Using AI and ChatGPT for Mental Health Progress Notes

  • If you're a counselor or therapist, you may wonder if AI can save you time on case notes and maybe even write better progress notes than you! Alternately, you may be wondering how mental health clinicians can ethically use AI at all.
    In this livestream Dr. Maelisa McCaffrey of QA Prep reviews the ethical considerations for mental health professionals when using AI for progress notes and other documentation. She discusses the current climate, how AI is currently used to write progress notes, HIPAA and security concerns with AI, and what questions you must answer before you could ethically use AI in your private practice.

    As a future therapist who has always had note-takers in school (I have ADHD), I love that this is an option. This would allow me to take on clients who use insurance without being anxious the whole session about how I’ll do on my notes after the session.

      Just remember that you really do have to know what goes in a note first! Most of the AI notes I've generated in testing need to be edited. But they can definitely help!!

    It's not AI, but I created an Access database that lets me select the interventons I used in a session from a list. Each intervention has a pregenerated sentence associated with it. The database then generates everything but the evaluation portion of a PIEP note. If the interventions are all in the system, it takes my note writing time down from 20 minutes to 5 minutes. It's been a godsend.

      What do I have to do to get this from you? I will pay, it sounds like you have created quite the hack. I would like to follow this format even if you don't want to share yours. Please tell me how to create one. I haven't used Access in a while but I am familiar with it. Thank you!

    I'm a psychologist and I really appreciate this video-wow! We need much more discussion about this in the field. I would never feel comfortable having an AI "listen" to a session (it will alter the process..."the observer changes the observed" etc.). At our company we are trying to incorporate AI (in an ethical manner) into our Note Writing software. I joke that our original software uses "Actual Intelligence" (point and click from our default statements)...but re-writing our statements with AI has taken our program to a new level. I do think anything that can reduce the administrative burden will help the profession but we need to be cautious. I do hope young clinicians still take the time to learn how to write their notes on their own...as I believe it develops their ability to conceptualize and think clinically. Supervising many therapists over the years, I see how this is something they struggle with. Thanks again for this! Patricia

      I agree! I notice that often time difficulty writing notes is related to difficulty conceptualizing what happened in a session. And also that clinicians tend to over-focus on what they do in session and under focus on what the client brings to the session.

      ​​@@MaelisaMcCaffreyI tended to overwrite and lose focus in note taking until I seen how AI wrote notes. Wants I seen how AI out it I adapted so it wasn't a play by play of session. I do think the clicians should be aware of what they are discussing and making sure AI isn't putting something not discussed. Thanks for the video

    I think AI is great in some aspects as it will put what we need in a professional setting and professional language that insurance companies understand. One would need to make sure what AI is spitting out is what the provider and clients discuss in session.

      I'm not sure of a specific resource for prescribers but do often work with prescribers one on one in consultation to discuss their specific needs.

    My patients would definitely limit what they say out of fear. I work at a substance abuse/mental health outpatient clinic...

      all of the apps, don't listen in. You can try an app that you type notes instead

      This is where AI may not be the best option. Use checkboxes instead to keep your notes succinct and to keep comments very objective.

    So if a system, like autonotes, is not HIPAA compliant, but you don't enter any personal info ...is that okay? Also, what is the tool that listens to your session and writes the note? Thank you. And by the way, You are so cute! Your eyes are beautiful. Thank you for all of the great videos!

      It’s nearly impossible to add enough information for a progress note without including what could be PHI, especially in a system like an AI. My recommendation is pay for something that is HIPAA compliant. It’s worth it! Check out Orchid, Mentalyc, or Upheal :)

    I am skeptical. Years ago I tried a scone. Failed they put Waaay too much data. I now use Dragon to dictate and pretty good.

      Make sure the APP has tech specs online you can review and provides a BAA for you to sign

      @@MaelisaMcCaffreylooking into a company called Mentalyc which is HIPAA compliant. You record your sessions, then upload them from your laptop or IPhone and they write the type of report you need SOAP, Intake, progress notes with MHS, etc. You then review it for accuracy then save, print etc. and finally send it to the insurance companies. Game changer if it works. I’m going to do a trial run next month.

    AI should not be used. It's not human intelligence and writing is important for intellectual processes that could lead to further conclusions about the client's problems and potential solutions that would otherwise be missed. Notes are "for" the client and a part of treatment, not just an ethical and legal requirement but a tool for clinical judgment.

      I have to respectfully disagree. The idea that every therapist uses notes in this way is simply not true. Not every person processes through writing. In fact, writing can take people out of that process sometimes. I say this as 1) a psychologist who used to specialize in learning disorder testing and then in ADHD and 2) as someone who is actually more like you and does process through writing and had to learn that does NOT work for everyone.
      In addition, many therapists with whom I work are so focused on what “should” be in a note that they don’t have the time or capacity to use notes for case conceptualization. Yes, it would be ideal to do that, but in my experience that’s more rare. Even therapists who do that well get stuck in the “problem of the week” since overall conceptualization requires looking at the full record regularly, too.
      While I don’t think AI should be used by everyone, we have lots of resources guiding people in how to use it ethically. Thats the key. More than one therapist has used the phrase “life changing” to describe how using AI for progress notes helped them. We cannot ignore that AI meets a very real need, and that it is possible to use it ethically… and that in some circumstances, AI can even make you a better therapist.
