Hola, no hay (que yo sepa) reglas sobre eso, pero yo usaría fuentes estándar como Arial o Times New Roman a un tamaño normal como 12, y a especio simple o 1.5. Simplemente usando el sentido común y que luzca profesional. Saludos!

Personal statement can be a different document to attach if you consider it can add value. It's like taking out some of the content out and then refering to that separate document in your cover letter. I did not have a personal statement in my application but others may choose to. Of course. In reality there is nothing carved in stone here. the way I would do it is to add the plan as exhibit and in the cover letter you can cite it when need it. But other ways of organizing things may be successful as long as the information flows well

You can't mention someone who is not your dependent, so no, it would be once they are actually your dependent
