After having just beaten Old World Blues again I think its mentioned in the Courier's conversation with Dr Mobius that the bits of bullet still lodged in your brain lets your cranial implant pick up a signal from your original brain, so that sounds about right I suppose
Well that sounds really stupid. Since the game tells you in multiple fashions from multiple sources your organs and spine were replaced, not simply removed. Supported further by not being fucken dead after the surgery.

Its not ideal however, and there are cases when fighting actually happens with "inactive" half taking over "active" and doing weird stuff. And then there are animals that use halfs of their brain one at a time and then switch, i cant imagine how that even works with self-perception and stuff.

That entire dlc was some of the best characters and interactions in the whole entire current series. The weapons and the quests where also top tier.
Old World Blues is the GOAT of New Vegas DLC. Better than Honest Hearts and Lonesome Road in my opinion, thought they were still both good.
