Pathogenesis of Preeclampsia | Pregnancy induced Hypertension | Nursing Lecture

Preeclampsia is a pregnancy-specific disorder, caused by the gravid state of the uterus. Here in this video, the pathogenesis of this disorder is explained, from where this disorder begins.
The Etiopathogenesis are-
1. Abnormal Trophoblast Invasion -
In Preeclampsia the Extravillous trophoblast fails to invade the spiral arteriole in the 2nd trimester thereby the diameter of the vessels remains narrow and causes high resistance and low blood flow thereby causing hypoxia, ischemia.
2. Genetic Factors
Many of the genes are responsible for Hypertension.
3. Immunological Maladaptive Tolerance
Trophoblast express non-classical MHC (major histocompatibility complex) which is recognized by the dNK cells and stops the cytotoxic response by the T cells and these natural killer cells allows the trophoblastic invasion.
As the tissues become hypoxic there is less expression of the MHC molecule thereby the T cell-mediated response is increasing and by the various inflammatory mediators, antiangiogenic factors, and the imbalance between the vasodilators and vasoconstrictor causes endothelial dysfunction or the endothelial cell activation.
So as the vessel starts to damage there is vasospasm which causes hypertension, and leakage of fluid into the interstitial space causes Oedema and proteinuria.

Inflammatory mediator causes vasodilation of vessels that are leaky and promote edema and facilitate immune cell infiltration. Oxidative stress is the disturbance in the balance between the production of reactive oxygen species (free radicals) and antioxidant defenses, thereby causing tissue damage.
