    People with sciatica usually describe aching and a sharp leg pain radiating below the knee and into the foot and toes. The pain can have a sudden or slow onset and varies in severity. Most people report coexisting low back pain. Disc herniations affecting the L5 or S1 nerve root are more common and cause pain at the back or side of the leg and into the foot and toes
    What are the causes?
    Compression of the nerve root and resultant inflammation play a role in pathogenesis of sciatica. Disc herniation resulting from age related degenerative changes, and rarely trauma, is the commonest cause. The inflammatory response induces resorption of the herniated disc material, and is thought to be the reason why most people improve without surgery.

    Really appreciate this video. About 3 months in with intense sciatica and hearing you call out how overwhelming this can be physically and mentally meant a lot (sounds odd I know).
    Have been disciplined with rest, small walks, and gentle lower back and core work. Over the past couple of weeks the pain seems to be retreating from below my knee and now more in my buttock. Hoping this is progress.

That doesn't sound odd at all, I can't tell you how many people have told me that statement has helped valid what they are experiencing. This condition can truly be debilitating. Fortunately it almost always gets better, however slower than we wish. Sounds like you are doing everything right, keep up the great work and let us know if you have any specific questions.

      Not if the issue is sever and acute. For more persistent conditions, it may be, we recommended starting slow and see how the body responds. The massage gun maybe help the associated back pain/tightness.

    Thankyou for this video I am having severe sciatic nerve pain right now! What can I take for bringing down the inflamation and help with pain! I feel like my flesh is being rip from my legs and a little bit of my calf, thingly down to my foot. I have no back pain though! I cannot do any excercise or stretching! Doctor where is your clinic located?

      Sorry to hear that you are dealing with this. This condition can present without back pain. When the pain is severe and acute, the only thing is time and injection/surgery. We are located in Redmond, WA, 30 minutes east of Seattle.

    I'm going on almost 5 months of extreme pain. My MRI shows a disc protrusion at the L5-S1 and clearly shows a pinched nerve. That said, I've done PT, accupuncture, chiropractor, had injection in my S1 and SI with 0 relief. I've taken prednisone burst, 0 relief. I can't sit in the car without extreme pain. Sleeping on my back only for 2-3 at a time if I'm lucky. I feel like I'm losing my mind and I'm so exhausted. Constant movement is all that helps. Ibprophen, Tylenol, flexeril, nortriptyline, gabapentin don't touch it. I started to use CBD, CBN, and THC to help me sleep! It doesn't really help much though.
    I'm meeting with a neurosurgeon on Tuesday and I'm afraid they'll want to do surgery. While I can't live in this pain forever, I'm afraid of surgery. Note... I have extreme nerve pain in my R shoulder and they told me a disc replacement in my neck C4/5 would probably help. It didn't. I still have extreme arm shoulder pain and spasms. Any suggestions or do you know of anyone like you and your practice here in San Diego, CA? Thank you for your most helpful videos (and I've watched a lot). Note, only Ice and hot showers seem to help. Or laying flat on my back with a small pillow under my back. Elevating my legs makes the pain worse though.
