He has sent me to proclaim liberty to the captives and recovering of sight to the blind, to set at liberty those who are oppressed, 19 to proclaim the year of the Lord's favor.” 20 And he rolled up the scroll and gave it back to the attendant and sat down. And the eyes of all in the synagogue were fixed on him. 21 And he began to say to them, “Today this Scripture has been fulfilled in your hearing.” The year of the Lords favor is the year of Jubilee, was the 50th year where they would rest the Land and free slaves and stuff. We get to live in liberty and rest every day. The Sabbath is no longer a commandment but a daily reality. The passion translation puts it this way: 18-19 “The Spirit of the Lord is upon me, and he has anointed me to be hope for the poor, freedom for the brokenhearted, and new eyes for the blind, and to preach to prisoners, ‘You are set free!’ I have come to share the message of Jubilee, for the time of God’s great acceptance has begun.” 20 After he read this he rolled up the scroll, handed it back to the minister, and sat down. Everyone stared at Jesus, wondering what he was about to say. 21 Then he added, “These Scriptures came true today in front of you.”

I think it's a part of the same song because usually if there's another song in one video they put a slash next to the original song with the name of the second song (example dust/rise on reprise, or sabotage/home)
It makes me think that even when we're striving to be content w/ or without an answer from God, there are times when God responds. Sometimes there's a period of silence like in the Bible. You think that's it from your life but, as the song implies, God responds clearly on his own accord and He encourages you with an 'you're not alone. (x2) Hold on (3x), Kinda like affirmation from the Trinity and a last, 'Hold on to me' from the Father as a whole. To hold on to God in Jesus Christ. Its lovely. :)

Early in the song he said "This hope is my reality" so then he says "I'm not alone, I'm not alone, You're my hope, hold on, hold on, hold on to me!" We don't need to hold onto God, if it was up to us to hold on our grip would slip and we'd let go, but praise God that He holds on to us and He will never let go!
