Fasting-- an age old practice resurfaces with Megan Ramos - Diet Doctor Podcast

- Humans have practiced fasting for centuries as part of religious rituals, but the medical community has shunned it as risky and dangerous. Megan Ramos, along with her co-founder Dr. Jason Fung, have embarked on a mission to change all that. At IDMProgram.com, Megan and Jason counsel patients how to use fasting safely to help with weight loss, diabetes, management, and overall health promotion.
They recently published a case series showing how they can help people get off their insulin in as few as five days! But fasting can be a double edged sword with significant potential risks of hypoglycemia, loss of lean muscle mass, and feeling awful. Megan helps us understand how to overcome those concerns so that we can benefit from fasting in a safe and sustainable manner.

I am a nurse of over 30 years. The increase in type 2 diabetes alone speaks to her legitimacy. It is hard to convince patients to try such a significant lifestyle change, as they would rather have a new prescription to try. I believe she is wise beyond her years and it is all because of her own health struggles that it came about. Thank you! Thank you!
I think the collaboration with Jason Fung might have something to do with it too
Fabulous interview, thank you, Megan.
This video was made in 2018 and she says she was born in 1984. She says she has been in clinical research for over 20 years. She was 14 or younger when she started in clinical research?
In other videos, she shared that she was 15 when she started working a summer job with Dr. Fung.
@@debclair822 Wow! Thanks, that solves it.
Excellent video. Has rekindled my interest to include the 24-36 hr fast with my Keto plan. Have been struggling and need to change it up. Thank you so much. Great info regarding the 4 day, 4 food start-up plan to start the Keto diet. Sounds like this would be great even for a stall and restart. 👍👍👍
I wish I knew about IF 20 years ago I was taking insulin and 2 other diabetic medications after a month I stopped taking insulin and my blood pressure is getting better I know it is hard but don't waste more time I'm doing 5/19 5 times a week.
Great interview. Good to meet the people who work alongside Dr Fung . I like the fact that the problems related to poverty and obesity were raised . Someone once wrote a book ' Fat is a feminist issue ' but here in the UK ' Fat is definitely a class issue ' . Middle class doctors lecturing the poor on will power , self discipline, and poor lifestyle choices . I hope one day we get a collective apology from the medical authorities regarding this .
I prefer OMAD most days - definitely no breakfast 🥞
I eat twice a day. 11 o'clock in the morning and 6 o'clock in the evening
I struggled to hear all that Megan had to say as my hearing is impaired. It would be great to have a transcript of this podcast. I was able to hear Brett, who is an excellent interviewer. I gave up halfway through, even though I was really interested. I have been on Keto IF for over 5 weeks. I am 88 and feeling the benefit. Many thanks.
Click the little CC box in the bottom right of the video to get the transcript with the video and audio.
Thank you very much for your reply. Unfortunately I can't find the box you mention. I clicked on a box at bottom right of video, but it just rotated the video.
On diet doctor's webpage you can see the podcast with captions www.dietdoctor.com/video/podcast#7
In the video window, you can click CC on the bottom right and select "English (United States) CC", you should be able to get subtitle.
Thanks to the Engineers Project l have found and read the transcript. I really appreciate your help.
Megan’s work is with very sick people, so not very relevant to me. I moved fairly naturally from
Sorry, hit the wrong button.....
I moved fairly naturally from Keto to having two meals a day with about 18 hrs fasting. It’s just a new way of life. My original goal was to overcome sugar craving, and this was achieved, to my amazement, immediately I started Keto. I am experiencing other benefits, so continue this life style.
Thank you for this interview. I really appreciate the level-headed discussion on fasting. Like Megan said, there isn’t enough information to support longer fasts, and the potential risk of lowering BMR long term. I too believe that a maximum of 4 day fasting should be beneficial with minimising risks (for metabolically healthy people, 36-48 for metabolically impaired)
Another great interview. Your interviews are always very coherent and I get so much out of them! Thanks!
fat fast prep:
bacon eggs olives avocados as much as u want for 4 days before u start fasting
Someone questioned Megan's statement she has studied this issue for 20 years and she is only 34. She also said she was diagnosed with fatty liver and as I recall polycystic ovanian disease at age 14 (34-14=20). So she no doubt started her studies at agec14.
I have been on intermittent fasting for the last 4 months. I read a lot of literature, books and am also part of the IDM program. Still this is one of the most informative piece of information I have come across, this can be very helpful to anyone trying out fasting.
How do I get to participate on this IDM program? Thank you

Fasting does this to your skin...
Brilliant thank you
What a fantastic story... Really enjoyed this interview. 👍
Love to listen to stories like this because I was once in the same situation as I will end a 12 day fasting period tonight. And I could surly continue with no problems but I don't want to let the meat I took from my freezer to go bad. I feel fantastic and will go back to OMAD carnivore that I've been doing after a year of Keto.
@steven -- I have been doing keto-ish + 16/8 intermittent fasting with pretty good results. How long have you been doing IF?
how do u sustain 12 days? whats your regime
Most common pattern I see here is that doctors who stuying it extensively and approve Keto and IF, practice it themselves.
No other popular study done by doctors, where they implement the new study themselves. Interesting
My take away point is to try doing 36-42 hour fasts 3 days per week to treat diabetes T2 for best results within shorter time frames to hopefully become normalized with blood sugars and off meds.
Have you fasted before? Have you already cut out sugar and processed foods? If not, you may find it very difficult to do fasts of that length. Why not start with 12 hours, moving up to 16-18 per day? Check out the work of Dr David Unwin who is successfully helping people reverse T2 diabetes in a gradual manner. Here is one of many good videos: ua-cam.com/video/OlKDM3SDHPA/v-deo.html&feature=share&fbclid=IwAR1UiRy0dIfIiTrYyfXMNIki-Nl8Gpzdu0-CV0waakSdlYGxJlYi-qA43l0
Excellent. I wonder if the body will adjust to the 36+ hour fasts eventually. I guess I'll find out.
Learning more every time I watch.
This was very informative and Megan is beautifully spoken.
Do some research before you make comments. She is a clinical researcher, not an MD and has been working with Dr Fung "about" 20 years ago, before she was "not legally able to drive yet." And yes, she is very, very smart, obviously.
Fantastic and refreshing!! With real example 👏🏻
Y’all are some fast talkers! Both yankees I guess. I do enjoy your podcasts and I always learn things. Thanks 🙏🏼

Very good vedeo.thanks for sharing.
Exceptional information. Thank you! We need teens on keto and fasting to establish their healthy lifestyle habits into their adult and senior years. Pro activity will defeat obesity, hypertension and diabetes in the future.
We recommend that teenagers not incorporate fasting as they are still growing and need the nutrition.
@@dietdoctor even given teen obesity and teen diabetes?
@@magma9138 Would it be quite dangerous for people with Type 1 Diabetes to fast? Type 1 is an autoimmune disease which can't be cured by fasting. It can be managed with daily insulin injections.
I was thinkin about that. Totally agree with u.
Hi Megan, I love the videos/ podcasts that you, Nadia and Jason make. I have been fasting for a year now ever since watching first video from Jason in summer 2021. I was pre-diabetic, uncontrolled hypertension despite two medications, etc. I started 72 hour weekly fast and transitioned to OMAD after 75 days and losing 40 lbs. I have been doing OMAD (2 to 4 hours windows) since and maintaining one year in. It has changed my life. Eating is fun again. It is liberating and life changing!!! I love watching and talking fasting! Keep up the great work!! Mario, Brockville, ON
Great topic ! I’ve been following Dr Jason Fung for almost a year. Recent blood test shows great improvement and now part of my life.I am now in control of my hunger. But what really helped me the most was when I changed my drinking water. The kind of water that is alkalized, electrolyzed ,mineral rich, micro clustered ,hydrogen rich , super hydrating water. Really changed my life.
Thank you. All of you that are helping us and getting this information out .
Great video. Will share when anybody asks me questions.
15:05 fat fast diet (bacon eggs olive avocados), this sounds really good. Does it matter if the protein amounts are high (from eggs and bacon?) Some purists say it's too much protein to maintain ketosis but to me, that doesn't make sense. I wouldn't think proteins would cause a significant insulin response nor would it cause your body to not use ketones, if anything, gluconeogenesis would be a pretty expensive process!
I started with time restricted eating but I wasn’t getting good results, so I switched over to prolonged fasting and that finally got me past my blocks. I had to use the prolonged fasting to get my body back into a state where a keto diet and time restricted eating started working for me.
EXCELLENCE IN EDUCATION ! Thank you !😊

How can I make my own pickle juice
Look up homemade pickles, or lacto fermented pickles. Recipe will have no vinegar in it. Just small cucumbers, water, salt, dill, possibly some spices too. Homemade lacto fermented sauerkraut is very goid and easy too.
Great interview 👍

Can I take my diabetes medicine during my fasting day.?
Please discuss medicine dosage questions with your doctor.

Hey all fast for 30 days as muslim do, check the timing and remain on keto durng the eating window. After 30 days rest one day and then fast 6 more days. Durng the whole time, keep calm, forgive others, dont get mad, try to be happy, help others and spread smiles. Once that done follow the good habits and if you can fast 2 days a week if not 2 then just 1 day. For girls during that time do not fast durng the time of menses. but do cover missing days.
Hi can I take lemon and salt in water please someone help me
Yes, that's fine! --Kerry
This is fantastic
It's too bad that the keto community seems to have adopted fasting as theirs, frequently propounding the opinion (across many media) that the only way anyone could fast easily is if they were low carb/keto first. Totally not true. I and many others follow an IF protocol without going keto (nothing against it, just doesn't suit me). IF is such a valuable lifestyle on its own; I wish someone would make the clear point that there really isn't a best or certainly only way to eat when you IF (especially if there are no urgent health issues to address). And to say that a keto diet is the healthiest way to eat is just a bit too biased. We are all different and there is no one way of eating that is best for all of us. But seriously, it is possible to fast and not follow a ketogenic way of eating, because ketosis is achieved during many IF protocols, regardless of foods eaten during the eating window.
I see people talking about different diets to lose weight and wonder who’s right. When I hear their meal was satisfying and they’re not hungry, I think, ha ha, they’re fasting.
I don't think I've heard anyone in LCHF / KETO take ownership of intermittent fasting. Anyway, who cares where the idea came from as long as it works. If you think about, IF has been around for centuries.
True, but IF does go much easier if you cut out processed carbs, which is lowering carbs. Fat burning increases and reduces hunger, while stabilising/increasing energy levels.
@@emmanuel-in-greece6954 That's true. If you wean yourself off the carbs there is less hunger pains, therefore easier to fast. Kind of goes hand in hand.
I was doing low carb (under 30 gross carbs) before starting OMD and didn't have keto flu. Time restricted eat workscfor me. I am not hungry during my non eatin phase. Only problem (if it is a problem) is getting my BMR Calories in in the eating window.
Doing research for 20 years yet she's 34 in this pod cast. This isn't adding up.
She started to help do research at the Nephrology clinic with Dr.Fung as a teenager. I’ve listened to several of her videos and read up on her. I’ve been a nurse for over 30 years and believe she is right on. It is often hard to get severely sick patients to go out on a limb and try something like this. They are often hoping for “the magic pill” that will fix it. The older I get, the more convinced that nutrition is the key to healing or improving many of the illnesses in our culture in North America.
The bacon... cooked in the microwave... ??
At wurst better than no bacon.
Fascinating brilliant discussion.
I have personally done a 21 day water fast and I know how really hard it is. So I don't believe at all the story of the 120 day faster who continued to do physical exercise and don't lose lean mass! Absolutely ridiculous! She is naive if she believed he wasn't cheating. Anyone on a full fast will lose approximately 1kg per day.
nonsense
@@inventionscience1934 Join their program for a fast of 7 days, you will realise the fasting benefits. Unless you are not in the permitted group to fast.
I think it may depend on how much you have to lose. I have never personally fasted more than 24 hours, but I have lost as much as is possible without getting into the anorexia stage.
@B Smoove
Fatties lose more than a kilo a day if they stop eating altogether. I know one fatty who lost about 12kg in 5 days.
When you say Diabetes do you mean Type One Diabetes or Type Two Diabetes? It would be helpful if you could be clear about the difference.
Dr.Fung and Ramos specialize in treating type II diabetics. They have had a number of successes of reversing type II diabetics.
When doc talked about diabetes its almost always type 2....if it type 1 we usually say it...
@@abumnm Not everyone out here in the real world, not in the medical profession, understands the difference. Because of stuff that people see in the press and on the internet Type 1's get 'helpful' advice about how they could get rid of their diabetes if only they would make an effort and go on a fast. Perhaps that could be quite dangerous. It would be useful to get into the habit of simply saying Type 1 Diabetes or Type 2 Diabetes.
@@abumnm Out here in the real world people don't always understand and Type 1's are made to feel it's their own fault that they've got diabetes and if only they would take the trouble to fast they could reverse it and stop taking insulin. It would be a good habit for medical professionals and journalists to take the time to say 'Type 1 Diabetes' or 'Type 2 Diabetes' to avoid confusion. It could be quite dangerous if people follow the wrong advice.
Found this interview very hard to listen to. Often Megan does not answer the questions directly and succinctly. Not a lot of value for me.
