How to treat ACNE SCARS | Dermal Fillers

    Acne scar revision at a medical level has a complex algorithm based upon physician approaches & patient factors. Patient’s understanding of the BIOLOGY of scar revision in the context of the level at which scars lie is important.
    .
    .
    Scar Types: Understanding the different scar types is the very first step. Scars can be broadly classified as ATROPHIC (hole) or Hypertrophic- Keloid - raised. They can be divided as to the colour- hypo, hyper, PIE (red) etc...
    Atrophic scars include boxcar acne scars, icepick acne scars, rolling acne scars, & 'divots'. This broad classification can be further divided into others including narrow boxcar acne scars, broad, linear, deep, undifferentiated, mixed etc... same goes for icepick acne scars- pick, pits, deep pits, icepicks, deep icepicks. The algorithm of how to manage is then based upon one's skin type (1-6), as well as patient downtime.

    Skill sets: Revision specialists should be able to surgically operate as well as fill, laser, peel, & operate suitable devices. The order is flexible as there are many ways to the same destination. For example the use of RF #microneedling can be a substitute for lasers.

    Bell curve & biology. Patients sit on a bell curve. This applies to immune response, mental understanding, & other important factors. Some are fast responders, some are slow responders based upon physiology.
    .
    .
    Scar types : Deep vs superficial #acnescars. Rolling, tethered & atrophic scars are much harder to treat cf superficial scars. This video illustrates the concept of volumes, atrophy & thought processes for #dermalfillers
    .
    .
    Age related issues: Loss of volume both dermal, fat, bone CONTRIBUTES to the APPEARANCE of acne scars. Scars do not worsen (if inflammation is absent), you lose the SUPPORT for atrophic scars. No two ways around this- treat age related changes & scars will improve.See previous posts that emphasise this point

    In the majority of cases results are largely predictable. If skill sets & correct application of techniques are met, & if patients understand the concept of support, the vast majority of acne scars / traumatic scars can be improved

    Gosh, how I wished you had your practice here in the U.S.! You’re AMAZING. I have a lot of acne scars and seen quite a few dermatologists but none as knowledgeable as you when it comes to acne and acne scars.

    I'm 64 now and I have had acne all my life. When I was younger people were far more careless about what they said to me about it, even strangers would joke about it to my face and call me 'pizza face' etc. At this age, I have finally grown to accept it and get on; I've lived my entire life with it now from being a teenager, and I'll likely die with it. I have terrible atrophic scars all over my cheeks, forehead and even my nose, with acne spots on top of them. People nowadays don't mention it. I know they're there and even spent my life as a young man not going out in the sunlight as that shows up every blemish,spot and scar i have (I live in England so it's gloomy most of the time luckily ha ha) but I have grown to accept it now. It is what it is.

    Thanks for providing clinical based information on acne scarring. People that struggle with scarring may not be able to visit you, but at least they have accurate information about suitable treatments and achievable results, and will not waste money or time on ineffective products or treatments.

      Im 32 and still dealing with acne. Not so much on my face, but mostly on my shoulders, back and chest :( I have scarring that make me wish I had never picked at them.

    The only realistic dermatologist in the world who talks about acne scars without selling ineffective procedures.. i Lost money for years doing lasers and everything to minimize the scars when only fat transfer would resolve This.. and nobody told me that.. thanks for the vídeo!!

I was seriously going to book an appointment 6 months-9 months in advance so that I could have enough time to purchase my ticket. I've a sister that lives there part time so I figured I would kill two birds with one stone. I'm very careful which doctors I go to because some Drs will push anything just to make money.     I wish you could remove my bellafill filler in my cheeks. I had it injected in my tear troughs 10 years ago and for some reason now at 42 I’ve developed huge granulomas 😢 not sure if it’s because I just had my 2nd child a year ago? I’ve talked to plastic surgeons and none of them will touch my face one plastic surgeon who specializes in eyelid surgery said it’s common for filler to act weird after childbirth because honestly, I never had any bad results until a couple months after my son was born. I feel horribly self-conscious, and I’m terrified of getting hit in the face or heaven for bid getting into a car accident and having the airbag go off I don’t know what kind of damage that would do and I don’t wanna find out I wish I had the money to come and see you. I think you’ve removed filler with the lasers before, I wish there was a way you could work with me on the price maybe video the surgery to teach others what can go wrong by getting some of these permanent fillers because like my plastic surgeon said these granulomas and problems after surgery or only becoming more and more common oh, and I forgot to mention, the lady that injected me is no longer in business. Her company went bankrupt.

      Or actual plastic surgery as in a facelift, if one gets older. Not recommended to use fillers indefinitely when facelift is better.

      Try Derma stamp with numbing cream, Be well sterilized before use in one go and desert used needle. Cost only 40-50 $ per session done by ownself. Rejuvenate with Vitamin E and B3 and Hyarulonic acid. This will heal with fews day. You can see tutorial of derma pen and stamp in youtube. This is the cost effective method of scar fixing.

    i Have a scar on my cheek just above my laugh line, goes in the same direction as my laugh line ,is just a lil above it, it becomes more deep when i smilee!! i was also wondering if dermal fillers will help ? if not in curing the depression, will they help in making my scar less deep when i am not smiling

    great video doctor, i have a scar on my cheek which becomes very deep and noticeable when i smile. i want to have a smooth smile and hope fillers would be a good option after a subcision.
    do the scars go back to normal after 6-9 months? or does the filler show significant improvement?

    Dr. Lim, what lasers are you using for acne scars on ethnic skin? (I am dark skin Latino) I wanted to go with the CO2 laser but my derm said I can not have that treatment because of my dark skin but she said that the Pico laser would be great. But then I saw your video on how the Pico laser clearly won't help for scaring so, what is out there in lasers that will work great for dark skin folk with acne scars? Thanks in advance! :-)

    I have to be very very diligent with my skincare routine from years and years of cystic and bacterial acne.
    My skin now is clear and very healthy due to lifestyle and a good skincare regime.
    I’ve found massaging in products like a flavonoid and retinol will help nourish the skin (it’s the only thing that’s I’ve found worked) and occasional laser treatments and microneedling.
    I’m super interested in clear and brilliant, any thoughts on that? Can it be done on the neck to address neck lines?

      @@gabriellanoelle1606 Hi Eddie! It took a long time, but I cut out dairy, refined white sugar, wheat/gluten, and animal products for a while (I'm eating eggs, chicken & bone broth now). I also use Nerida Joy's skin care line called Educate you skin...she has a ton of videos and her line is really great.
      Just remember, good skin starts within..if you have some skin issues that's a sign something internal is going on:)
      Dairy is a big cause of acne, but if you're suffering from scarring a good skincare regime will help tremendously. If you email the team at Educate your skin they can assist you with the right products! Good luck!

I want to give an update of what I am doing since November when I stopped my treatment on the clinic,I do some DIY stuff at home and surprisingly it did helped on my scars how I wish I knew all this before so I didn't spend a lot of money, so here I do TCA, derma pen, once a month then apply rosehip oil, MSM cream and mederma I've seen dramatic improvement on my skin as compared. going to some clinic.       Eric Castle , doesn’t matter. He won’t see anyone anymore. He’s not taking new clients but I heard acne.org is a great place to find a local good derm and it’s not bias. Only people who have had real experiences. No derma are even allowed in the site. Hope this helps. Cheers!

    Hi! I have an appointment next month to do this. I am a perfect candidate, as you say, my cystic acne scars are getting worse with age and are very deep. I was just wondering what is your experience with the longevity and product used for this kind of procedure? They will use Juvaderm Ultra and said it could last up to 2 years in the scars as opposed to 9-12months in normal situations. Thanks for the great videos! Xo

      if ur sill have acne esp cystic, you need to get on the Vitamin A derivative Acutane, it will dry up the acne for sure.

      @@christinac4818 I did. Best 600€ spent of my life. Its been more than a year and it did go down a bit but its still not noticable. I did it with an experienced prominent plastic surgeon in Paris, I wouldn't trust my face to someone even if it's for cheaper.. The procedure itself is unpleasant but not painful. I think in combination with laser for more superficial scars it can make your face almost flawless. I just don't have the budget so I am happy just with that also! Hope it helps.
