    I had bilateral patella tendon repair surgery from a freak accident 2 years ago. I was in the hospital for 22 days and in a wheelchair for a month. I'm back to running and working out after nearly a year of recovery. Recently I noticed an issue with the cartilage in my left knee. I got xrays and MRI's done two weeks ago and my surgeon told me to just work on my quads more and I'll be fine. After hearing you I think you are right. Working my quads feels good in the moment but afterwards I was to stiff and the pain was worse. You are a God-send. I wish I lived in Texas I'd go to your practice. You have a new subscriber.

      I'm just here to share that I have severe knee cartilage damage on both sides, and I managed to restore some function (like climbing stairs, walking long distances ~ 20km in one go, exercising) only once I accidentially started training the gluteal muscles. I didn't know it would benefit my knee (just got into this through 'get glutes' classes at my local gym, and after 2 years of consistent class attendance, my knees improved. I just wanted to say that this logic of investing in glute training to strengthen the knee, absolutely works. While at my local gym most females chase the 'glute pump' for the sake of size, I'm into glute training now to keep my knees 😄🙏 Almost any day, before going for a walk or impacting my knees in some other way, I'll get on a (thick) mat (to protect the knee touching the ground) and do at least: 50 donkey kicks, 50 firehydrants, 50 pulse donkey kicks...( 50 on each leg), and if I'm finding it too easy, I'll add a loop-resistance band. It activates my glutes and positively changes my walking/stair climbing movement. Yes, it took me years to feel the improvement in my knee, but it's sustainable 'restoration' 🙏 And my previous attempts at strengthening my quads and calves had hardly helped.
      Anyway, I hope you'll have great success with glute training and that you will fully recover and strengthen the knees 🙏

      @@paolaalmeida8088 Thank you for your kind message! Yes, I can truly confirm that my knee pain fully improved and it wasn't just ' a bit better' ( know what you mean!). However, that result took me years of consistent exercise, I didn't miss a week and did between 3-6 sessions a week for my glutes. It was also useful to me to strengthen my quads and hamstrings to offload the knees. Once I had built fairly substantial muscle mass (I wasn't bulky; like I'm a girl, I am not aiming for a wrestling physique), I could skip a few weeks of training without feeling any pain. However, longer periods of no training definitely affected my ability to sustain knee load (i.e. hiking with a backpack wasn't advisable after a few weeks of no muscle training).
      I'd also like to share 2 other insights in case it is helpful:
      1) A really useful, easy to do exercise for me was: hip thrusts with no weights, at home, using my couch or bed (whatever has better height for you). Just doing this for 3-5 minutes with no added weight, really contracting the glutes really did amazing things for me. Later, I was able to add light weights. I'd only do this if it doesn't aggravate your knee pain. For me, this worked really well, especially when I had knee pain that didn't allow me to do the firehydrants, donkey kicks on 'all fours' on the floor, regardless how thick the matt was. For me, it also gave me a better butt shape, which I'll take as a bonus 😄
      2) For years, I had knee pain, next to pain in my hips, shoulders and achilles tendons. I've tried anything under the sun, including accupuncture, blood tests (I was 'healthy'), laser therapy, physio. Nothing helped, until I found out that I had oxalate deposits 'everywhere', including my knees! It came from my 'super healthy', anti-inflammatory diet, loaded with spinach, almonds and other nuts (and butters), black/green tea, some chocolate, loads of greens, potatoes, salads, turmeric etc. And taking collagen supplements - all of these converted into oxalates. It was an absolute revelation to me and explained why I had persistent pain for so long. Only after I sorted this 'root cause' out, was I able to do all exercises to build hip/glute strength. Sally K Norton (you can find her on youtube) and others have extensively discussed the oxalate issue. Just thought I'll mention it to give the full picture of my story. It showed me how important it was to not just treat my 'symptom' (like knee pain) but to get to the root-cause 🙏. Once that was resolved (it took a while, years!!) the exercises 100% helped. For the record, the MRI showed I had a 2nd degree meniscus tear on both knees and patella tendon damage (fairly signifcant) on both knees, so yes, the exercises can address such issues and as long as you retain/maintain muscle mass in those areas, you've great chances to be pain free. I sincerely wish you the blessing of a pain free life and recovered knees to do whatever your heart desires 💝🙏

      @@paolaalmeida8088 I forgot to add: of all the supplements I tried, the one that truly helped my knees was a high dose Glucosamine Chondroitin supplement - took about 6 weeks to feel the benefit. I took between 1500mg-2000mg per day (spread across 3 portions with 3 meals or so).
      Also, to add to my 'oxalate' information below - some anesthetics that are given for surgery can cause an oxalate spike. Worth checking you're dealing with any of this to rule out a root-cause. I've just unconsciously suffered from this for absolute years and am so frustrated nobody, no doctor, no knee surgeon, absolutely nobody mentioned this to me earlier.

    Hi! Thank you for you content and makes sense. My kineto therapist focused on quads 4 months and got worse into an end. The full leg extension make way more to increase the quads. i have Chondropathy 4 degree and i m 45 years old. Its possible without cartilage implant surgery so i ever be able run again and do skipping rope, boxing if i do all the natural repair technique? Or surgery its the only way. Doctor said without surgery, just swimming and cycling and no running but still risq for protetic knee in the future. Thank you!

You’re awesome👏🏼 ..this gives me hope.. cuz after a whole year dealing with knee then it travel to hip labrum tear it’s been a journey of Debilitating pain I must say.. today I saw another Ortho for second Opinion and it’s all the same😢…they don’t really care to investigate my biomechanics.

      @@KeemvsKeem I had a torn meniscus....I really wouldnt do surgery for many reasons! Right now I think you are making a huge mistake to allow them to do surgery on you....medically and financially you are being taken advantage of. You body will heal given time....absolutely no reason for surgery! Please dont do it!

      @@KeemvsKeem When you go under for anesthesia your brain is subjected to a neuro toxin, you come out a little less yourself every time you go under. Your surgery is removing tissue that should be healed and absorbed by your body. The surgeons want you to do surgery and are not educated on the bodies ability to heal itself if given the chance to do so. It's really a terrible system we have in place right now, hospitals are a business and look at us as cash cows. You are taking a chance every time you go under.

      matute11 if its okay with you, can you please share that whether it was medial meniscus or lateral meniscus and which grade it was. I have again torn my meniscus, previously i had a torn medial meniscus at the posterior horn. I had got it repaired through surgery. Know i have again torn my lateral meniscus and medial meniscus at the anterior horn. I am also thinking of going through this 28 day course before deciding for any sort of surgery.

    Thanks so much for these videos, I’ve been focussing on glute exercises and am noticing an improvement. I’m avoiding all quad and hamstring exercises but would like to return to them at some point so as not ton lose too much strength there. Are there any knee-friendly ways of exercising those muscles?

      ​@@gameart6084 My knee isn't as good as I'd like it to be but working on my glutes has definitely helped. I've had to stop doing deadlifts since about a month ago due to tennis elbow and my knee has been feeling worse. Totally anecdotal but I do believe them to be linked and I need to find another way of working my glutes whilst my elbow recovers.

    I am thinking about ordering your program. I recently started doing squats and noticed a small improvement in my knee. I've been doing a weighted tailgate swing since March. When I walk, the back of my knees gets very stiff so i have to stop walking, stretch my hamstrings and calves, and do a standing tailgate swing of about 20 to 30 swings per leg. then I can continue walking.

      Hey Donna! Great work doing the tailgate swinging. If you can tolerate some walking without getting too flared up then, you're ready for the program. The tailgate swinging will only take you so far, you need to go further into the process of fixing your knee problem.
      You'll get a robust series of exercises to do 6 days per week for 4 weeks. Learning all of those exercises will give you much more control over your knee health.

    I have been diagnosed with osteoarthritis on both the knees. My right knee is the one that was bothering me a lot and I had a tibial osteotomy to relieve some pressure off the joint. There is plate in my thigh bone. The pain went away after the surgery. But I have developed knee stiffness, especially as soon as I wake up. Stretching relieves the stiffness to a great extent. But I'm having great difficulty climbing up and down the stairs. Will this knee program work in my case?

      Sorry to hear about your knee osteoarthritis and knee stiffness :-(
      But you've got a great question Meditation Home!
      The fact that the stretching relieves the stiffness is a sign that you've got some severe muscle imbalances. You're likely stretching out the over-dominant muscles (probably quads and hamstrings) and you need strength in your glutes and core. Possibly lower leg too.
      Getting this muscle imbalance fixed should theoretically make stair climbing easier since you'll have to increase your strength.

    I have hamstring tendonitis that I have been dealing with for over a year, lately i have been doing targeted (iso) hamstring exercises to try and strengthen the muscle....in addition to this I have been doing targeted glute exercise along with other general workouts (lunges, squat, wall sit etc.) and although the hamstring exercises appear to be helping that muscle, my knee pain has been steadily increasing. I have articular cartilage damage in my right knee and had arthroscopic surgery 1.5 years ago to unbridle the damage cartilage. Surgery was very successful and i was almost pain free up until that past 2 or so months...entire lower body is getting stronger as a result of the workouts but knee pain is worsening :/ very disheartening to think that i cannot work certain leg muscles in order to avoid knee pain....any advice??

    Would this work for full thickness chondral loss at the medial trochelar articulated surface with degenerative cyst ?

      Hey Madhab, yes this would work for your situation. Even with a full thickness chondral loss and degenerative cyst, taking pressure off the joint should allow the area to gradually feel much better.

    Every doctor i went to told me that cartilage can not repair. I had in my knee place where cartilage was torn and they said only scar tissue could grow there and even for that you need more blood circulation so they made holes in my knee for blood to flow in that area. Now i'm fine i don't wanna make MRI again and see hole there. I don't feel pain thanks God. But again cartilage can not grow again as it was.

      Hey Natasha! Glad to hear that you're feeling better. You're right, cartilage won't be 100% like it was before. But it can scar down and be healthy...just like skin gets scars and can be healthy. That's the best situation for cartilge healing

    I have osteoarthritis in both knees. Was told I need to have hardly any cartilage left in both knees and need surgery. I work 4 hrs a day 4 days a week. I'm in constant pain while standing (I'm a cashier). Need to lose weight and having issues doing it because walking for to long also puts me in severe pain. What can I do?

      It's likely a good thing for her to start to gently cycle so that she can get her knee joint fluid circulating and promote healing. She's just got to be careful to listen to her body and not over do it.

    Not only Glute but also Hamstring, If training glutes regrows your cartilage, then hamstring will sustain your "regrow cartilage" and also strengthen your Tendon and Ligaments (Two components that can make your knees' cartilage stop being destroyed gradually)

      It depends - do you feel your quad muscles work a lot during Zumba? If you do, it will make your knee problem worse. If you can make your glutes work more in Zumba then it should help the knees.

    My cartilage issue is at the back of the knee I had an mri scan , I play tennis and it hurts me I’m 50 years old now ..

      You're still young! You've got a great shot at healing and getting back to tennis. You've just got to figure out how to get pressure off your knee joints by getting your glutes stronger.

      Hey there, glad these exercises are working! The important thing to consider is that cartilage heals very slowly. This can sometimes take a full year to notice a difference. It becomes very important to be consistent with these exercises during that time.

      Great question Simon! Isolated hamstring exercises usually add excessive pressures to knee joints, so we don't recommend them.

    Hi, I am Neha from India. I have knee pain from 2.5 years after a very minor injury . I visited many doctor. Most of the doctors are recommending different different issues. I am not sure what is problem. But I have pain in my knee & it’s get severe if I have to stand at one place for longer duration.Also pain is at one place for few days than it shift to another place for few days & similarly it keeps switching.       Hey Neha! Sorry to hear about your 2.5 year long knee problem :-( and getting mixed answers from doctors.
      Knee problems can be very confusing for people in the medical field. Most knee problems that start out with a minor injury are usually set up from a muscle imbalance that develops as a result of the injury. That muscle imbalance can create bad pressures throughout the knee joint. This causes cartilage to get worn inappropriately over time and can lead to chronic knee problems.
      There's no way for us to be able to do an examination since you're in India.

      Hi Don'tbustthecrust, We understand that the advice about limiting hamstring and quad exercises may seem counterintuitive, but let's think of it as finding a balance, just like a seesaw.
      When it comes to knee pain and muscle imbalances, the hamstrings and quads tend to be more dominant, like the heavier side of the seesaw. This can put excess pressure on the knee joint, leading to discomfort and potential issues. Our suggestion is to focus on
      strengthening the glutes and engaging the core muscles, which act as the other side of the seesaw, helping to restore balance and reduce strain on the knee.
      You can go back to working the hamstrings and quads as soon as you are confident the glutes are stronger. You will know when the pain subsides in your knees but you will need to make sure that the strength in the glutes, hamstrings, and quads are balanced.
      Of course, we acknowledge that everyone's situation is unique, and what works for one person may not work for another. We encourage you to listen to your body and if you don't feel like focusing on the glutes is going to work for you then do what feels best.

    I'm curious and open minded, but I can't see how glute exercises can heal torn knee cartilage. Before I drop $100 into a program I need to understand the science of how it works. The other person's comment made a good point, that cartilage has no blood source. Can you elaborate on how this works ?

      Hey there, the trick with this is to improve the musculoskeletal health of the knee by taking excessive pressure off of the knee. Once we improve the health of the knee, the body itself is capable of making the necessary repairs. This is a slower process, but over time there will be noticeable improvement. Hope this helps!

But I had seen his explanation of the mechanical workings of the knee joint in a previous video, which was good, so I didn’t. And the rest of this talk wasn’t disappointing either.     Essentially, it was 8 minutes of promotion pointing to the program one needs to buy. Was compelled to view the entire 8 minutes in anticipation, telling myself every moment that "now he would demonstrate the glute exercise/s..." and ended up frittering away 8 mins without getting "the secret exercise" in the youtube title that attracted me to view it in the first place. Seems to have lots of similar titles that are of interest such as "3 Tips For Knee Cartilage Problems-How to heal your knees without surgery" etc. that I am cannot risk viewing.

      Literally virtually EVERY video he posts that BUILDING GLUTE STRENGTH is key to help heal hip and knee damage. This was "education" about WHY glute strength it is critically important.
      And, of course this this info, you could look up glute exercises on his videos. His videos are THE BEST! I recommend we listen. 21 days from excruciating pain to being able to walk 15-20 minutes. Incredibly valuable info!! Now, maybe a long term for "healing the cartilage" but, I am 95%+ pain free! My kudos and thanks to Dr. David!

      With all due respect, that is not a fair criticism. The Doc offers tons of exercises on this channel completely free. The 28-day course is an optional extra for the cost of one personal consultation.
      The "secret" is that the most important thing is to strengthen your glutes, and the solution is not a single exercise, but a family of exercises, which is covered by many videos on this channel, for you to pick and choose from. e.g. go to the page for the channel as a whole ("El Paso Manual Physical therapy"), and then use the search function to look for "glutes" or "glute" or "glut". Then also probably look for "knee" or "knees". And then just browse though the whole channel to make sure you haven't missed anything possibly relevant.
      Good luck, and I hope you find what you are looking for.
      Thank you doctor for this wonderful free service. I'm not aware that many doctors in the USA offer consultations free; free YT videos are the next best thing, and you are most generous in this regard.

    He did not tell us what specific exercise to do to repair the cartilage in the knee. His goal was to sell us his video. The title of this video is very misleading and I do not appreciate this type of behavior. I will not be buying the video.

      Essentially, it was 8 minutes of promotion pointing to the program one needs to buy. Was compelled to view the entire 8 minutes in anticipation, telling myself every moment that "now he would demonstrate the glute exercise/s..." and ended up frittering away 8 mins without getting "the secret exercise" in the youtube title that attracted me to view it in the first place. Seems to have lots of similar titles that are of interest such as "3 Tips For Knee Cartilage Problems-How to heal your knees without surgery" etc. that I am cannot risk viewing.

      Hey Sherly, unfortunately doctors aren't always right...
      But you're free to do whatever you like 😊
      Who knows? Maybe you'll actually improve a little if you try the right exercises. What have you got to lose? Doctors will replace your knee if your cartilage gets bad enough - that's a bunch of bull!
      If this helps you, you could avoid a knee replacement in later years.
