The two main treatment options for scoliosis are the traditional and the conservative. The traditional one focuses on preventing progression, hoping a curve doesn't become larger to the point it is surgical. Conservative treatment, however, focuses on controlling and reducing the curve, so the curve has a lesser chance of reaching surgical levels.
Traditional treatment is reactive, while conservative treatment is proactive, and when looking at the different braces used for treatment, there are braces that match each of these treatment approaches.
The traditional bracing uses braces such as the Boston and Providence braces. These braces slow down curves from worsening. For example, a patient with a projection of a 60 degrees curve, currently at 40, might only slow down the progression to 50 degrees, but it will not correct the curve because these braces are not designed to reduce the curve or straighten the spine.
The conservative approach uses modern brace designs to correct scoliosis and help reduce the curve. An example is the ScoliBrace design, which will start reducing the curve from the moment the patient starts wearing it. So, if a patient comes with a 40-degree curve, we want to get them down to 30 degrees when using this brace.
Traditional braces normally squeeze in the spine two-dimensionally from the sides, and this squeezing type only manages to slow down progression. This type of squeezing can weaken muscles, cause muscle atrophy, and impact spine longevity. These braces can also increase the deformity of the ribs and spine if patients have a rotation problem with their scoliosis.
Traditional braces are mass-produced and work the same regardless of the type of curve you have, which makes them cumbersome for the patient. These are also hard to sit in and make patient compliance to wearing them a problem.
Compliance is also a problem because curves continue to grow as patients wear their traditional braces, making them hurtful and more difficult to wear. Also, as patients see no progress because their curves continue to grow, they become discouraged. This happens often with kids.
Now, ScoliBraces and corrective braces address all the issues the traditional braces do not. These modern braces address the spine in a tridimensional manner. Instead of squeezing, these braces push the spine, producing results in the scoliosis and the body that does not worsen existing deformities while reducing the curve size.
The way corrective braces work to reduce the curve is by combining them with a rehabilitation program. These braces use 3D scanning and technology that only builds one brace at a time, each one customized to each patient.
In fact, these custom braces fit better in clothing than the traditional braces, and they get more comfortable and easy to wear as time passes, making it easier for patients to move and have a normal life while wearing them. Patients also see results when looking in the mirror and with how they feel, making compliance with these braces achievable.
Both braces types (traditional and conservative) can be worn under clothes, but the Boston brace design will be bigger and bulkier than the ScoliBrace or other corrective braces.
For braces to work, they must be worn as prescribed. This often means 18+ hours, which is full-time bracing. For this to be achieved, the brace needs to be comfortable; otherwise patients will not be compliant. This is why many patients struggle with traditional bracing. The squeezing effect design makes them uncomfortable to wear. While the modern brace design is more corrective, patients see faster results, and it overcomes all the problems traditional bracing can not.

My son is 2.5 years old. It has an 8 degree curve. In my country it's very little to get a belt like this. Is there any way to get one? When Dr showed it, he was told to put it continuously until he was 5 years old
