    5 questions in and he hasn't answered a single question that she's asked - he gives a different answer than what she asked.

      Well, to be fair her audio is messed up (background echo of her own voice it sounds like), and you can visibly see on his face that he's having difficulty following the questions sometimes.
      Also, dude is a scientist and therefore can't always answer a question if he doesn't have definitive proof to back up his claims. The first question is a great example of this -- he doesn't definitively know which bacteria are responsible for H2S SIBO (I'm sure he has his theories on that), so all he can do is talk about the specific things he's been able to show in the lab so far. In general, that's true for all good researchers, but it's especially important for Dr. Pimentel not to "go out on a limb" to answer because SIBO is only just becoming accepted by mainstream western medicine. Even then there are going to be many skeptics looking to discredit his and others' work in this area. If he makes any claims he can't back up with his studies (or those of his colleagues) his reputation will suffer. Which means his research and funding will suffer, and ultimately those of us with SIBO, IBS, etc looking for treatments will lose out!
      In other words cut the guy some slack :)

    It is 2021 and I tried to get my GI Doc to test me and was told we have no machines in Alaska. He told me they have one for Vets at our Hospital here and I told him to get me an appointment. He made more excuses than Carter has liver pills. Pissed me off till I told him I was a disabled vet and told him to get me in with the doc. He still doesn’t believe me. Still living in pain, constipation, in spite of all sorts of diet changes. I also have mesenteric stranding with mild fluids that he cannot explain.

    I have the feeling that whenever i eat a lot of onion or garlic my digestive tract is starting to work better. But at the same time i get a lot of gas coming up and out(farting). I am just not sure if this is a good sign(MMC working) or a bad sign(gas). I feel like i might have methane or hydrogen sulfite. I just can't test it here my gastro interologist doesnt have these tests. I would say more on the constipation side probably methane. When I get rid of meat and also onions and garlic i do feel like my gas and acid reflux is going away. But at the same time it fells like when I keep onions and garlic out of my diet my small instestines are not working as hard as they should.

    So cagey...he dodges almost everything she asks him. Big fat nothing. thankfully there are much better sources on the topic of sulfur intolerance and its relationship to SIBO and possibly SIFO. Dr Greg Nigh
    is an excellent go-to on this topic.

    It's March 2020, a lot of questions he said would be answered in months here haven't been answered yet. The Pimentel group hasn't published anything else regarding H2S other than the machine to measure it. I'm curious why? Couldn't they get data to support this hypothesis?

      Breath testing is a notoriously difficult method for detecting SIBO in terms of reliability. It is an indirect measure with many potential confounding variables and unknowns. Even in the standard method, the hydrogen lactulose breath test, that's been in use for many years and studied widely (by many researchers with no affiliation to Mark) things are tricky. There are many false positives, false negatives, and some researchers cannot even agree on what levels constitute a definitive cutoff for a positive result for hydrogen producing SIBO. All that is to say, my guess would be that he doesn't want to publish prematurely. As per the clinical trial applications listed by his research lab, they are still testing H2S breath tests, so rest assured they are still working diligently to design and prove (with scientific rigor, able to pass peer review) the efficacy of an H2S breath test.
      Also, Covid... who knows what kinds of challenges and interruptions it has introduced in research design, adapted protocols for patient and researcher safety, patient follow-through, confounding factors (what if people in the test get Covid, which includes gastro-intestinal symptoms!!) etc.
      It is also possible that the breath test won't pan out, which would be unfortunate, but that is how science works sometimes... a promising initial result doesn't pan out for any number of reasons, and new studies have to be designed or an entirely different method must be developed. Here's to hoping that isn't the case!

    Is IBS related to SIBO. I am having IBS for many years. Hope there is a cure soon.

    When I had hydrogen dominant, I lost 12lbs in 2 months, and despite having an upset stomach and bloating, I was not starving myself. I am now told I have methane dominant and am not obese by any means, and I was just doing everything correctly for my gut before this happened. I had a ton of stress, and it kept spiraling from there. I am trying to learn the difference between methane and hydrogen sulfide dominant as I’m hoping the dr’s diagnoses me correctly.

      swtpersianangel methane and hydrogen sulphide overgrowths are caused by two different bacteria/archaea. They also typically cause different symptoms.

      The Healthy Gut I’m sorry, I don’t know what distant symptoms means. I had primarily very very bad constipation, like some of the worse I have ever had, and I thought it was worse before. But there has been a couple times where I do need to rush to the bathroom...so I just wanted to be sure my diagnosis was correct because I had the lactulose breath test.

      @@swtpersianangel and what did the test results say? Did it show methane and/or hydrogen gas levels rising? And apologies, that was 'different' not 'distant'.

      The Healthy Gut the results said an excess of methane gas. When I look at the numbers, the hydrogen numbers start I think normal (10) and get higher, to 17, and go down to 8 at the 120 mark. My methane starts at 21 and just continued to increase from there to the 120 mark. But I also took the test in July/August and things have gotten worse since then, and I had a couple times where I had to run to the bathroom.

    Thank you both for your body of work!! I know that the CS low fermentation diet allows aspartame. I’ve heard Dr. Pimentel mention that Stevia is not ideal. What is their position on saccharine?

    Some good things about hydrogen sulfide...
    Stalin was greatly disturbed by pain in the joints of the arms and legs. He believed that it was rheumatism, which he "earned" during his four-year exile to Eastern Siberia. The hydrogen sulfide waters of the vicinity of Matsesta (Sochi) helped Stalin, and the pains practically disappeared. So Stalin had a dacha on Matsesta, and on the Agura River the first structures providing hydrogen sulfide water to the Soviet leader. Later, public concrete fonts and uncomplicated infrastructure necessary for the treatment of people appeared on Agura.
    The number of those suffering from healing grew, but the debit of Agura's hydrogen sulfide source was small. As a result, a new source of hydrogen sulfide water was discovered on the Matsesta River with a higher debit.

      @@TheHealthyGuts my hydrogen on breath test 60 and methane 15. My gi dr. Said only need rifaximin. But i heard it can make things worse when i read reviews. Herbals did nothing. Waste of money.

      ​@@affordableaquariumservices1663I personally haven't heard many reports of it making things worse, and I do read a fair number posts from people who've used it. There is, however, a "die-off" phenomenon where you basically feel like you have the flu during the treatment... but that actually means it's working (assuming your doctor knows your symptoms and agrees ,of course). It is much more common to hear that it worked for a while but then the SIBO came back. Rifax is the gold standard as far as pharmaceutical treatments for hydrogen predominant SIBO.
      However, I'm not sure if your methane test suggests you would need to be treated for that as well... if that is a positive result for methane you will want to treat with both rifax and neomycin according to things I've read. Obviously I'm not a doctor, so maybe your GI doc knows better. Methane is supposedly more difficult to treat than hydrogen, even with these combined drugs. The other thing to be aware of is that after antiboitic treatment, assuming remission is achieved (confirmed with negative breath test), you will want to do maintenance with a pro-motility drug or herb, and also follow a dietary protocol. Many people treat with antibiotics, get a negative breath test, and then call it good, only to have it come back, usually in a matter of weeks or months (I hear 5 months is fairly common, but it can be much less for some, especially if you don't do any follow-up with the recommended maintenance).
      Lastly, if this antibiotic regimen doesn't work for you, or you decide not to do it, there is the elemental diet (you will want to double check that it's appropriate for hydrogen + methane, if that's what you have, since I only research for hydrogen since that is what I'm positive for). The elemental diet is reportedly very unpleasant, as it is two weeks of drinking chalky, unpalatable nutrient goop and eating nothing else. On the other hand, it has an 80% success rate which is by far the highest remission rate of any SIBO treatment. IMO it sounds worth it, but it's clearly not for everyone. Usually, Drs won't recommend it unless antibiotics don't work first or there's some specific reason. As with antibiotics, follow up treatment is necessary to prolong remission, unless you are one of the lucky few who get outright cured the first go around.

      @@Nate-dy9ie thank you so much. In addition to rifaximin she prescribed flagyll 250 3 times a day i guess for the methane. Im also prescribed linzess for motility but im taking it during my 2 week treatment. So you think i should take it after treatment? Also how long after treatment should i retest on breathtest. Im not sure if this is die off. Im more bloated and bm is not as good.

      No. It is normally taken with another antibiotic which I cannot remember the name of. Only when the two are taken do you get an 85% chance of getting better.
