking crazy but I'm just feeling like I'm being selected by God to be aware of who i am as a person and my duty to my ancestors and children it's unfair. somebody help
I probably should read the comments but anyway.. What's the dosage for...clinical use, say, a dislocated shoulder.. Cause for me as a "recreational user", the k-hole is a desirable experience, but I'm thinking, if I'm a tea total sober person, ending up in hospital and they shoot me up with ketamine and have to spend 20 minutes in this completely indescribable and previously unknown state of mind... that must be pretty terrible. And I'm wondering, is the emergence reaction.. like going from unconsciousness to the confusion of the k-hole? Or from the k-hole to a just wobbly state that low amounts of ketamine cause?
I know this was a while back but it all depends on your tolerance really, with no tolerance even 100mg intranasally can send you into a long and deep K hole, I've been (ab)using it recreationally for about 6 months, tolerance builds quick so if I want to use it for pain management I only need maybe 150mg and then redose 50mg or so every hour, helped tremendously when I got a massive gum infection and they had to drill my teeth and all this bad stuff, with no tolerance it would probably be like 50mg and then wait a few hours and redose if necessary
While i will say that intoxication is not the intent . I'd really like to expand on the positive psychedelic effects. Or is it purely a Euphoric addictive response that im having ?
This was so damn interesting thank you! My interest is in opioids for pain as I firmly believe they are necessary for many myself included, but I love the pharmacokinetics of all the psychoactive compounds, so much so that I've decided to pursue nurse anesthesia. Also as a note if I'm not mistaken isn't ketamine C3?
What positives can be taken from recreational or therapeutic usage ? I feel that this agent has psychologically , emotionally at least artistically and somewhat financially helped me in ways that can't be seen to the observer. What is the original intent of its creation ? Why do i feel so connected to the origin of man , space and time and everyone's journey throughout the lifespan of the universe ? I have been using this agent on and off for 20 years now . I am a successful family man although i have my ups and downs . I'd really liked to get to the root of this agent and intent of its creation . because its helped me with ptsd and creativity within my work and career as an artist. i understand the scary parts of its mis use as well .. if any one reads this , please i just want to have a dialogue . would i be the same person i was 20 years ago before it or did i get stuck because i liked it once ? I've only expanded and multiplied on what i had 20 years ago a million fold. God Bless you all.
Fantastic presentation. Will be safe to treat a patient with a overdose of MDMA with ketamine to antagonized the effect of MDMA? Of course under ventilation or oxygenation control
Thank you. MDMA is a psychostimulant or is also called a substituted amphetamine. Therefore, sympathetic effects can occur with MDMA use. I would probably steer clear from something like ketamine that can increase sympathetic activity in addition to other potential side effects. If a patient who has taken MDMA is highly aggressive or agitated, benzodiazepines would be my first choice. If severe hyperthermia was present, active cooling and dantrolene would be the therapies of choice. All this in addition to supportive care of course.

Yeah, thankfully it's rare for dextromethorphan to be in a pure form. The NMDA receptor is nothing to take lightly. Very peculiar the broad transcendental experience it can cause.

Good supportive care, benzodiazepines for agitation, atropine/glyco for excess salivation, possibly clonidine.
