Just use a metronome to keep the quarter notes for you and focus just on the pattern. If it might help you write down on which stroke the quarter notes are going to be, then work on speed
quick note: There's simultanously a pattern of 4/4 on the background, which makes the whole thing actually 45/4. That's the actual time signature of it.

I can play just the pattern but then once the constant quarter note comes in it messes me up for some reason
Well it is a polyrhythm, so that doesn't help, but aside from that the pattern changes substantially once the ¼ note comes in

Time signatures are indeed subjective... so you can hear it in 45 if you'd like.
