  • So many people ask me: "what does a TMJ orthotic look like?" OR "where can I order one of those?" Well, a few things to keep in mind - first, it's not so much what the orthotic looks like as it is the unique prescription built into it. This prescription is UNIQUE to EACH INDIVIDUAL - similar to the prescription built into corrective eye glasses. Just like all different types of eyeglasses, and orthotics and look similar to one another (or can even look like a nightguard!) but what sets them apart is the prescription built into them.
    A skilled and experienced TMJ practitioners knows how to find this unique prescription. In this video, I will be showing you the different types of TMJ orthotics I use most commonly in my practice, with which I celebrate an over 90% success rate!

    You need to put on a clinic! I’ve been to a few Dr’s and none have come close to explaining things as well as you do. I am incredibly thankful for all HCP’s and would never disrespect any. But some experts like yourself are in a class of their own.

    I was fitted for a prescription custom made orthotic by a neuromuscular dentist in April. I have had it adjusted twice and am now 95% pain from TMJ. I was in severe pain from December until April, ear fullness, constant ear pain in both ears, face pain, hard to open my mouth very wide, could only eat soft foods, and it even hurt to brush my hair on the right side. I’m not saying this will work for everyone; however it did for me. It was expensive, but you can’t put a price on your health.

      @@dazzelpebbels1207 The ear fullness lasted around 6 months. It was there day and night. It went away gradually. It started to go
      away after I started to wear the orthotic and thankfully hasn’t returned.

    My TMJ doctor has me wearing the mora during the daytime and the ARA during the night. He said I will wean from the daytime one in a few months. My jaw has never locked though- I hope these are the right appliances for me. The daytime one is really hard to talk with.

    I think I watched your video that was on night guards and how they sometimes make TMJ worse. Wish I had known that before I spent $$ a few weeks ago having one made at my dentist office. I tried wearing it for about a week before I noticed my TMJ symptoms were worsening. Now I can't use it and I don't have the $$ to have a one of these speciality guards made or even go to a specialist for TMJ...😔 I've suffered from Tinnitus since childhood. Used to be off/on but since I've gotten older its an all day, every day thing. My TMJ symptoms seem to make my Tinnitus worse when it flares up. The pain is sometimes all around my head and down the sides of my neck. When its at its worse my scalp is sore to the touch. I hold a lot of tension in my face muscles. I try catching my self when I'm doing it but being a nervous/worrisome person it's hard. I clench not grind that's why I was hoping a guard would help...💔

    Does the TMJ appliance differ from what you would prescribe for bruxism? I ask because I have both but only bruxism treatments are covered by insurance.

    I had a consult from an ortho that recommended I get an MSE to break my palate and expand it. I am starting to see less pain from wearing an occlusal splint (flat plane) although I still have popping maybe once a day. I'm really nervous about doing anything permanent and that invasive. Is splinting for life really an acceptable option? I'm being told it's a band aid, but I don't feel good about the other options being shared.

    Hi mam, i want to know that how the emg tracks muscle activity...basically what the instrument reads in order to determine the different facial muscles activity.............
    .for eg .if someone's massester muscle reading is say 50 on k7 system ...does that means it is tight? or it is over-stretched???....please answer

    What questions are good to ask to make sure our drs can successfully treat tmjd? I’m getting impressions done in a couple days and a tongue tie release. From there I’m getting essentially “extra teeth” put on top of my back bottom molars to move my jaw forward. What questions should I be asking to make sure this is the right move.

    Hi Dr. Mistry! I told you how im starting my treatment with Dr. Hitesh Patel now. This is my 3rd day wearing my orthotic but im a little worried now because it doesn't have those indentations like yours do. Also it doesn't go on my front teeth at all like yours does. He called it a TMJ rescue appliance but I read my paperwork he gave me and found that it is a mora orthotic. But another reason im worried now is because im having problems with a closed lock and you said you use the ara orthotic for that right? It is on and off for me and it's been a little while since it has locked closed, so I was wondering if you only use the ara orthotic for your patients that are permanently locked closed? All I can say about mine is that it fits but not perfectly. But while im wearing it my jaw doesn't click or pop as much and that I haven’t locked closed yet. But when I take it out I still click and pop. I don't expect it to solve my problems in just 3 days but im worried now because the treatment is very expensive. Sorry for the long paragraph. I absolutely love your content btw! ❤

      I don't use the ARA for every patient of mind that is locked closed - only if we are not progressing in treatment. As for your specific treatment, I can't really advise you much over the internet. Sorry and best of luck to you!

    Awesome video! It made me so happy! Quick question? Can I use both sometimes at night I feel like my tongue wants to jump through my teeth and I end up biting my tongue a lot every night.

      can you use both what? I make a TMJ orthotic to fit against one arch - upper teeth or lower teeth - not both.

      My patients don't wear the TMJ orthotics when eating. I use K7 and TENS therapy in fabricating my orthotics.

    Would you recommend a tens machine? I'm about to break. The past 5 year's, I've said, I can't survive another year in pain like this. I'm at the point where even 6 more month's is not doable. But I've done it all. Perscriptions, bite guard, heat, ice, physio, acupuncture, and massage therapy with someone specialized with tmjd. I've used CBD numbing creams, tylenol and advil. Is tens machine or upressure something you would recommend?

      The TENS unit I recommend is the one I talk about in the video linked below. Having said that, it's not common that the TENS unit will eliminate the pain entirely.

    When I first got Invisalign they suggested I have jaw surgery to fix my jaw alignment. What is the difference with this orthotic and Invisalign? Why can’t Invisalign realign my jaw like this device and would it be wise to find a neuromuscular dentist once my Invisalign treatment is complete?

    I have never had an orthotic that I could sleep in because when I clench into them it hurts and wakes me up. I have had regular OTC guards of every type and failed so my dentist fitted me with a custom ($500) guard intended to prevent tooth damage as she isn’t trained to treat TMJ. This was the worst and causes a full blown flair whenever I try to use it. Is the type you show enough different to make it worth trying?

    Hi Dr Priya
    I have gotten my orthotic in April. I consistently found the need to have my upper teeth come in contact with the orthotic but my doctor advised that they should never be in contact. However, when i have the contact I feel much more comfortable.
    Are patients usually advised to never feel contact from their upper teeth to the orthotic?

      I advise my patients to have their teeth gently contact the orthotic in the grooves/indentations built into it for the upper teeth.

    I recently went to a TMJ specialist cause of horrible pain in my jaw. He did a CT scan and actually gave me these splints here, but they all just gave me flare ups and I feel like made it worse. Are there other options besides splints?

      @@theTMJdoc Yes. I was emailing him a lot. He tried changing them, but every time I put them in my mouth it was immediate discomfort. He told me I’m just a tough case. But they both told me that I should only experience relief from my symptoms, and that if they hurt me to take them out. But now they just want me to keep trying even though they hurt me so bad.

    Hi Dr, can you eat with the splint on? My doc tells me not to remove it while eating but it completely closes my oral cavity and i can only slip some foods inside and there isnt much room for my tongue to move??

    I got tmj arthrocentesis done as i got my jaw locked couldn’t open it more than one finger after the treatment i can open upto 3 and i was given a splint to wear in lower jaw idk which type is it they just took impression on wax and looks like just a copy of my lower teeth set, my question here is can i wear it for lifetime or not ??

    Do you think that it will take longer for a custom made orthotic to work for those who have been in chronic pain for 8years?
    Also, is it normal to experience some throbbing while wearing it because the bite might be changing ?

    What is the difference between a neuromuscular orthotic and an anatomical orthotic? Can one over the other reposition jaw joints that are posteriorly positioned?

      I don't know what "anatomical orthotic" refers to. A neuromuscular orthotic is made via measurements of where the muscles are most relaxed.
