Excellent presentation Dr. Donnally and thank you so much for this great work. I had my L5-S1 fusion 16 weeks ago and I am going through exact same steps that you are advising here. Right now, I am doing PT focusing on core strengthening and balancing. I think I am doing well here with some pain here and there but the severity is way less than prior to the surgery. However, I still have burning, pain and tingling around my both heels. Also, I am recently noticing my buttocks feel a little numb or less sensitive. I wonder if it is due to fact that I have been a patient of LBP since late 2009 and some of nerve got dormant or something. Hopefully, these neurological issues gets resolved also down the road. How long does nerve regeneration usually take ? Is it a fairly long process ?
@@DonnallySpineConsult Thank you for your response sir . I sure can ask my surgeon. However, I was trying to seek your opinion based on your experience.

I have (according to MRI 3 years ago) a grade 2 anterolisthesis of L5 on S1. Pain has increased and is quite significant. A week ago I had a change in the pain. It has always been more on my right side. Suddenly my left is experiencing tremendous discomfort down my leg. Hoping to get in for a new MRI this week, it was ordered last week. I will see the neurosurgeon after that. My question for you is, how do I find the best possible neurosurgeon. The one I am to see works with my sister in surgery so she is obviously partial. I am terrified of becoming paralyzed. I am struggling with what I should do. Any suggestions?
Hi Dr. Donally, thank you for such an informative and motivational video. I underwent single-level spinal fusion surgery at L4-L5 on 2nd September 2024. It has been more than six weeks since the surgery. My only concern is that I feel pain when changing positions from lying down to sitting or standing. It usually takes me more than 15 minutes to feel normal, and I often need to relax in a chair. Is this normal?
Hey there, Dr. Donnally does about 15 new patient telemedicines💻 every week. Here is the best path forward: 1- email your insurance card to Spine@TSCspine.com (We can’t take Medicaid. Private pay is sometimes an option, unless Medicaid) 2- if we take that insurance THEN upload your images to my portal for your 1st visit. (The only HMO we take is BCBS). 3- I must have X-rays and MRI, X-rays are critical! “Just” MRI isn’t enough and is a misuse of your time. It needs to be from last 12 months. Nothing older is needed or wanted. Cant stress this enough, must have BOTH: x rays and MRI. 4- You need to get your CD of your X-ray and MRI (imaging centers will NEVER send them to us) 5- On the top of my website (www.DonnallySpine.com) is the link to UPLOAD images, from YOUR HOME! 🥰(or at a CVS with Physician LookUp Code k5037438) 6- To recap: email us, get CDs, THEN upload! Step by step: ua-cam.com/video/HpKOfI_1FSs/v-deo.html - Again- no Medicaid. Must have your X-rays and MRI.
Dr. Donnelly, I am the person with spinal stenosis from Philly. My second surgical opinion was aHami laminectomy at L4/L5, would the recovery time from that surgery be similar? I’ve tried injections with semi success. I would be appreciative if you would answer. Thanks
I’m wondering why this doctor’s timetable is so much faster than what seems to be standard procedure? Standard procedure seems to be no physical therapy and no “blt” for at least three months until an x-ray shows signs of fusion.
TDR 3/4 with 2 level below from one surgery sounds like a cash grab. Odd to need three levels at once- yet one could be TDR. A lot to digest. Anyway- many many many ppl after a PSF L4-S1 go back to marathons, why not!!
Thank you for explaining what to expect after a spinal fusion. I am scheduled to fuse and do a laminectomy at L2/3 and just laminectomy at L4/5. Will recovery be similar to just one fusion? Btw. The first doc wanted to fuse L2/3, 3/4 and 4/5. I elected the second opinion.
No just a lumbar laminectomy . Now the doctor taking about a 4 level Fusion. Went to a nother surgeon he said .he wants to use coflex. At 68 years old 4 level with rods to much at my age. 😊
Im at 4 weeks been having a burning sensation at one of my incision site then radiates into my butt , no infection at incision site though , any ideas what it could be ?
Great video. Honestly, very beneficial. Thank you.
Glad it was helpful!

Hi Dr. Donnally, Do you provide surgical services for those suffering from craniocervical instability?
Not via telemed and “non local”. Too much going on with it

Maybe don’t if they work with family bc then if any issues it’s super awkward or emotional. See a partner of his or hers. Or see me

It’s a big surgery, likely will have on and off pain for while. You’d need to ask you doc

How about multi level 2-3 fusion and lifestyle expectations after 6 months to a year. Also what about a hybrid, say L5/S1 L4/5 fusion with ADR at L3/4
