Thank you , really enjoying your channel, regarding ketamine lower doses, as per Rosen it possesses a threshold response rather than an additive
dose-response continuum. Smaller doses of ketamine cause
analgesia and disorientation. Dissociation occurs when a dosage
threshold of 1 to 1.5 mg/kg IV in adult patients or 1.5 to 2.0 mg/
kg in younger pediatric patients is reached. Rosen page 57 !
Thanks for the question. Books vs. real life. Very different. You have to look to the literature to fully know. Doses that Rosens sight will be too much and cause hypotension in patients with elevated shock indexes. Thanks for your comment

what are your thoughts on inhaled ntg / milrinone - eg as recently featured on emcrit with Sara Crager?
Haven’t used NTG but seems reasonable.
