    Awesome video! I will definitely be watching this a few more times. I really enjoyed going through the scenario at the end. I am a new grad starting a job as an RN in the ED soon and this is very helpful. Also, I literally laughed out loud when you said “The patient is not getting any deader. Great video, but I need to correct something. At 13:27 we see not a typical VT but a Torsade de Pointes. We actually defibrillate as it is in defibrillation rhythms, but the pharmacological management changes. Amiodarone is not recommended and lignocaine is given instead. In addition, magnesium sulfate (2g iv/io bolus) becomes the first-line drug in this case.

    Great video and review for ACLS! One note is that the first rhythm you display at 13:30 looks to me like polymorphic Vtach that could even be Torsades de pointes in the right scenario. Doesn’t matter if the patient is pulseless as you would shock anyway, but would also give some magnesium, especially if the patient still has a pulse/is venturing from stable to unstable. Great content and thank you for your videos!

    • @ICUAdvantage
      @ICUAdvantage  Рік тому +2

      Yes true. I didn't want the focus on being exactly what rhythm, but more on the algorithm flow. In hindsight I should have use a more clear example.

    This was super helpful! I'm a new LPN on a tele floor (3 months) and had my first code blue the other day. I really didnt learn a lot about the rhythms and how to treat them in nursing school and this video has helped me prepare for my ACLS class. I understand now which med to use, when, and why. Thanks a ton!

    Epi. works poorly in the acidotic heart. One of the "VT"s was probably coarse VF. Organized VT should first be shocked with low energies (syncd), esp. in thin pts. It will often respond to 5-10 watt-sec. if the pt. is not acidotic. High energies sometimes promote conversion to VF. Asystole can be "shockable" ....low energy. It's done very frequently in the OR during cardiac surgery. Remember that there are other very rapidly acting antiarrhythmics, not just amiodarone. NEVER give up unless everything has been done. I've had 2 very successful resuscitations go over 6 hr. including one in a young pt. who had already been pronounced by the code team. Fortunately I just happened to walk by while the equipment was being rolled out of her room!!

    QUESTION 🙋 Why would a persons top of ears turn black during venting,they were on high oxygen why would they code 3 times, said they brought him back when they let the family in he was already gone but they try to make us believe he was alive but wasn’t. When brought attention to the nurse said he’s already gone no alarms went off they just started pulling tubes. It was very messed up. I just don’t understand why they all of a sudden let us 6 kids mom,dad,wife 2 friends in all at the same time. When only 1 person was allowed in fo 2 hrs a day. I’m totally confused.

    Presentation was clear, precise, and very helpful. I was expecting a comment about magnesium when the torsade de point rhythm was presented, but recognize that would be an understood part of the overall treatment process. Thank you for taking the time share this information.
