    • @ThinkYourHealth

      It could be.One of the things your Doctor can do is orthostatic blood pressures.These are blood pressure readings done in a supine, sitting and standing position. In this way your health care provider can figure out if the dose of the medication has to be changed.
      Obviously there are a lot of reasons that can cause someone to feel light headed including cardiac & neurological reasons, however, your health care provider knows you and your medical history better so they rule in or rule out other reasons.

      It is appropriate to start at the lowest dose and titrate up as you asked in your question.The question would be why your BP is higher(looks like your diastolic pressure.Is it because there is too much salt in your diet, it it being overweight, is it stress, is it because of alcohol consumption, is it because of the lack of exercise and physical activity or even side effects from medications such as NSAIDS.The most important thing one can do to control blood pressure is diet and exercise: i/e lifestyle changes.

    • @ThinkYourHealth

      For short periods the medication should be safe. However, with my patients, usually those with cardiovascular issues I do get a baseline EKG which helps calculate QT.If I need to adjust the dose this helps make that decision.
      These are the main side effects with percentages for fluconazole in parenthesis according to uptodate.com a peer reviewed journal site: Cardiovascular: Angina pectoris (6%), atrioventricular block (1% to 3%), bradycardia (≤10%) (table 2), dependent edema (5% to 6%), edema (5% to 6%), exacerbation of angina pectoris (1% to 3%), hypertension (1% to 3%), lower extremity edema (1% to 3%), palpitations (1% to 3%), peripheral edema (1% to 7%), peripheral ischemia (≤1%), peripheral vascular disease (1% to 3%), syncope (≤8%), tachycardia (≤1%).Hypotension also usually less than 20%.

You mentioned the list of side effects related to this medication such as fatigue, if that is the case woukdn't it be better to take it at night than morning? Yes, the medication can be taken at night time as long as you get the ok from your health care provider and it is taken consistently at night time.In that way you can also avoid a sudden precipitous drop in blood pressure and side effects such as fatigue or dizziness.

    • @ThinkYourHealth
      @ThinkYourHealth  8 місяців тому

      As I do not know the details of your hospital visit I can only take an educated guess.The Bisoprolol was prescribed as you had high blood pressure but also protective for future cardiac events.It can also be used to help with chest pain.Not sure if you are maxed out on your other blood pressure medications, however Bisoprolol is a beta blocker so it is in a different family of blood pressure medications compared to the ones you are already taking.Given your presenting symptoms(chest pain with radiation to the jaw) a cardiac cause should be in the differential pending further testing.

      Yes, it does raise it.I do think you have to be conscious about the decisions when you take in food and restrict or avoid certain food types.You want your potassium level to be around 4 mmmo/l, that is not too high and not to low so that your heart continues to get the benefit of adequate potassium.

      Drugs start working within hours ( like this drug).However, to be truly effective to reach its full "potential" it usually takes a couple of weeks.This has to do with the half life of drugs.Pharmacologically, this is referred to as steady state.It takes somewhere between 5 and 6 half-lives for a medication to reach steady state.This translates to a couple of weeks for some drugs.

      Hydrophilic Beta Blockers require renal adjustment
      Examples: Atenolol, Bisoprolol, Nadolol, Acebutolol
