Alright so update. After 4 months off and taking clomid for 2.5 months. My test never got over 55. The longer I went the worse I felt. I had heart palpatations and low iron as well. They did heart monitor, echo cardiogram, colonoscopy and endoscopy during the past 5 months . I was having horrible anxiety, panic attacks. My Dr wanted me to go 6 mo ths but I couldn't make it. It's crazy how important testosterone is. I started testosterone cypionate 4 weeks ago and since then I feel way better. Palpitations have almost gone away completely and I'm motivated and feeling so.much better.
Stopped trt 5 months ago to try get wife pregnant, initial sperm 0, gonna take new sperm analysis next month (month 6 off). Felt bad the firt 2-3 months of quitting then gradualy a little better every day but now as of last two weeks it feels like im declining again and i feel so depressed and lethargic, i do nothing, i dont want to do anything, feels like i just want to sit and stare and just cry, also a bit suicidal thoughts. But im hanging in there, hope wife get pregnant soon, fortunately we have 1 child already.... 34 years old, been on trt 4 years.

If I were you or anyone else for that matter who may be looking to maintain your testicular size and function while on TRT I'd use one of the online clinics like TRT Nation that still offers HCG instead of taking a much less effective drug in enclomiphene. They charge $100 per month if test and AI is all you need and $150 to add the HCG into your protocol. There are other online TRT clinics out there as well that have aquired the new DEA licensing needed to compound HCG and it's all around the same price. You can also go with a UGL (underground lab) if you can source it or you know a guy etc. Just don't settle for less when it's absolutely still available.
Hi doc, have you ever known levels to come back after 3 years of supprssion? My age is 35 years old The last time I went on HCG, I felt great. However, once I stopped HCG, a month later I felt terrible so I'm guessing my test levels dropped. What is the point on going on HCG if your levels will drop to zero once stop taking
Assuming you are not taking trt…? The point would be like priming the pump but you may have more luck w clomid and that depends on your pituitary output.

That's the lowest dose of Clomid that my mens clinic provides. You can't get Clomid where you live??? HCG is your best bet. Back before the state of CA got strict with HCG, many men were taking 500iu (50 units) Three times per week. It sucks being off T if you really need it. You have to find a way to get Clomid or HCG and your dose of Clomid was very low. Many men take 4x that.
Yeah bro. HCG 1000-2000mg every other day and 75mg hmg twice per week seems to be the golden ticket after 3-5 months, depending on your response. It costs a good bit, but hmg is becoming more affordable. there are a lot of anecdotal reports of enclomiphene being better for bringing endogenous test test production back online. These are the 3 compounds that I would use to get myself back into range.
9 years? It’s going to take at least a year of recovery for you… I took clomid and it wrecked me. My emotions were as extreme as the ocean waves. I stopped clomid after 2 weeks and have been cold Turkey for about 6 weeks now. I have been focusing tremendously on my sleep diet and training. I will be getting my blood work done in the next couple weeks to see where I’m at. But last time I quit, I was at my lowest of about 150 and my free was around 26. Hi Dr. T., I was on Cypionate, dropping off about 6 weeks ago. I was not on it very long (a few months). Over the course of the last six weeks I have felt progressively worse (fatigue all day, muscle tiredness, pain sensitivity, testicular pain, etc). I really feel the muscle fatigue in my back muscle and core. Is this a normal response? If so, when should I expect to level out? I was on the max recommended dose (100mg x2 a week). Yeah I think that is normal and you might want to look at some of my videos around stopping TrT what happens week by week and similar videos to get a better idea.
Question. What if you were already on an HCG? I was on 200mg a week of test C. Along with an HCG and an estrogen blocker for 7 weeks. is 200 mg too much to where is shut my LH off completely? And while everything returned back to normal quitting this cold turkey I haven’t taken anything for a week now. My hematocrit was way too high so I had to get off of it
As far as I know TRT is around 120mg per week and people usually don't need estrogen blockers/aromatase inhibitors. Your dose was high and can be considered as enhancement not as replacement therapy.
200 ow left me at 23-25 mnol ISH ..1ml 250 week takes me to around 36-38 ..that's low doses to me I'm normally used to 350 PW long term trt if you can call it that 👀 then up it to 500 with other steroids like deca but ive cold turkied now ..my t level is 26 mnol at week 11 ..clomid should elevate it slightly I hope
This has always interested me specially after having my son 4 years ago, used testosterone and other anabolic from age 18- 27 and was on test and deca when my wife got pregnant . Have had a few friends with the same outcomes even though they have been on for 10 years plus

I’m reading that DNA Fragmentation is a big issue with TRT that is not shown on a basic sperm analysis. Can you comment on this? I’m worried my husband’s count looks “ok” because he’s also taking HCG when in reality the DNA is the problem not the count (he’s in 200 a week of TRT after abusing anabolic steroids for years) but we’ve been trying for 5 years, 2 failed IVF and 3 chemical miscarriages. My doctor thinks his sperm genetics are an issue but he hasn’t done the test yet.

Relax brother I’d been using heavy steroids for 20 years. I came off last year an used 2000iu HCG Monday, Wednesday and Friday. 75iu of HMG Tuesday, Thursday and Saturday and my levels went from 35,000 per ml to 25 million per million within a year. You’ll be fine

I'm 58 years old, TRT for a decade because I was morbidly obese until 54 yrs old since 4 years old. My last testosterone shot in July 2022. February 2023, my free testosterone was 56. I then started hGC. As of July 2023 my testosterone levels are now at 613 with my testicles anecdotally appear 400% heavier and larger. I am also no longer on any other medications as I no longer have medical issues, except for this topic. My question is what happens when I stop the taking the HGC shots? I can't find an answer, just speculations. Thank you!

I have noticed a connection using my Apple watch's VO2 max rating as a surrogate for my testosterone levels. And so far it's been holding after consistent days of running. I this past Friday I swam for an hour and a half then on Saturday ran 8 miles and then on Sunday I ran 10 miles. No loss of power or "push" to run! Which is great because I have a 20 K coming up on August 1, 2023. I wish there was someway I could show my photo at 400+ pounds the few years ago - 2019. I have been debating on getting on TRT for the past year and half. My biggest concern is, does this increase chances of birth defects. If I was to get on it I will take Enclomiphene. Since Enclomiphene helps raise sperm count would you still have to get off of TRT to get your misses pregnant?
It’s hard to say because everyone’s different some people don’t have to take anything when they’re on TRT and they’re still fertile but that’s not typical. Most do have to stop.

I was prescribed clomid and hcg to help with fertility and my test and estradiol went up super high while my LH AND FSH dropped tremendously in one month. I thought those two were supposed to help my fertility? Should i continue or stop? Thank you
since HCG mimicks FSH and LH it can reduce the output from the brain somewhat. But that is often why you have infertility to being with. Clomid only increases these hormones. hope that makes sense.

Not even think about taking HCG off cycle! You need to make your pituitary gland, hypothalamus, testes work! Clomiphene + anastrozole are perfect PCT and restore spermatogenesis therapy. But you need check your luteinizing hormone, follicle-stimulating hormone, sex hormone binding globulin, prolactin, estradiol and total testosterone at least once in a month. If you would use HCG off cycle then forget about that your hypothalamus would ever work. Dosages of clomiphene and anastrozole are individual. Smh about HCG as PCT therapy. Run from doctor who would tell you that. HCG is a same cycle as roids usage.

you already did hcg and clomid so youve done a good job, if your fsh levels arent as high as youd like just tell your doctor to put you on hcg for a few months.

high amounts of hcg did not restore my fertility unfortunately so i had to get off of trt, 3 years @ 200 mg. i spent a month going down to 100mg. i stopped 6 weeks ago. started enclomiphene and hcg (stopped hcg after 3 weeks) and i still dont feel like testicles have grown? is it normal for their to be no growth? and is it possible that im still producing testesterone and sperm, without noticing enlargment yet? i still feel sluggish and sometimes depressed. was going to get bloods at 2 months. was fertile before trt
yes it does take time for that transition to take place and 6 weeks is no quite long enough to see a noticeable change. I would expect more change to come in the next mo or two.
My boyfriend came off TRT in June last year but was taking anavar 50mg daily. Then stopped in October and did a 45 day pct cycle which included clomid, tamoxifen and HCG. Yes you can, you could also stop test injections because HCG and Clomid will restore and raise your test higher then you were when you were natural. So as long as you stay on both you are pretty much at your trt range anyway. Ask your doctor if I am right then thank me later.

I have noticed that my testicular volume has increased however i am still experiencing dry ejaculation (meaning i do not have any ejaculate upon climaxing) is this normal? and does this provide any indication as to where i am in the restoration process? Been on TRT for the past 10 years (age 21 to 31) at anywhere from 400mg (earlier on) to now closer to about 150mg per week. I usually take a break off TRT for a month every few years which includes stopping TRT and taking HCG for 4 weeks. At age 25 I purposely cycled off for 3 months and took HCG while being off so my wife and I could get pregnant with our first kid. 5 years later we decided we wanted another child so I cylced off with the same protocol and it took another 3 months for us to get pregnant. Both kids are healthy and normal. Cycling off now so we can start trying for our third child!

Use for 3 months, see where you at. Continue if boosting. If not, gotta use HMG. Expensive shit, but it should get you back. I'll be doing this journey in a few months with ya.
We finally decided to start trying to have a baby. This are the recommendations from my doctor: We're cutting your T dose in half, adding Clomid 3 days per week with HCG for fertility. Anastrazole would be the same dose as well, .5mg. HCG is great and works quick to elevate LH/FSH levels Is that a good protocol? Any suggestions? Thank you

My husband is 29 and was on test for 2 years but he'd inject small doses and like twice a month. He has stopped now for a month. We both take chlomid , folic and he hcg because he has the issue. How long till healthy sperm restores after 2 year use of test
Something else is going on. Two injections a month, big or small shouldn't inhibit fertility much at all. The half life of Test is very short. His 2yrs of test use, if it really was 2x a month, did virtually nothing good or bad
@@swintegrativeCurios why time on TRT matters... once the T is flushed out of the body, and when the body detects the right signals it will start production again. How do you suppose the body keeps a clock for how long on TRT?
@@the42nd Yeah, I don't think anyone has a good answer for that question but it is more so that the tissues become more dormant and the question is do they wake up. Also we know that with age the tissues no longer respond to the signal regardless of if it is there (this is primary hypogonadism). So it is not always if the signal is there.

Yeah bro. HCG 1000-2000mg every other day and 75mg hmg twice per week seems to be the golden ticket after 3-5 months, depending on your response. It costs a good bit, but hmg is becoming more affordable. there are a lot of anecdotal reports of enclomiphene being better for bringing endogenous test test production back online. These are the 3 compounds that I would use to get myself back into range.

I took clomid and it wrecked me. My emotions were as extreme as the ocean waves. I stopped clomid after 2 weeks and have been cold Turkey for about 6 weeks now. I have been focusing tremendously on my sleep diet and training. I will be getting my blood work done in the next couple weeks to see where I’m at. But last time I quit, I was at my lowest of about 150 and my free was around 26.
I’m hoping to be at least in the 2-300s right now with optimizing all areas of my life.
Anecdotally I feel better than ever and my stiffies are back so 🤷🏽♂️
I found out that with hormonal deficiency, you gotta augment what is deficient.
Testosterone and estradiol is an end product of DHEA and Pregnenolone
Try these 2 first before testosterone therapy, and make sure it's legit

I was on Cypionate, dropping off about 6 weeks ago. I was not on it very long (a few months). Over the course of the last six weeks I have felt progressively worse (fatigue all day, muscle tiredness, pain sensitivity, testicular pain, etc). I really feel the muscle fatigue in my back muscle and core. Is this a normal response? If so, when should I expect to level out? I was on the max recommended dose (100mg x2 a week).

Question. What if you were already on an HCG? I was on 200mg a week of test C. Along with an HCG and an estrogen blocker for 7 weeks. is 200 mg too much to where is shut my LH off completely? And while everything returned back to normal quitting this cold turkey I haven’t taken anything for a week now. My hematocrit was way too high so I had to get off of it
Yes that much would likely shut off LH but all those things would generally return to your normal or baseline, including hematocrit.
As far as I know TRT is around 120mg per week and people usually don't need estrogen blockers/aromatase inhibitors. Your dose was high and can be considered as enhancement not as replacement therapy.

200 ow left me at 23-25 mnol ISH ..1ml 250 week takes me to around 36-38 ..that's low doses to me I'm normally used to 350 PW long term trt if you can call it that 👀 then up it to 500 with other steroids like deca but ive cold turkied now ..my t level is 26 mnol at week 11 ..clomid should elevate it slightly I hope
This has always interested me specially after having my son 4 years ago, used testosterone and other anabolic from age 18- 27 and was on test and deca when my wife got pregnant . Have had a few friends with the same outcomes even though they have been on for 10 years plus

Recently got blood work as the last time I did any testosterone was almost over 4 years ago and my levels came back at 327 total and free was at 10, reason for getting blood work was the fact that I was feeling sluggish and fatigued all day. According to doctor all levels are “normal “

I’m reading that DNA Fragmentation is a big issue with TRT that is not shown on a basic sperm analysis. Can you comment on this? I’m worried my husband’s count looks “ok” because he’s also taking HCG when in reality the DNA is the problem not the count (he’s in 200 a week of TRT after abusing anabolic steroids for years) but we’ve been trying for 5 years, 2 failed IVF and 3 chemical miscarriages. My doctor thinks his sperm genetics are an issue but he hasn’t done the test yet.

He should come off TRT, taking Clomid, HMB, and HCG. Can take up to a year. But the TRT isn't going to help his natural sperm production.
I'm so sorry you tried so much already and it all failed. 😢 that's my nightmare. My hubby is off TRT and has Azoospermia now. We're hoping we can recover it and then try IVF ourselves but the costs are insane. Insurance won't cover a thing.

Brother , I have been on TRT for only about 3 months and also want to have kids. It just in that short amount of time killed my sperm count . I would recommend getting off if asap if you want kids than maybe get back on afterwards . I’m now on clomid
use hcg while you’re on trt. when you want kids increase the hcg dose and add HMG for about 3 months at least then check your sperm count.

Relax brother I’d been using heavy steroids for 20 years. I came off last year an used 2000iu HCG Monday, Wednesday and Friday. 75iu of HMG Tuesday, Thursday and Saturday and my levels went from 35,000 per ml to 25 million per million within a year. You’ll be fine

Very helpful video, thank you. I had a recent sperm count, following five months of TRT (gel application), this revealed a sperm count of practically zero . I have stopped taking the medication, however, I have a small non-functioning pituitary tumour, will this affect the restoration now that I am off testosterone replacement? For context my sperm count was very high prior to testosterone replacement therapy, despite the fact that my LH and FSH was low ( pre TRT). Thank you
If the FSH and LH are normal then you should go back to previous levels.
@@swintegrative my LHS was 1.8 unit/L (just in range), my LH was 0.8 unit/L, out of range. Thus, I was pleasantly surprised my sperm count was so good pre TRT. Would you expect this to generate a positive sperm count after stopping TRT?
So to recap we can see a return in about 4 weeks? Then it takes another 2 months after that to be fully normal? So a total of 90 days 100% baseline. Can you talk about using hcg to speed this up?

I'm 58 years old, TRT for a decade because I was morbidly obese until 54 yrs old since 4 years old. My last testosterone shot in July 2022. February 2023, my free testosterone was 56. I then started hGC. As of July 2023 my testosterone levels are now at 613 with my testicles anecdotally appear 400% heavier and larger.
I am also no longer on any other medications as I no longer have medical issues, except for this topic.
My question is what happens when I stop the taking the HGC shots?
I can't find an answer, just speculations. Thank you!

I have noticed a connection using my Apple watch's VO2 max rating as a surrogate for my testosterone levels. And so far it's been holding after consistent days of running.
I this past Friday I swam for an hour and a half then on Saturday ran 8 miles and then on Sunday I ran 10 miles. No loss of power or "push" to run! Which is great because I have a 20 K coming up on August 1, 2023.
I wish there was someway I could show my photo at 400+ pounds the few years ago - 2019. As I never ran my whole life, only to the buffet line.

I have been debating on getting on TRT for the past year and half. My biggest concern is, does this increase chances of birth defects. If I was to get on it I will take Enclomiphene. Since Enclomiphene helps raise sperm count would you still have to get off of TRT to get your misses pregnant?
It’s hard to say because everyone’s different some people don’t have to take anything when they’re on TRT and they’re still fertile but that’s not typical. Most do have to stop.
I was on 400mg of test weekly for 12 weeks. I came off that and went on yet now for around 8 weeks.

hard to say for sure but that is reasonable thing to say. Yet sperm production and testosterone production are not always congruent.

I was prescribed clomid and hcg to help with fertility and my test and estradiol went up super high while my LH AND FSH dropped tremendously in one month. I thought those two were supposed to help my fertility? Should i continue or stop? Thank you
since HCG mimicks FSH and LH it can reduce the output from the brain somewhat. But that is often why you have infertility to being with. Clomid only increases these hormones. hope that makes sense.

Not even think about taking HCG off cycle! You need to make your pituitary gland, hypothalamus, testes work!
Clomiphene + anastrozole are perfect PCT and restore spermatogenesis therapy. But you need check your luteinizing hormone, follicle-stimulating hormone, sex hormone binding globulin, prolactin, estradiol and total testosterone at least once in a month.
If you would use HCG off cycle then forget about that your hypothalamus would ever work.
Dosages of clomiphene and anastrozole are individual. Smh about HCG as PCT therapy. Run from doctor who would tell you that. HCG is a same cycle as roids usage.
You can run HCG at 1,000iu EOD while lowering your test to 100mg/wk. This protocol has worked twice for me while being on trt

Hey doc, I was on TRT for 2 years and come off in November to try and improve my fertility. I feel like my testicle size and sperm volume is not where I would hope. I did a pct of Hcg and Clomid for a month and then let my body try and recover. I’m going for bloods in a couple of weeks. Any advice on what I should do to help this along quicker?
you already did hcg and clomid so youve done a good job, if your fsh levels arent as high as youd like just tell your doctor to put you on hcg for a few months.

high amounts of hcg did not restore my fertility unfortunately so i had to get off of trt, 3 years @ 200 mg. i spent a month going down to 100mg. i stopped 6 weeks ago. started enclomiphene and hcg (stopped hcg after 3 weeks) and i still dont feel like testicles have grown? is it normal for their to be no growth? and is it possible that im still producing testesterone and sperm, without noticing enlargment yet? i still feel sluggish and sometimes depressed. was going to get bloods at 2 months. was fertile before trt
yes it does take time for that transition to take place and 6 weeks is no quite long enough to see a noticeable change. I would expect more change to come in the next mo or two.

My boyfriend came off TRT in June last year but was taking anavar 50mg daily. Then stopped in October and did a 45 day pct cycle which included clomid, tamoxifen and HCG. Now in march we still haven’t conceived and he has started taking HCG again, I know it can take a while to conceive but I’m worried his sperm count won’t come back? is it worth him taking HCG still? And how long is normal for sperm count to come back to normal levels?
It may be on your side also, he can do everything good. But what if you are the one with issues.

Yes it’s possible with HCG but not certain.

Yes you can, you could also stop test injections because HCG and Clomid will restore and raise your test higher then you were when you were natural. So as long as you stay on both you are pretty much at your trt range anyway. Ask your doctor if I am right then thank me later.

I don’t see any reason not to use. I have patients that don’t use and are fine though.
You'd be fine it would just depend how long you were on. Typically PCT helps get things moving a little faster if you've been on a long time.

hello I am currently taking 100 MG clomid every day and have been for the last 3 months. I have been off testosterone for about 4 months now. I have noticed that my testicular volume has increased however i am still experiencing dry ejaculation (meaning i do not have any ejaculate upon climaxing) is this normal? and does this provide any indication as to where i am in the restoration process? I am working to produce sperm so my partner and I can do IVF. we are looking for ANY sperm production (not even needing to get back to baseline) I have read so many studies that say it can take 3 months to 1 year and I am feeling discouraged at this point. also, i am wondering, can HCG in combination with Clomid speed up this process at all? Just for context I am a 41 year old, and I was on testosterone for 5+ years.
Been on TRT for the past 10 years (age 21 to 31) at anywhere from 400mg (earlier on) to now closer to about 150mg per week. I usually take a break off TRT for a month every few years which includes stopping TRT and taking HCG for 4 weeks. At age 25 I purposely cycled off for 3 months and took HCG while being off so my wife and I could get pregnant with our first kid. 5 years later we decided we wanted another child so I cylced off with the same protocol and it took another 3 months for us to get pregnant. Both kids are healthy and normal. Cycling off now so we can start trying for our third child!

You sound like my twin, but I'm 40 and my lady is 30. I'm also on TRT and juice for 10 years. Sperm also 0 😅 Ok my friend, you need to get on Enclomiphene and then HCG. Use for 3 months, see where you at. Continue if boosting. If not, gotta use HMG. Expensive shit, but it should get you back. I'll be doing this journey in a few months with ya.

We're cutting your T dose in half, adding Clomid 3 days per week with HCG for fertility. Anastrazole would be the same dose as well, .5mg. HCG is great and works quick to elevate LH/FSH levels

Something else is going on. Two injections a month, big or small shouldn't inhibit fertility much at all. The half life of Test is very short. His 2yrs of test use, if it really was 2x a month, did virtually nothing good or bad

Probably some, it all depends on things like, your dose protein intake and workout routine.

It should be depends how long you have been on it and current life factors that can influence LH
@@swintegrativeCurios why time on TRT matters... once the T is flushed out of the body, and when the body detects the right signals it will start production again. How do you suppose the body keeps a clock for how long on TRT?
@@the42nd Yeah, I don't think anyone has a good answer for that question but it is more so that the tissues become more dormant and the question is do they wake up. Also we know that with age the tissues no longer respond to the signal regardless of if it is there (this is primary hypogonadism). So it is not always if the signal is there.
