Nightmare octaves for me when I was young was the E maj section of Chopin’s Ab Polonaise. I could manage one time through, but the section had all to be repeated and I would start to struggle. I am sure I held too much tension. I also was unsure of whether to go with 1-5 the whole time, since I tried 5345 or 5445 but never got that up to speed.
I had a battle with octaves when I was about 14 years old. I really wanted to play the Liszt/Schubert, "Erlking". I know now that I should never have gone near it until I'd figured out how to play without tension; I could very well have injured myself. I read once that when Horowitz had e.g. the octave cadenza in the Tchaikovsky to learn, he would practice 3 hours slow octaves and then 3 hours fast octaves. How do you feel about that?
One thing I´m really struggling with is when there are extra notes added to the octaves. For example (Nussknacker/ Schneeflockenwalzer) where there is a c major scale in octaves but with added thirds (but only sometimes) and I need to shift my hand to reach them... It´s very annoying!

Not so quickly..and with 1 4 finger not 1 5..but is difficult
