All the ER scenes here are Season 15. Carter needs a kidney transplant because he is in renal failure (one kidney stopped functioning after he was stabbed in season 6, and for the other he got an infection while working in Africa). I hope that helps ;)

Benton's voice sounds so much more mature and older than the earlier seasons! Kind of sad to realise that many of the cast were on ER 15 years!

It's not shown onscreen, it's something that Carter reveals when he returns. And it's not a single incident that causes his kidney to fail.
