I like the job itself but the stress levels are actually affecting my health.
Sometimes it's hard to be polite, but it's the professional thing to do. You don't want to burn any bridges just in case your future employer knows your current employer.

If you are writing a letter, and not an email, then you would include the general information, such as the manager's name and title, company name and address, etc.
