  • Hello Friends! In this video, I review how to release the lateral pterygoid muscle. This is a small muscle, but it is fierce! When the lateral pterygoid is overused and when it becomes dysfunctional, it can lead to jaw pain and headaches! If you can release this muscle successfully, it will relieve symptoms of jaw pain and/or headaches and help increase jaw range of motion. Please watch and enjoy!

    This video is much appreciated. I'm an M.D. in Atlanta and this technique has helped me with my severe pterygoid tightness. It really hurts badly when I first do this, so I try to breathe deeply and relax (if possible) to get those angry muscles to 'give.' It is heaven afterwards when they do.

    Great video. Quick tip from a Craniofacial PT that does this technique everyday.
    Wear 2 gloves on your hand. It will reduce friction , add cushioning to make it more comfortable, prevent tearing of the cheek and prevent inventions

    Morning Doctor Priya, PT is working on this very muscle. I am not encouraged to perform this at home, but I am. I still have the sensation of slipping in that area after I bite down on something and am completing the process of biting. It sounds like the crunch of walking in the snow (a sliding sound). I can chew without difficulty still hearing the pitch of crunching celery (but better). I did buy a roll that you put in your pillow case because my neck and shoulder area are tight. Doing exercises for this. Thanks for all you are doing for us. I have a FU with oral maxillofacial doc in April. Also have my on Tens unit which I will be using once a day. Of course this is part of PT, I have the until at home so I will do that on my own.

    Okay. This sends me into a massive flare every single time. I’m lucky if I can just stick my finger back there without it causing a big issue in my face and around my eyes. I know it’s causing me problems, I just don’t know how to gently start.

      If it sends you into a flare it may be too aggressive. Are you seeing a TMJ dentist or tmj physical therapist?

    Hi, I noticed I have a very painful lateral pterygoid muscle, when releasing. How can I find out what the root cause is? How do I know if its because of malocclusion or grinding or both?

      It can be due to clenching, grinding, the way the teeth fit together, or injuries to the head, neck and jaw. Seeing an experienced TMJ dentist or TMJ physical therapist will likely help you.

    Does this help when you have tinnitus secondary to tmj? It could also be from my cervical spine since I have issues with my C-spine.

      Thanks for taking the time to reply. I had a feeling you would say that. That said, I did this massage bilaterally, and it did seem to help relax the LPM, but it was temporary since chewing triggered it again. I guess I need to be more aware of my chewing and food choices.

    Could you tell me in general if it is possible to differentiate a lateral pterygoid in spasm and inflamed state? Or do they often go hand in hand? And is it somehow a good sign if the muscle is less in pain after released?

      Yes, it's a good sign if there's less pain after it's released. I don't know of a way to differentiate if the muscle is in spasm vs. inflamed.

      @@theTMJdoc I have a jaw noise for half a year and I think the cause is an imbalance in the neck because of bad habits and the noise (crepitus/grinding) appears every time I swallow now I also think the petrygoid is not working / weak, so I wanted to know if this muscle is weak / not working properly can cause this crepitus ?
