    I'm really thankful for your channel. My worst symptoms are cognitive and neuropsychiatric, so the idea of doing research to understand my illness is so overwhelming to me. The amount of effort you put into organizing and explaining all of this stuff, and going into the research is something I'm really grateful for. And I appreciate the skepticism and discernment, sometimes it hard to talk to other lyme/bartonella communities about these things because they are kind of prone to spreading misinformation. Thank you

      I've had a lot of difficulty describing my illness to other people because of the misinformation that's out there on top of my worst symptoms being psychiatric. I feel like a lot of people are quick to write me off as being someone who needs therapy and that I'm a hypochondriac and it's all in my head. I've had a hard time describing how debilitating this is for me and i question the validity of my condition (despite being responsive to treatment in the past) because I rarely am validated by the people around me. So again thanks so much dude:( having someone to explain the science of this stuff to me helps me when I dont have the brain capacity to do it myself. I'm slowly building the funds to start treating again

      Aww Zoe thank you so much for your comment. It's comments like these that keep me going when I'm not making any progress in my illness. I definitely agree that the Lyme/Bartonella world can suffer from pseudoscience and a lack of discernment and that's what made me question my diagnosis. So I started this channel to be a good steward of the science. I am sorry to hear of your suffering but it's encouraging to hear that you have been responsive to treatment in the past and I hope you get to treat ASAP!

    Funny, I just took a SIBO test by GDX and my provider said “oh, flat line results, probably H2S SIBO which can’t be tested for yet, so, sorry...” (clearly not a gastroenterologist... but I digress). I have been doing all the research in the last few days and told him about the Trio test... and that he owes it to me to order it for me... so he’s working on that. But in the meantime, I’ve read and/or listened to every journal you referenced in this video and am thoroughly impressed with your ability to detail it all out so perfectly. Since I’m basically treating my IMO and SIBO on my own (I have the symptoms of IMO but the flatline hydrogen and methane tests), I tried to start on the route of treating the symptoms (beginning the herbal treatments for IMO/constipation) before working on the H2S, but strangely my body is NOT having it. I have been taking supplements on a daily basis for years that have only recently starting bothering me (ALA, Avmacol (sulforaphane), NAC, glutathione) and yesterday when I started with the allicin I realized... it’s not in my head. I am going to throw up from the stomach agitation. So since I literally can’t tolerate the herbals for treating methane producers... I’ve switched to treating the H2S... which is making me feel like absolute garbage. I have gotten recurrent UTI’s since 18 so with all the antibiotics I’ve been on, I’m just not excited about taking more for this. I wonder if I’m making a mistake by treating the SIBO before the IMO? (Lab scientist here, so I’m clinically trained, not a person just randomly regurgitating things I read on the interwebs..) aaaanyway, came across your UA-cam channel. You’re quite entertaining. Thanks for your video.

      So glad you found my channel and thank you for watching! Your doc definitely owes you a test! Damn right lol. If I had to guess, the reason you're having problems is because NAC and allicin are highly sulfuric. I can't tolerate much of NAC and I kid you not, I sipped one drop of allicin in water and had belching within minutes. The good thing about Xifaxan is that the majority stays in the lumen so it's pretty benign as far as antibiotics go. I think combining the antibiotics with a mucolytic is key so that the antimicrobials, whether they be pharmaceuticals or non-pharmaceuticals, can penetrate the mucous in the GI tract better. And then also, if you haven't already looked into MCAS, that may be fruitful

    All I have is a mild to distrubting but not excruciating pain around my ileocecal valve that comes and goes, sometimes wanders to the other side of my lower abdomen or hangs around my navel for a little while. I have this for 3 month, won't be able to get tested for sibo/imo for 5 month due to work. I hate my life. I hope keto and restricted diet I'm about to start will help me reduce the pain.

    Jake, I’ve you ever gotten a GI map test done? I know can point in the direction of SIBO, but can’t actually diagnose it. I feel like that with the breath test can paint a pretty good picture of what’s going on in the gut. I am definitely going to get breath test done, because I am definitely thinking I am methane based SIBO... burping incessantly, SULFUR smell you talked about, and have always dealt with constipation. So was curious if you had had the other test done as well... Also, do I seek out just a GI dr? Or is it a functional medicine doctor?

      I've had lots of GI maps done. It's a super sensitive test and it's hard to determine what is clinically relevant. I actually think too many doctors are obsessed with stool tests and any doctor who thinks the stool test shows you what is going on in the small intestine is not up to date with the latest reserach. You cannot, I repeat, cannot! tell what is going on in the small intestine from a stool test. People used to think the sulfur smell was related to hydrogen sulfide SIBO but we now know that that's not a very good marker. You should just get the trio breath test done. Constipation however is correlated with methane while hydrogen sulfide is correlated with diarrhea. I see an integrative GI doc who recognizes Bartonella and other vector borne illness and I wish I had gotten to him wayyy sooner

      @@Boojerella yes it's possible but it's just less likely. The correlation is between H2S and diarrhea. However, I didn't have diarrhea or constipation. My SIBO manifested as MCAS. So there are trends but everyone is different

      I heard in another video with Dr. Siebecker (you can find the link in the video description box underneath my video) that it is not available outside of the U.S. yet but when it is, they will probably sell the machines so that doctors can process locally because of the stability of the gas

    Would you recommend sbo probiotics for hydrogen sibo?

      I haven't done a deep dive on probiotics and I am not familiar with that brand/kind (especially because I can't tolerate probiotics right now). I know that many functional practioners use probiotics with success but also we don't have a lot of good research proving which probiotics are good and under what conditions. Dr. Pimentel also makes the argument that since SIBO is an overgrowth of bacteria with impaired small intestinal motility, probiotics have the potential to exacerbate that overgrowth. Long way of saying 🤷‍♀️

      I would say the best way to get help is to join SIBO Facebook groups. There is a hydrogen sulfide SIBO group specifically on Facebook. The SIBO doctor, Dr. Nirala Jacobi, and Dr. Ruscio have some great SIBO podcasts too

      I KNEW I COULD COUNT ON YOU FOR THAT BIT. Wow the emo song really hasn't aged well lol. The Xifaxan has brought down some of my MCAS symptoms. It's really been quite miraculous. I think it was a very important puzzle piece for me that has been missing for two fucking years. I asked Dr. M if he thought Xifaxan might kill some Bartonella in the gut and he was like maybe. It was a non-committal maybe lol but let's hope!

      Yes it’s a super legit test. Lots of research to back it up. Search Dr. Mark Pimentel on Google Scholar or Research Gate to see his body of work and print some of the abstracts out to show to your doctor or ask your doc to look him up. I believe you can also order a test directly from the website through one of their affiliated docs but don’t quote me on that

    idk if this is what relates to me. I'm a little confused but I swore I never had a burp until like a couple of years ago. it's so excessive that I can't breathe right and my burp gets super stuck. idk what that is. like I couldn't sleep last night until I burped a million times.

      Sounds like something is definitely wrong with something in your GI tract! I know that isn't very specific lol. SIBO caused incredibly painful, relentless belching in my case. The gas from SIBO has to come out somewhere so if it's not coming out your rear end, it's going to come out your mouth and/or make you bloated/distended. If you haven't tested for SIBO for all three gases, that could definitely be something to look into

    Can anyone answer the following which I've have always wanted to know?:
    Does eating a decent amount of boiled cabbage make EVERYONE, including healthy gut people, release gas?
    And is that gas considered to smell as rotten eggs?

      I feel like eating a lot of any cruciferous vegetable could make one gassy. The standard American diet is pitifully low in fiber but there is such a thing as overdoing it on fiber. But I don't really have a definitive answer to your question

    Hi Jake, you are so funny and smart! I live in the UK and I was told by a company that only tests for Hydrogen, and Methane, that Hydrogen Sulfide is not accurate. What are your thoughts?

      Haha thank you 🙏! And who told you that? Lab tests in the US, whether they are from big labs or small boutique labs, are not transparent. It’s actually something I talk a lot about in my videos. Labs can say their tests perform well based on studies they do themselves. It’s like a teacher letting you grade your own paper and you giving yourself an A. The only way to feel confident a lab is doing their science correctly is through publishing in peer reviewed journals so that the public can see your methods are sound and your scientists know what they’re doing. Dr. Pimentel and his team publish prolifically so if there is one SIBO test I trust, it’s the trio smart breath test

    Hi you mention belching as a symptom. I'm really confused Bessie I only belch around 3am as I get major discomfort top left of chest area. Also sometimes between shoulder blades. I thought this was gastritis. Maybe the sino casuss gastritis? I'm so confused. But I'm usually fine when I go to bed but it gets me in middle of night even though I havent eaten anything since 4pm! Help please!!:)

      Gastritis literally means inflammation of the stomach. I think it is possible that SIBO causes, or at the very least, is associated with gastritis. SIBO can also exacerbate/cause MCAS and/or histamine intolerance. You could try an H2 blocker before going to bed. Many people with MCAS experience what is called "histamine dumping" in the middle of the night. I don't experience this but many do. I am not sure why this happens but maybe Google would know lol

      @@BartonellaBabe I've tried cetirizine but that doesnt seem to help. Can you tell me where you think I should go from here? Pantoprozale calms things down during the day (if I had a really bad night episode), yet doesnt do much for me at night. I feel like my small intestine really has such a tough time digesting amd moving things along that it causes a chain reaction that makes it's way all the way to the top of my stomach.

      @@josepe2448 well first, have you been tested for SIBO? If you haven’t, that something to pursue and then obviously you would need proper treatment followed by a prokinetic for at least several months. Second, cetirizine is an H1 blocker. Most MCAS patients need an H1 blocker and an H2 blocker. An example of an H2 blocker is Pepcid. Also, many MCAS patients need higher doses than what is recommended on the bottle. I’m not a doctor and this isn’t medical advice but you can ask your doctor about higher doses of H1 and H2 blockers.

      @@BartonellaBabe i take both famotidine and cetirizine. I think I'm gonna start this adventure by consuming the raw honey and teas and aloe drinks and dgl licorice extract. I'll also start saving up $ to see a naturopath doctor or similar that can supply the sibo test. If you have any other suggestions or you really think I need to do something else etc please let me know. I'm getting a lot of anxiety about teaching hs kids with this condition.

      NAC is a sulfuric compound so it can cause problems in those that are sulfur sensitive. I can handle it at small doses every few days. There is no such thing as NAC that is made for sulfur sensitive people, as far as I'm aware

      Do you know if Mo-zyme forte can cause irritation in abdomen if cysteine levels are increased? I have irritation in my stomach when taking Mo-zyme forte, and I am just wondering if I need at first to lower cysteine levels in order to take Mo-zyme forte?

      I think molybdenum sounds like a great way to deal with the sulfur in one’s system. I couldn’t take it because with my severe MCAS I have to really only take one or two things tops so I chose Xifaxan and a mucolytic instead

      I'm really not sure as I don't know about environmental H2S, just H2S produced by microbes in the human digestive tract. I know that H2S is toxic in high levels in the air but Google will know much more than I do 🙂

    Yes, the I in IMO should sound the same as Intestinal.
    In fact I wonder why everyone says SeeBO, then.
    Maybe what would be linguistically correct would be CYBO(RG).
    Curiously, in Portugal, SIBO with the I like Intestinal, is almost the exact pronunciation of a word that means Sebum.
    If you've got SIBO H2S have a low Sebum diet! > ; )

      I actually don't have either. My SIBO symptoms more manifest as mast cell activation syndrome. I am on 150 mg of Benadryl a day to eat so perhaps I would have diarrhea if I was not on this much Benadryl but I need it for MCAS management

      I hypothesize that what is key is using mucolytics. A lot of the microbes that are found in SIBO live in the mucus membrane so you need to get the antibiotics to penetrate there, regardless of the type of SIBO

      @@osmanabdi1214 mucolytics include things like NAC, diatomaceous earth, and monolaurin which may break down the mucus layer of the small intestine a bit and allow the xifaxan to penetrate there. Xifaxan cannot penetrate the mucus lining. The mucolytics I listed above are supplements

    Does drinking milk work against h2s for worker ? , do you have any resource

      Ya know I haven't done a deep dive on milk but what I do know is that harder to digest sugars (like lactose) may "feed" the bacteria that cause SIBO because they "eat" the sugars before your body can.     Lmao I love your personality and sense of humor. I’ve been suffering from SIBO for a couple years now (diagnosed in 2020) but I struggle to remain in remission due to being Mexican and LOVING dam good food😓😝😂😂 I look forward to watching all your videos and interacting with u. I’m going to try dr pemintels LFE diet long term and see if that heals me forever!
