His second biggest blunder was to trust Uryu and give him Antithesis, instead of trusting Haschwalt more who considered him his God and father. Making these 2 blunders sealed Yhwach's fate.
I always thought the shrift abilities were already a part of a quincy, that his blood just fully released it like a bankai. That's how I rationalized that they didn't forsee Uryu betraying them. It could also explain why Uryu, the last half blood quincy, was immune to Auswahlen. If antithesis was accidentally activated at that point...

I have a theory that the Almighty would be less effective against a large group. Keeping track of so many future outcomes might overwhelm him, and changing one furure to his benefit would lead to unintended consequences.
I have a theory that the Almighty only lets you see the future. The Almighty Moustache lets you change it. My proof: Jugam technically has it at night, but he can only see the future, he lacks the mighty holy stash that lets him control it.
Tsukishima only manage to fix Ichigo sword because of two important factor. 1. Yhwach already left, if Yhwach was still there then it would be no doubt that The Almighty will over overwhelm The Book of The End. 2. For it's effect to of Book of The End to truly work in this situation Tsukishima need the help of Orihime and her Shun Shun Rikka. Which is also an overpower ability in it's own right and theoritically the Shun Shun Rikka is the only thing that can reverse the effect of The Book of The End in the fullbring arc and save Ichigo friends and family.

It was the real power of Ichigo's final bankai, which was to break fate, which meant whatever Yhwach saw in the future was broken by Ichigo's bankai...
personally my favorite theory on the weakness of his allmighty is that he can only see the futures of mortals. if a god in any form is involved, he cant see it. hence why he couldnt see mimihagi attempting to save the soul king in the future, why he couldnt see his power from the quincies that died and being used in the silver arrow, as it had his latent power in it, why he couldnt see through aizen's illusions, as the hogyoku is a near god-like item that he has fuzed with, uryu being able to use the power that yhwach gave him. the only outlier i can see is tsukishima for this theory, but meh
I think there is an art from Bleach Brave Soul game, of which Kubo did designed Quincy Ichigo (aka what if Ichigo awakened his Quincy power instead of his Shinigami power) The game is not canon, but Kubo did designed it as "What if" scenarios.

When did Ywach see Kyoka Suigetsu's activation sequence? I remember in FKT Arc, Unohana said only people who have seen the activation of KS were effected, making ichigo immune to the effects.

But allowed things to happen this way because it was the best future they could hope for. Almighty have an effect on the past and you cannot reverse it's effect. Orihime's power is to reverse time to "heal" her targets but it didn't work on ichigo's zanpaktou. Tsukushima bypass this by inserting false "past" events hence Orihime's power work.

personally my favorite theory on the weakness of his allmighty is that he can only see the futures of mortals. if a god in any form is involved, he cant see it. hence why he couldnt see mimihagi attempting to save the soul king in the future, why he couldnt see his power from the quincies that died and being used in the silver arrow, as it had his latent power in it, why he couldnt see through aizen's illusions, as the hogyoku is a near god-like item that he has fuzed with, uryu being able to use the power that yhwach gave him. the only outlier i can see is tsukishima for this theory, but meh

But allowed things to happen this way because it was the best future they could hope for. Almighty have an effect on the past and you cannot reverse it's effect. Orihime's power is to reverse time to "heal" her targets but it didn't work on ichigo's zanpaktou.
Tsukushima bypass this by inserting false "past" events hence Orihime's power work.
