  • The following video highlights my approach to the management of urinary symptoms related to BPH or benign prostatic enlargement. There are several treatment options now available which have allowed the traditional paradigm of alpha blocker or TURP to be superseded. Ultimately the choice of treatment will depend on patient desire as well as prostate specific characteristics. The holy grail of improving voiding function with minimal hospital stay, no catheterisation, no erectile or ejaculatory dysfunction and being maximally durable is hard to achieve. For most there exists the need for some compromise which will dependent upon a particular patient's wishes. I hope the following video highlights options that men have when deciding upon treatment choices and provides some valuable insight.

    I have been searching for a video that shows details of BPH treatment for a long time and have not found any until this. You have provided a simple to understand presentation answering all the questions regarding various treatments, who's the likely to benefit and what to expect after treatment. Great Job!

    Steam treatment is best. Walk in, walk out. 5 min treatment no pain.

    great video. would u please do one on aquablation? some videos i've watched suggest it's the "best of both worlds" in that it removes mass from the prostate but does it so accurately that it doesn't compromise the bedroom. i would love to hear your take on it.

    My laser surgery was not good. I bled and bled for 5 years after it and finally clotted and had to go to the emergency room and have turp to fix and stop bleeding. I'm not a fan of laser.

    Hello charles, Superb video! Can you say which bph treatment is best to keep either one or both sphincters intact, incontinence and wearing diapers is a real concern! The obvious for me would have been urolift, but because of some low grade cancer and mri monitoring of this, the metal pins used in urolift wouldnt allow ongoing mri! Extremely interested in aquablation and rezum but because these are new treatments would maybe not be done yet because of the pc! Many thanks

      Hi Roger thx for the feedback. Stress incontinence is rare after most BPH treatments these days. It is possible to have an MRI after UroLift👌🏻 there can be some artifice related to the clips though

      Yes Rezum and Aqua ablation are newer modalities so less longer term data

      Thanks for your question. HoLeP is a form of caveatting surgery so amongst the options discussed above it is most similar to GreenLight laser prostatectomy. Both are forms of laser treatment but in view of the different properties of GreenLight and Holmium the surgeries are performed with differing techniques. GreenLight vaporises tissue whilst HoLEP in essence cuts it away. GreenLight is more haemostatic therefore there is less blood loss and has a lower risk of bleeding related complications. Both produce big improvements in flow and bladder function. Hope this helps!
