Does your hair follicles that fall have a little ball attached to the end? If so, it's hair falling from the root and not hair breakage. Either way, it could be just hair falling/breaking without that meaning that you experience real hair loss.

Not permanent. All of mine several thousand are now gone 20 years later
Did you do any maintenance during the years? Prp injections, biotin, finestaride or minoxidil?
Isn't the life span of a single hair roughly 5-7 years?
Bottom Line: There is no good options if you are going bald !!
