    John Gibbons is a registered Osteopath, Lecturer and Multi-published Author and in this video he will be teaching pathologies of the Hip joint and how they cause pain to the lower back, disc, groin, buttocks like piriformis syndrome and knee.
    He discusses an acetabular labral tear and also a femoroacetabular impingement called an FAI - these are cam and pincer type of impingement. He also mentions perthes disease and osteoarthritis of the hip joint.
    Hip pain can be caused by a multitude of issues like a Gilmores groin or a sportsman hernia, bursitis, SUFE - slipped upper femoral epiphysis, dysplasia, stress fractures, pubic-symphysis dysfunction. Sensory nerve distribution from iliohypogastric, ilioinguinal and genito-femoral.
    In another video John will teach you if the facet joint is stuck in an open position called an FRS (flexion, rotation and side bending) or fixed closed in an ERS (extension, rotation and side bending).
    John also shows you in his other videos to assess the nerve root level of the adductors (obturator nerve through L3 reflex), patella tendon (femoral nerve through L4 reflex), semitendinosus (tibial / sciatic nerve of L5 reflex) and Achilles / plantar (tibial nerve of sciatic through S1 reflex) and a normal response is classified as 2++. Any reduced hypo-reflex (1+) or increased hyper-reflex (3+++) with Clonus and Babinski positive as might indicate some form of upper motor neurone lesion like Multiple sclerosis, stroke, spinal cord tumour or brain injury. Any altered reflex will need further investigation as a possibility of disc or neural pathology. These techniques and more is taught on the Neurological Master-Class at the University of Oxford.

    Hi there, I watched this video and the one after John. Very informative. I have suffered with back pain for 5 years and its made me quit football, running, the gym and now work. Its ruined my life. I continually looked and researched online to find out what's goin on as the NHS gave up trying to fix me. I had MRI, Xrays, Physio, acupuncture, chiropractors, changed my career, bought new mattresses and money on many other things. One day I asked my doctor to check my hips......... There it was!! I have FAI cam impingement and sever OA in both hips. They have tried Hip arthroscopy but it has not worked and now waiting for a THR. I am 35 years of age which is not great but if i can get out of pain maybe i can still have a future. Thanks for your videos, I would love to pick your brains on the topic.

      Just wonderful, I've been looking for "will foot numbness from sciatica go away?" for a while now, and I think this has helped. Have you heard people talk about - Sonictoria Stupefying Smasher - (should be on google have a look )?

    Dear John Gibbons. Ths is the first time that I have heard a satisfactory explanation to what I am feeling. My hip was x-rayed and diagnosed as ostheo arthritic. The pains/discomfort I feel seems to shift from the side of my lumbar spine to middle of my buttock, to groin, to outside of the knee, front of knee, and when doing yoga, especially trying to balance on the affected leg, i feel unstable/unsupported by my hip joint, and with certain rotations, it feel like the femur moves over a bumpy area and clicks. I am 70, and from 4 years ago I had a couple of difficult years, when I stopped doing yoga and other exercises. When I started to practise again, I found my left hip to be rather stiff, and foolishly did kick-boxing for a couple of months, when my hip started to give me these problems. I very much believe in natural cures. Do you think that there is any chance of regeneration with special diet and special strengthening exercises?

      Thanks for the message, your hip will not regenerate as not possible for the articular cartilage, especially once it has become degenerative - change of exercises might help reduce some symptoms, regards JG

    Love your channel. 4 years with knee pain that shift from sometimes left of the patella tendon to sometimes on the right side of the patella tendon. also a tooth ach feeling at the acl region at the top of the tibia. Mri and xray was negative for injury to the knee and 3 months of physical therapy for knee strengthening was also ineffective. My knee doctors in America was not helpful in diagnosing what causes the pain. requested a hip doctor that only wanted to take a xray of the hip. that showed a cam. i requested a mri of hip and lower back and that revealed a herniated disk and a labrum tear. injection to the herniated disk did not relief pain and i am considering surgery to fix the labrum tear and the cam in hopes that it would relief the knee pain.

      Thanks for the message and a lot of people suffer knee pain and the cause is directly coming from pathology of the hip - typically a labral tear is generally involved with a Cam FAI - injection to the disc - mmm....not sure i would recommend that as doubt it was the issue as the hip is the source for sure - good luck and pleased you like my channel. regards JG

    Hi Great video and explanation. I am a 59 year old nurse. My left hip has a "tight gripping pain", groin ache, lower back pain, and when i walked my patella was floating but locked back to position. Pain for a week then went away. Any Ideas?

    Excellent video. My recent Xray and MRI has confirmed hip impingement and labral tear. I feel practically zero pain in my hip but have chronic knee pain 24/7. Knee MRI and X-ray are totally normal. Sitting and biking worsen symptoms. I’m 33 years old. Generally speaking, would you advise someone in my position to opt for surgery or simply work on conditioning and avoiding problematic movements and hope the issue subsides? I know it’s hard to tell without seeing imaging, but I’m stuck on which path to take and any advice would be helpful. Thank you!!

      It is difficult to comment on what to do without seeing you - the FAI of the hip can cause knee pain without presenting with hip symptoms - maybe try a steroid into the hip rather than surgery and see if that helps as many of my patients have opted for that option and has been almost miraculous for some, regards JG

    I had hip replacements in 2010 and for the last 3 months I’ve had pain in my right but cheek that increases when I do stretches that indicate it’s coming from the facet joint. The pain stops me in my tracks and comes and goes similar to a labor contraction. The pain is somewhat constant and flares up into the contraction type of immobilizing episodes happening every time I walk . Turning right leg first seems to consistently trigger the intense pain. I’m normally very active and have not had any limitations or complications with the porcelain and titanium DePuy replacement. When Covid -19 happened my lifestyle became sedimentary by 70% and that’s when the pain started. The bone or joint that I can feel located on the lower outer rear to side of by butt cheek feels somewhat numb on the skin or surface layers with a isolated intense pain deeper in the the low middle of butt. As the contraction type pain grows in intensity it radiates but rarely travels down my leg unless I’m laying down, then the pain can be felt throughout my entire leg and is unbearable. Squatting down to pick up something feels like the hip joint is unstable or weak. Note: I successfully had T9-12 and C4-7 discs removed and fused in 2015 and am not experiencing any changes in how they have felt throughout.

      You have had a lot of surgery - I would get someone like me to check the hip again if possible as the symptoms in the buttock region is probably hip related, regards JG

      Getting hip replaced in 2 month’s and was wondering if osteoarthritis in one’s hip can cause other muscles in your leg to degenerate?Have had hip pain for a long time and has caused a lot of my other muscles mainly psoas muscle to get so tight and week they initially diagnosed me with psoas syndrome but mri says moderate osteoarthritis. I’m wondering after I get this hip replaced Will my other muscles in leg heal and regenerate on their own to where they feel strong and normal again? Thanks you clearly no what your talking about

    Hi John, I fell down from a swing with my hip hitting down from then I feel crackling in lower back and also groin pain occasionally (once in two weeks) , will that be a tear ??

    This guy is literally the best!
    I had 2 hip surgeries when I was 20 with cam lesions and impingements in both (both with infection and cyst). To this day (I’m 28 now) I deal with stiffness, soreness, and pain daily. Is there a way to rebuild from this point or is it something I’ll have to ‘manage’ the rest of my life?

    Hi John...Thank you for this video....I am recovering from terrible triad elbow operation less than 2 weeks ago and since fall developed sharp pains in right buttock.I have told the doctor and asked for x-ray as I have an existing disk replacement. Nobody will listen to me...I feel I have a tear as you suggest... right foot pain and feel bruised under buttocks...What
    shall I do please?

    Thank you so much. I have been having knee and buttock pain lately after Years of HIIT exercise. Pain is not that severe. My question is should I rest completely or activities like walking or biking ok? Thanks a lot!

      Hi, if it was me i would continue to exercise, especially walking as that is one of the best things to do, maybe not so much cycling as the trunk will be in flexion so might exacerbate your symptoms, regards JG

    I have osteoarthrits, in my hip. Prior, I was very active, but always had a slight limp, after I hurt my back. Got over that, but my limp got a bit worse. Physio suggested, I use a cane, now I wish I never started to rely, on walking aides, as I cannot walk without 2 sticks, due to my leg being very weak. I get pain down my leg, various from back, buttock and down my leg. I did not know I had sciatica, yrs back, but research told me diffetent. Even physio who fixed my back, did not take any notice, when I
    described my symptons. Now just trying to strengthen my hips.

    It’s good to be aware that groin, leg and back pain may not be anything to do with the hip. Impingement of the nerves in and running from the lower spine can all cause these symptoms, with the precise symptoms depending on which nerves are impinged. The impingement could be due to bulging disc or discs or arthritis narrowing the spinal canal or spinal joints. The point is if you have symptoms, which can be extremely debilitating, you need to get them properly assessed.

    Jhon I've had bulging disk l5-s1. But also the hip glute psoas and knee feels as if they right leg is tight tilted or raised. Could it be the pelvis causing all this or the bulging itself?

      I also have bulging in same spot. And tilted pelvis and labral tear. And this sacrum pain started 2 years after injury because that hip is constantly pulled

    I have FAI and my dad was diagnosed with perthes. It sucks so bad appearing to be normal and healthy and being expected to push through pain! Several years of treatment for back pain probably just made it worse. They said I have degenerative disc disease too but I don't think my back actually is the issue. Hard to say. Most doctors don't know how to look at the big picture. Sometimes I do get back spasms as well but those are a different pain than in my hip.

      FAI does'nt always cause symptoms but I guess for some (like you) it does and can cause a multitude of pain in other areas - lower back, knee, groin, hamstrings etc - so maybe try a guider steroid injection if you can as will substantially reduce your symptoms and probably the back pain as well. regards JG

    Hey doc, wonderful video u have made.
    I am an professional weightlifter ,
    I had hip labral tear in right hip.
    When I undergone for MRI scan for first time 18months ago it showed tear in labrum. After that I started doing strengthening of hip.
    Recently I have done my mri scan it is showing tear is cured.but pain is not decreasing. What is the solution. Even doctor said the tera got healed u can do excercise.

      Not convinced the labral tear will fully heal and it is still probably still causing the issue, a steroid injection (guided with ultrasound) is next step to see if the symptoms reduce, regards jG

    I’ve had 3 surgical labrum repairs over the years. I now have a ganglion inside the hip area plus a bony growth both are causing terrible discomfort along with osteoarthritis. Waiting to see orthopod, took a whole lot of tests to find the cause.

    I had THR 9 months ago. I have had consistent groin pain. I did PT and am currently biking or walking 2-5 miles per day. The pain was easing, especially after a greater trochanter steroid injection a month ago. But, a few days ago, while getting out of the bathtub, my hip "popped" or something. It was very painful. I saw the surgeon's PA the next morning for an already scheduled appointment. At that point the pain had eased but over the next days up to today, the pain is increasing. It goes across the full front half of my leg at the very top. Now what?

    This is very interesting. I have been having hip, groin, buttock, and knee pain for about 4 years. Recently I went to the doc and we did an xray which found hip dysplasia then we did an MRI and it found "mild osteophytes" as well as bilateral hip joint effusions and free fluid in the pelvis. My joints exhibit hyperlaxity/hypermobility. Would I need to get an MRA to see any possible labral tears? Or does that seem unlikely since it wasn't apparent with an MRI (without contrast)?

      An MRA would be the better option to see a Labral tear and sounds like the hips are responsible for your symptoms, regards JG

      What have your symptoms been. I have impingement in one hip now the other is off. When i lift my leg it feels unstable and it clicks alot.

    Getting hip replaced in 2 month’s and was wondering if osteoarthritis in one’s hip can cause other muscles in your leg to degenerate?Have had hip pain for a long time and has caused a lot of my other muscles mainly psoas muscle to get so tight and week they initially diagnosed me with psoas syndrome but mri says moderate osteoarthritis. I’m wondering after I get this hip replaced Will my other muscles in leg heal and regenerate on their own to where they feel strong and normal again? Thanks you clearly no what your talking about

    I can't stand or walk without pain. I have the sensation in my left groin that something is so ripped and or torn. Hopefully my MRI will show this. Also I have sciatica from my left buttock going down the leg with knee pain.

    I've had surgery twice in my right hip and once in my left for CAM-lesion and labrum tear. After my surgeries, I've had complications. I have bain in my ancles, knees, hip, groin and my back (especially the teres muscles). On the right side my groin pops everytime i walk, and om the left side my hip pops. Also feel pain in the middle toes on both sides. I've taken new MRI scans and they couldn't see anything wrong, my fysio means it has something to do with my nerves. What frustrates me is that I don't get better, or at least it is taking SO MUCH TIME!! I am only 22 years, used to play soccer and jiu jitsu but haven't been able to practice any of those the last 3 years...

      Thanks for the message and sorry for the delay as you can imagine how many messages I receive - it is a shame about your hip and what you have gone through and sadly the surgical intervention is not always a positive outcome. Why dont you request a simple ultrasound guided steroid injection into the hip to see if this takes away all of your symptoms and if it does you know that the hip is the culprit and nothing else. On a positive the steroid injection can last a long time in terms of reducing the symptoms, regards JG

    Mine seems to be getting worse. I've had the steroid injection and guided injection with the contrast (which was quite painful as no numbing was used at the time and ouch that was horrible). I couldn't walk after I'd had the injection. But it eased after a few hours.
    1 week in it seemed to have fixed the pain. 6 weeks later I feel its got worse so no idea what's going on. I'm getting results from my mri tomorrow so I'll find out what's causing this awful debilitating pain 😢

      The steroid doesn't fix the pathology - it will only ease the symptoms for a period of time and naturally if it does reduce, it would indicate a pathology within the hip joint like a labral tear / FAI that requires further investigation and perhaps surgery is the prognosis for long term, regards JG

    Leg-Calve-Perthes is a similar injury to American NFL football 🏈 player Bo Jackson. The team didn’t realize that he ruptured the artery supplying hus hip which ended with Bo requiring a Hip replacement. Such a major loss b/c this guy was 6’1” and weighed 105Kg

    Hi there, apologies in advance for the long comment.
    I have had multiple scans and investigations over the past year for hip pain
    I've always had a clicking hip due after being pushed into the box splits as a child and have had pain ever since
    I've since been told I have a labral tear and something else is showing deep inside the hip and now waiting on an arthroscopy surgery
    I also have developed lower back pain and knee pain in the opposite leg to which they've said I have a slight groove and potentially something else which doesn't require treatment right now but could cause issues in later life
    I'm only 31, have always been active (I ran a half marathon in September 2018 where I felt a pop from my hip) but since then have been in constant pain and getting nowhere
    Is there anything I can be doing/asking as I've been told I'm on the waiting list for the arthroscopy but probably won't have surgery until October-December this year (was told that in November)

    I’m waiting for a hip replacement but 3 weeks ago I had a fall.Ive been on crutches for three weeks now and spent the first two stuck upstairs in my room.Since then I’ve had all of those symptoms that you mention.The pain changes from place to place but never disappears.Are there any instructions I can follow in the meantime as I’ve been told my hip replacement could be up to 12 months away and the thought of being in pain for all that time is something I don’t think I could stand.
    Any help whatsoever would be of great help

    Sir, I am a 30-year-old fit and active male in the military. The onset of my symptoms started after splitting wood with an ax. I’ve had groin pain, flank pain, buttocks pain, and lower back pain that radiates down my leg, and pain in the groin that runs to the side of the testicle. Also some hamstring pain. It is dull, then feels like burning pain the next day. I have had two physical examinations from a urologist and a primary care provider. Also had an ultrasound of the testicles and a full CT scan of the midsection and back. Everything was normal. Pain is still here. What is your recommendation. Thank you very much

      John Gibbons sir, results state disc protrusion with spondylosis. What I was feeling I’m assuming is nerve pain. It’s not terrible. Just annoying. Back doesn’t hurt too bad. But the burning/dull pain down my buttocks leg and into my foot is annoying. What’s your recommendation to manage and fix the issue? Thank you sir

      The pills wont repair the labral tear, ask for a steroid injection into the hip. Will need to be guided under ultrasound and hopefully that will take away the majority of the pain, worth asking as surgery is one of the main options but the steroid injection might take away your pain, regards jG

    I’m 20 years old and I have on and off pain in my left lower back/butt that shoots down my leg. When I mentioned it to my doctor she said I probably had a pinched nerve. The pain has continually been on and off for the past few years I’ve learned to live with it but now I’m starting to receive pain in my inner hip on my right side, on bad days walking turning and sitting down is very painful.
    Could it be something other then a pinched nerve since it’s on both sides ?

      That is the problem with diagnostics, they often do not reveal the cause of your pain or dysfunction. Find a orthopedic board specialist physical therapist to evaluate you, or an osteopath that practices. A lot of things can cause sciatic nerve pain.

      Hernia is normally traumatic related - even though couching can cause one....if your pain is weight bearing and twisting then somebody needs to look at the hip joint to rule out a labral tear, regards JG

      The labrum wont heal as its fibro cartilage and a very poor blood supply, they can settle down though and a steroid can help, regards JG

      Me too. I am 63 yrs old. Came on suddenly. I have pain in my groin, front thigh, buttock and lower back. (Apparently the lower back has arthritis too) I am told my right hip joint has severe arthritis. I got a cortisol injection. It helped the pain for about 3 weeks and then wore off. The dr said it will only get worse! I feel old!

    I need help I've been trying to get help in my town my hips legs back everything hurts and is inflamed I can't feel anything from time to time , can hardly walk and I'm numb in my butt right hip thigh nd foot everything changes from time to time I wake up ok sometimes I take medicine and get worst idkkkkkk I need help so bad and evrrytime I call the hospital they drug me up and send me home. I need someone..... please .... the way all this started was my hip was tight and for almost a month then I stretched and my hip cracked supppperrrr crazy very loudly then right after that I had to go to the hospital and they said I have a herniated disk but so much has happened. I have a knot in my the middle of my chest now . I can't smoke. Sometimes I can't breath , my hip and pelvis feels inflamed, my groin area went numb at least 3 times . I had temporary paralysis at least 4 times I been to the hospital 4 times..... ughhhh....... please someone! ... im scared, I've called the orthopedics 4 times !!!!!

    I definitely have one of the above mentioned mentioned. My groin did something funny the other day and I wasn’t even running hard.
    Never had any issues with my groin but a labral tear in the hip on that side caused my glute not to fully activate. Then my hamstring went soon after.

      I already had both knees replaced
      Got told about 5 years ago I would need hip replacements.
      Over the past 8 months that is coming true.
      But. I don't give up. I swim laps. I exercise the best way I can.
      But yes it's old age wearing out my joints.

    Interesting but need more layman’s terms . I had to constantly look up all these terms in dictionary. I guess this is really only for those in practice

    I need help I've been trying to get help in my town my hips legs back everything hurts and is inflamed I can't feel anything from time to time , can hardly walk and I'm numb in my butt right hip thigh nd foot everything changes from time to time I wake up ok sometimes I take medicine and get worst idkkkkkk I need help so bad and evrrytime I call the hospital they drug me up and send me home. I can hardly sit and I want to cry all the time this is making me depressed I've had all this for almost 2 weeks now . It's hard to shower . I haven't been outside . I need someone..... please .... the way all this started was my hip was tight and for almost a month then I stretched and my hip cracked supppperrrr crazy very loudly then right after that I had to go to the hospital and they said I have a herniated disk but so much has happened. I have a knot in my the middle of my chest now . I can't smoke. Sometimes I can't breath , my hip and pelvis feels inflamed, my groin area went numb at least 3 times . I had temporary paralysis at least 4 times I been to the hospital 4 times..... ughhhh....... please someone! ... im scared

      I'VE been in same mess and pain for 4 months it started to settle after weeks of walking 10 ft then kneeling down and crying with pain then walk another 10 ft and same again each day I got abit more stronger took vitamin D supplements which helped not sure why... I strapped ice packs for hours on different areas which helped even more...now I can walk pretty far but I started running and my groin is killing me.
      I couldn't sleep without pain all day was 24/7 pain pain pain I got depressed too.
      Get walking use ice straps lay on your back or side use pillow between knees if on side, massage your thigh..purchase back support and hip and leg support it helped me a lot, infra red heat light may help to I used all of what imentioned.
