      @@GoodDrGhave you had patience who have had joint, body and muscle pain? And being really cold? I took my first dose 3 days ago and about 10 hours later I woke up freezing and had terrible body and joint pain. I tossed and turned all night for 2 nights and just felt achy last night I slept a little better but today I feel really tired and my body just hurts all over.
      Also drinking 100oz of water a day. Tried to go for a walk yesterday but my muscles ached to much (only walked 4,000 steps and couldn’t continue.) is this normal and will it go away?? I’m almost too scared to take the second shot.

      @@GoodDrG thank you for responding. No I didn’t start any new exercise routine. The only thing that changed was me getting the injection. Everything else has been the same. I did find on another group that lots of other people are having the same symptoms so that’s interesting. It’s been five days since I had my first injection and I feel normal today so that’s good. I’m scared to take the next injection though because that was awful! The bone pain was the worst.

    Currently on Wegovy 1.7 and I want to back to 1.0. The nausea is awful. As someone who has been on this medication for a few months, I can say that EVERYTHING he is saying is spot on.

    I’ve been on Ozempic for 7 weeks. I eat blueberries, raspberries, almonds, chicken, salads with oil and vinegar dressing, fish, KETO friendly bread and vegan butter. That is it. I’ve lost 27 lbs Within 14 lbs of my goal. Drink lots of water and decaf tea.

      @@jooxumja Great Question! The answer is, it's totally up to you. As of today, there is nothing that says patients cannot stay on it indefinitely if they choose. I personally coach my patients toward making the necessary lifestyle changes to maintain their desired weight so they can come off the meds. This means learning to eat healthier, place themselves in winning situations, and move their bodies more often in order to maintain. In my opinion, the medication can and should be used as a tool to accomplish the goal and not a fix to the bigger problem. I hope that helps!

    You must….. literally must…. eat small meals, hydrate is utmost importance! Healthy weight loss is important! Every physician should make a list of this and hand it out along with the medication. People are so anxious to lose weight they ignore the instructions which is essential to health weightloss! Love this!!! This is Golden!

    I'm 56 lbs down and at my goal weight after 48 weeks on compounded GLP-1s and Wegovy!🎉 Your tips are spot on and I hope more folks realize the importance of movement, hydration, solid nutrition, sleep, etc. For me, compunded Semaglutide then Wegovy simply flipped a switch that made my body start "listening" to all those healthy habits again and made it much easier to stay committed because I no longer had to suffer through chronic inflammation, my hypertension was no longer a hindrance and I could finally see/feel the outward effects!🥰

    Finally someone who understands the physiology of medicines! I’ve been on Mounjaro for just over a year, for diabetes and have had no bad side affects! The weight loss is around 50 lbs. after listening to this, I can say the spicy food has caused a bit of trouble. I appreciate his encouraging exercise and mental presence of your diet. I will be following him and seeing what improvement I can do to this 76 year old body!

    Love this Dr G! I recommend a walking treadmill for the house to avoid being discouraged during the hot summer and rainy season. A 30 minute walk goes by very quickly in front of the tv 😊

      I love having my treadmill at home. I can’t make any excuses. Incorporating incline intervals during my walk has been a game changer in burning more calories.

    I am finishing 100 weeks on several GLP1 meds and everything you are saying is fully accurate. I spent a lot of it avoiding exercise and that was my biggest mistake. Once I hit 150 minutes a week exercising and weight training and focused on protein first for food… things changed for me for the better in a big way. Please listen to this doctor.

      I’m happy to share and appreciate the smart and honest info on your channel. I started a month after it was approved and a year later I was in the hospital with a quintuple bypass. Tirzepatide saved me since it helped increase circulation. Without it, I would have likely had a massive and fatal heart attack with 5 blockages. I’m very grateful that I had lost 65lbs before surgery. I may not have qualified a year earlier.

      @@GoodDrGwhen used efficiently. These medications are amazingly effective. I’m dosing down now and I’m on Trulicity as I slowly phase out or at least go on a very low dose eventually. It did raise my heart rate in my case as my body fat reached 12-13%. I have to continue to dose down accordingly to avoid the increased (+10bpm or so) heart rate, but that seems unique to me in my cardiac recovery.

      I was not able to exercise as I had a bad hip, which had a total hip replacement, which occurred May 6. so still not able to do much but walk a little bit

    I changed my eating habits and the foods I eat. I started in feb bc I became really motivated to lose weight after watching my 600 lb life and so I’ve lost 29 lbs in 3 1/2 months. I started ozempic yesterday and I’m really hoping it will help me lose even more

    I’m on Wegovy- for 5 months and have lost 30 lbs. I was so mad that it wasn’t dropping off quicker- but slow - really slow for me is frustrating- I feel better knowing this is common

      Yikes, 6 lbs a month will thrill me. I tried tirzipatide compounded and lost 4 lbs a month. Didn’t think fast enough, so gave up. Now trying semaglutide compounded, and have realistic expectations now. I had zero side effects on tirzipatide - been on semaglutide 20 mins, lol, hope same lack symptoms. Thanks Dr for this great advice.

      @@IrishPrincess128this is high level question!!!! 👍🏾 There are papers that look into this. I do believe this is the mechanism causing some folks who do end up with severe gastroparesis to have longstanding issues. I have not seen extended gastroparesis in non-diabetics. Remember gastroparesis is a complication of diabetes. We do not know for sure if GLP1s are the direct cause of the vagus nerve dysfunction but it appears there are GLP1 receptors that may play a part in that pathway so it’s possible. To be determined…..

    Changing my relationship to food was made really easy by Tirzepatide. Fortunately I am already exercising by going every distance between 2-10km by bicycle.

    So glad to have come across this video! My doc has had me on Mounjaro for a few months now but didn't explain any of this to me. In fact she pushed to have me take the higher dose, but I was so wiped out from side effects I requested to remain on the low dose. I still suffer from exhaustion and feeling frequently thirsty, but at least my appetite is suppressed. Just started walking every other day a week ago, now have an ankle injury but will return to walking at the earliest opportunity. 😊

    Excellent video! I'm from Broward also!!!! Now on the West Coast. I've been on the compound for 6 weeks. Making sure I exercise and drink lots of water ! And try to eat as clean as possible.     Thank you for taking the time to explain how to operate on these medications and for explaining the fatigue factor. It make perfect sense that since my blood sugar is low I’m always tired.     I have been on Mounjaro for 7 weeks tomorrow. I was put on it for weight loss, but my last blood tests in March shows that I went from prediabetes to T2. In June of last year I was still prediabetic so this is new to me. The good news is that I've lost about 34 pounds so far, but I have a long way to go. I have completely changed my food habits, but this med is amazing at shutting off the food voice (averaging about 1500-1800 calories a day) in my head. That and the gym plus walking as much as I can has a been a big help. My goal is to control T2 with diet and at some point get off this drug, but in the meantime it has been a godsend.

    Excellent video! I was just prescribed Mounjaro and the detailed pamphlet that the pharmacy provided really didn't cover anything I should avoid. This was great information and will help do the right things to be successful here BTW - Glad to see that you were a NYMC graduate!

My internist reccommended I consider taking Wegovy, but I didn't want to lose weight too quickly, and I wanted to make food and lifestyle changes (because the pandemic was not so great for exercise or eating well.)I feel so much better informed--although my family lives on broccoli so that will have to change.     This is such great practical advice on how to use these drugs to the greatest, healthiest effect. The trials on the drugs involved both exercise and dietitian guidance. People should not expect to continue on an unhealthy diet without exercise when using these drugs. But the drugs make it so much easier, even possible to stick to a healthy diet

      Great Question! The answer is as early as possible but make sure you don’t over do it too soon. Reason being one of the reasons folks quit on their weight loss journey is they make too many changes all at once. If you weren’t a workout warrior yesterday you won’t suddenly be one tomorrow. Add in the new nutrition plan and changing your relationship with food and it can all become overwhelming leading to a failed attempt at changing toward a healthier lifestyle. With that being said I typically recommend body weight resistance exercise at the very least by week 5-6. If you have been doing weight training prior to starting the meds/lifestyle modification journey then start on day 1!

    On modified Keto with higher protein, ultra low carb and not having any issues with constipation on Mounjaro. No gas, indigestion either. I eat cauliflower, purple cabbages, broccoli or green lettuces. I Keep my gut healthy eating fermented foods. I drink probiotic brine, or have a generous serving of fermented probiotic sauerkraut every single day.

    As therapist AND someone who has had wonderful success on this Zepbound. I eat a BALANCED diet and DO NOT DEMONIZE any food because this is actually disordered eating. Every person is different and the best thing to do is collect data about how food feels in your body. I don't think it's sustainable to AVOID foods if you don't need to. Focus on adding things into your diet (fruits, veggies, eat the rainbow).

    I’m 1.5 weeks in. Lost 5lbs the first week. Wish I would have seen this video earlier. I had lunch with a co-worker today and I’m so nauseous. This makes sense that’s it’s just sitting there. Ugh, I will not do that again!! Thanks for these tips. It really put things into perspective!

    Not a magic bullet is SO important and something that I wish the non-obese community could wrap their brain around. I'm not just sticking a shot in and sitting on the couch eating pizza and watching the fat melt away. I've made MASSIVE changes to my eating habits, I go to the gym 3-4 times a week, I've greatly reduced the amount of alcohol I put in my face etc and so on. Yes, I'm seeing great results. Yes, I'm getting the assist of being able to not feel full. But I'm still the one putting in the effort!

    I'm not on any weight loss, but I am on GAC injections for strengthening my bones, which seems to help me with weight loss. I am 72 years young with no health challenges, and I wanted something to aid with bone health. Amino acids is my go-to, which is GAC. Love the fact you are talking about the things that you shouldn't do because everyone is taking this medication as a way out when it's and assist to better health. We still have to do our part, which we should want to, it makes you feel good about yourself

    I really love your videos. I had to tell a friend that is not a magic injection. I still have to work out. I still have to watch my ache of calories and I still have to as well monitor pasta, rice, etc. refined sugars, and things that you have to do if you weren’t on an injection solike I said it’s not a magic bullet all of a sudden gonna take the injection and everything’s gonna fall off I’ve been losing weight. I’m down 50 pounds but it’s taking me like eight months so keep your habits. Your behavior modifications are really important for continued weight loss as well as we’re gonna maintenance that you’re gonna need after the injection stop.

    Middle of week 5 (5th injection of tirzepatide, first week on 5mg)….down 13 lbs. No nausea but I’m already an old pro with intermittent fasting and longer fasts and have found this medication really assists with extending your fast. But yes, definitely noticed that along with my MIA hunger cues….my thirst cues have also become blunted. Force yourself to “fill up” on calorie free fluids and protein. Otherwise your muscle will go bye-bye along with that weight.

      I was advised by the doctor prescribing tirzepatide for me to set out my daily ration of water in the morning and drink small amounts throughout the day. Setting out an amount of water in the morning is to ensure that I get enough fluids during the day because, as you said, the drug not only cuts food cravings but also cuts thirst

    I agree with most of what you said. I've lost 115 lbs in 8 months. Sill have about 50 to go. I was heavy... 3 months on Mounjaro, 1 month on nothing (washout) and 4 months on retatrutide. I'll say that reta is the future and if you're interested, I can go into the details why. Buy Eli stock now. When that stuff get's out of phase 3, it's going to be the primary tool for t2d, obesity and fatty liver. Probably throw some hypertension in there also

Retatrutide although showing great promise in phase 2 and now phase 3 RCT, has not yet been presented for FDA approval for any indication (diabetes or weight loss). Therefore although I have access to the compounded form I won’t be giving it to any of my patients until it goes through the normal processes established here in the US. I don’t have to tell you the issues that could arise by skipping the necessary steps in such a delicate process involving health and welfare of human beings. Safety is number 1 when it comes to me. I’m definitely waiting until the data comes out before I make recommendations on starting retatrutide. As of today’s date, you are quite literally taking an experimental compound. In the meantime I am pretty sure you will be just fine. I’ll feel better once the science support my assumption. TAKE CARE!

    Given the high risk of muscle loss on these drugs, I think resistance training should be much higher up your list. In the UK we are told to start resistance training as soon as we start Mounjaro.

      AGREED! I typically recommend resistance training at week 5 (before folks lose a lot of weight) and beyond. Muscle wasting doesn’t happen that quickly in my experience. If they come to me with a history of weight training they are encouraged to do so from day 1. In my opinion the problem with starting resistance training (in patients who are currently couch potatos) is they quit. Too much change all at once plus the aches and pains of starting a new resistance training at the very beginning causes “failure of the medication”. It’s like all the people who start a New Year’s resolution and are back to square one by February.

      @@bettertoaster Hey there! Resistance training = weight training. This could be body weight exercises like air squats or pushups or pull ups etc. Alternatively with actual weights. The point is to maintain as much lean body mass (muscle) while losing the most body fat as possible. If you neglect building muscle you run a high risk of losing too much lean body weight during your weight loss, which is not desirable. I hope that explains it. Good luck!

    The biggest issue I have is getting enough water in..I used to drink 128oz a day and I'm barely getting in 40oz. I have to force myself. And when I do I feel so so much better

      Get a big 40 oz tumbler or 2 for water! I drink 2 of these every day. I add ginger root, mint leaves & or fresh lemon, most importantly add 1/2 electrolyte powder stick in each fill. Been on Zepbound since Jan 13, lost over 30 lbs so far.🙋‍♀️

    Good information! Started a few days ago on Saxenda and had horrible nausea & indigestion after a meal of a large portion of kale salad & some chicken. Could still taste the salad the next day....now I know why. Will be a long time before I have that again! Bring on the pizza!! (kidding lol) Excited to where this journey will take me.

    I just finished my last dose of Mounjaro 5/21/2024. In January, 2024 I joined a program BrightLine Eating. No sugar, flour, watch quantities, weigh & measure foods.
    Praying I don’t get the craving/hunger with a-vengeance. I haven’t started exercise yet.

    The three vegetables that he mentioned I have nothing but those vegetables because those are the only vegetables that I can really eat to get my veggies in those and then salads. I wish I could share with him my freezer and refrigerator. The good thing is, fatty fried foods are really not my problem at all. I eat a lot of baked and broiled and grilled fish and things of that nature. I eat a lot of salmo, tuna, chicken, lamb and turkey. I eat black beans lima beans lentils and Black Eyed Peas. Now on the fruit spectrum is kind of hard for me also because I'm really not into a lot of oranges or apples and things like that. I do eat a lot of watermelon, cherries, grapes and grapefruit. I use grape seed or olive oils. I will have an occasional two slices of whole wheat toast with my white egg whites and my turkey bacon or sausage in the morning. I will occasionally have a cup of cream of wheat or oatmeal. Now I'm a "Pepsiholic" but I will say I've only had maybe one or two cups of Pepsi within the last 4 or 5 months. And when I did have it I poured it over a cup of ice just enough to get the flavor. My daughter was laughing at me because she was saying that I was wasting more Pepsi than anything and I told her I just needed the taste over the ice I didn't need to drink it so technically I don't drink it but I do have the flavoring over the ice in my mind is like shaved ice with flavoring. Now I have swept the Pepsi out for ice tea or hot tea. The purpose of me having the grapes is because I will get a cup of grapes and I will eat that when I have a sugar attack like when I really really want some type of sugary some type of candy or something like that, that's what I'll do and when I want chips and things like that I'll eat seaweed. I went through my pantry got rid of all the soups and stuff and gave that to my friend and my daughter. I also threw away every piece of candy cookie cake anything like that that I know that I didn't have before my mom died I threw it all in the trash. I didn't bother to try to give it away because I knew the longer it took for them to come and get it nine times out of 10 I would say all I'll just have one. So as my niece said to me I have no excuse because I know what to do and I was just playing out being lazy and didn't have a handle on my depression. So even though she said some very mean things she probably saved my life.

      NICE Question!. There is no specific recommendation for social drinking and while I don't see a glaring reason while you shouldn't have a SINGLE drink in a sitting most patients report the medication definitely curbs their appetite for alcohol.

    What are you seeing from patients who had bypass surgery a long time ago that are now on glp-1 meds? any advice. Thanks doc!

      Great Question!! I actually currently have 2 patients who had previous bariatric surgery who are doing quite well. Of course its necessary to stay on top of hydration and making sure not to overeat. Generally they are having the same success as everyone else.     I work nightshift 12am to 10or 11am. I blame my schedule on a lot of my weight gain. I have too push myself hard to get exercise. I just got Zepbound and want to try it but I’m nervous. Plus my insurance doesn’t pay for it so I am paying out of pocket.

      Hey! 3rd shift workers have it tough but it can still be done. Reimagining what constitutes each meal of the day is key. Getting your workout later in the evening before going to work also seems to help a few of my 3rd shifters!
