      Yes, the morning mismatch in ADHD families between expectations and actual performance can be huge, and a big source of conflict.

      Morning horrendous all my life ten drs said it was CFS or neurosthenia.until now I no why

      Thanks for pointing this out, ridiculous prices for brand name drugs is something I have pointed out in several previous videos. If you have insurance, then the price differential is unlikely to be nearly this large. And some day, the US will act like a civilized country and everyone will have coverage for their medical conditions.

      It's probably worth asking your doctor what they think. I have people who have gotten good results with either medication.

    Hey doctor! My son is about to start this medication this week. Concerta extended release works amazing for him…but, the lowest dose is not holding him, and the next dose up is way too much. I find our doctor just switches meds (quillivant, quillichew, focalin, concerta) instead of maybe adding a tiny booster of something fast acting to get through the day. Is that normal? I just want my son on the smallest therapeutic dose.

      I had answered this last week, but not sure where the reply went. Different doctors have different philosophies. Some want to minimize cost and maximize convenience by keeping the pill regimen simple. Some avoid short acting stimulants out of fear they are more addictive or prone to cause other side effects. It is encouraging that your son's doctor is at least willing to make changes, I just hope that the doctor has reasons for their changes, and explains them clearly to you and your son.
      Your comment also highlights one of the possible problems with Concerta, in that, because of the osmotic delivery system, you really can't split pills to get intermediate, or smaller doses; that disrupts the slow release mechanism, and the methlyphenidate is not evenly distributed within the capsule.

    My daughter was on 25mg of quillivant and changed to 60mg of jornay. My doctor said that "jornay is dosed much higher because of the delivery system" But it doesn't seem like thats entirely accurate. From what i understand from your video, higher dosing just extends how long the medication released for. It seems like the blood level is still going to be much higher than it was with quillivant.

      I would anticipate a higher blood level of methylphenidate from the 60 mg Jornay than from 25 mg Quillivant XR. But at some point with Jornay, when the ability of your colon to absorb it is maxed out, then the blood level won't rise higher, it will just stay longer at that high level.

    So, my child started taking this for the mornings, when methylphenidate usually works pretty well, and it seems like hes almost completely unmedicated? Could this be because of what you’re saying with how its absorbed slower?

      The top three possible explanations I would offer would include yours, that it takes somewhat longer - but someone should notice effects within 2-3 hours. When switching between stimulants, many prescribers start low and intentionally choose a dose lower than they expect will be an effective just to avoid problems with overdosing. Or some people respond to methylphenidate but not to amphetamine products (but the reverse scenario is substantially more common).
