    As an Argentinian, this hits really hard. It's a systemic failure at all levels. I am a Semi Truck Driver and take my job more seriously when I do my pretrips than these pilots did being in charge of an airliner with so many souls on board 😔

      @@adammcdonald798 I'm not 100 percent sure, but I believe they oversee licensing and regulations, sort of like the FAA in the US. 10-4 Driver 😎👍

      @@adammcdonald798 Back in 1999 the Argentine air force was in charge of overseeing all aspects of the civilian side as well. In 2007 that responsibility was transferred to the newly formed ANAC (administración nacional de aviación civil)

      There is a movie (with a former captain of LAPA as a producer) about it in which it diggs deeper into that. The plot its rather simple but the visuals are amongst the best as it didnt use any CGI only 100% real cockpit action (again the producer/screenwriter/director protagonist was a LAPA captain at the time) The movie: WRZ or wiskey romeo zulu

    There is a movie performed and directed by a former B737-200 capitain who´s name is Enrique Piñeiro. The movie is called "WRZ"; it´s in spanish and free on YT.
    Enrique was one of the few LAPA members who acted with profesionalism; due that, he was "grounded". He wrote a report saying if the company does nothing to improve maintainens and pilot skills "the outcome of an accident is not only highly probable but almost unevoidable". This report was translated and reproduced either in The Washington Post or The New York Times. One month later the accident ocurred.
    Now, he owns a B787 "Dreamliner" an does humanitarian flights.
    I hope someone reads this coment, watches the movie and finally realizes how greed and stupidity can lead to death.

    I worked in LAPA as an errand boy in 1990-1991, by then a very small airline that had only two daily flights to Montevideo in Uruguay which is half an hour away from Buenos Aires crossing the Río de la Plata. Other destinations were seldom served, sometimes every two weeks. It operated two SAAB 340 and didn't make much money, only having a handful of employees. Around 1996 a lot of used aircraft mostly from Eastern Europe were acquired through a rather obscure deal that allowed the airline to become the first low cost in Argentina, flying most Latin American destinations plus the US; it was able to compete successfully with Aerolíneas Argentinas and some other regional air companies. But it was vox populi that planes were being barely serviced as to cut costs. When the accident happened the whole country noticed everything at once, but many (myself included) knew it had been coming for a really long time. There were even some versions that spoke of sabotage, but they were never considered although it would have deserved some attention.
    The owner Andy Deutsch (Czech born) always denied his responsibilities; he belonged to a family that had founded the supermarket chain Tía, up to the 1970s the biggest in Argentina. He was acquitted of manslaughter in 2010, but ironically he died along with his wife when the light aircraft he was flying crashed in 2014 in the northern BA suburbs. Many saw that as rightfully deserved.
    PS - for those who don't understand our Rioplatense Spanish (almost an admixture of Spanish and Italian): the pilot and the stewardess are flirting and at a point they say that "he being married and she having a partner, they lost an opportunity in the past" but it can be implied they want to date.

    So many simple mistakes and it killed so many people without the plane even taking off. How was the pilot even let to be in that plane with his license already expired, not to mention that the airline has specifically told pilots to ignore alarms? I'm glad they were charged, this was reckless on all fronts.

      The whole system was corrupt. The incompetent pilots being allowed to continue in service was just another manifestation of the system teetering and falling over the edge......I am just wondering if they are still allowing pilots to ignore warnings, and more importantly, maintenance not being done on a regular basis.

      DL 1141 at DFW. NW 255 at Detroit Metro. Both crews neglected to extend their flaps. Delta CVR recorded almost the same type of conversations before taking off. Both crashed immediately after rotating. CRM (Crew Resource Management) & cockpit discipline is the only way to prevent these needless fatalities.

The attitude of the airline requiring error messages to be ignored and presumably poor training and certification of the pilots were major contributing factors. An accident waiting to happen. There is absolutely nothing good about this flight. This airline needs to be shut down.

      I keep thinking that even the Flight Attendants were distracted. Also if the passengers were people who had not flown before. How did no one notice the flaps were not down? Or not hear the normal sounds you hear when they are extended?

    There is a good reason why a "sterile cockpit" should be observed before takeoff and landing. This demonstrates perfectly why.

      Sometimes they can be taken too far, particularly when the flight is delayed and result in stress. They should equally apply to configuring the aircraft and doing the checklists, rather than just during takeoff and landing.
      Most flight crews take their job seriously and avoid lengthy personal conversations at all times, rather than just during takeoff and landing.
      The pilots on this flight did not have the correct personality or mindset to fly an aircraft and it should have been fairly obvious to the airline.

      Eastern Airlines Flight 212 is also a good example of why a sterile cockpit is necessary during landing. Stephen Colbert lost two of his brothers and his father in that accident.

    I happened to be in Buenos Aires city during this accident. I was driving (a Motorcycle) though a nearby highway and remember seeing fire near the airport, and we all kinda stopped on the side of the road. There's a movie about this accident which gives a greater detail (albeit with some "artistic license") about what lead to this accident. tl;dr: corruption and lack of maintenance and training by the airline.

    Finally, LAPA 3142 made it to TheFlightChannel. 👏🏾 Austral Líneas Aéreas 2553 would be a great addition to the channel...kind of the same maintenance "mistakes" (not an irresponsible pilot on that accident though).

      I think you would be surprised at the expertise of non U.S. or European trained pilots. Coupled with ‘questionable’ training, monitoring along with questionable maintenance.

    The fact that 2 of the video's "chapters" were solely about the incompetency of the 2 pilots really says it all.
    RIP to all the passengers, 2 people on the ground, and 1 other crew member who perished in this entirely preventable crash.

      He doesn't deserve any respect. The corruption exceeded the accident and all the judges assigned to this case took more than 10 years to get a verdict and, by that point, all the cases went down due to prescription. Only very low level LAPA managers where sentenced to very low penalties, but even them ended up being free of charge (yet again, prescription).
      Call it karma or irony, but one es the executives of LAPA that was accused of the tragedy died with his wife in a private plane crash around ten years ago (he was the pilot). The reason of the accident was the lack of proficient to control the plane.

    Ya gotta remember the 'keel me' items (the checklist items that'll put you on the evening news if you forget them) and double check 'em just before you push the throttles forward. There's usually just a couple of 'em. Big planes or little planes, it's all the same.

    Even though you think it might be a spurious warning, I can't imagine not aborting the takeoff or, as a bare minimum, looking at the flap setting when you get a config warning. This has got to be one of the most inexcusable crashes I've seen.

      Agree. The config warning should even have been picked up prior to takeoff if the pilots had quickly advanced and retracted the thrust levers to check it, but that might not have been on the checklist back then. Anyway, when the config horn came on during takeoff, it's beyond me how they didn't immediately look at the flap setting.

    I wanted, for a long time, to let you know about this flight, but figured you'd find about it eventually. I was a kid when it happened but it has definitely left a mark on most of the people, it is to this day the worst aviation accident in my country. Its combination of corruption and negligence led to an event which was so easily avoidable. As some others commented, there's a movie called Whisky Romeo Zulu, as in LV-WRZ, by Enrique Piñeiro, a former LAPA pilot who knew this was going to happen eventually, and shows with great detail the inside of what led to this.

    There is a movie, called Whisky Romeo Zulu (for the registration of this airplane, LV-WRZ). The plot and script was written, and the movie was directed and produced, by the same person: Enrique Piñeyro, who was a pilot in LAPA who sent a letter to the leadership saying that if the airline kept operating this way, an accident was not only possible but unavoidable. In this movie, where h plays himself, he tells the story of the events that led to this accident, the culture under which they were operating, and the disregard for any degree of professionalism and safety. The mistakes done by these pilots were not really the primary cause of the accident, maybe the direct cause, but not the primary one. Something like this bound to happen in this airline, to this or another crew. Because they were all operating in the same way. It just happened to happen to this crew.
    The movie is highly based on the true story and Enrique's experience. But it also adds drama to make it more entertaining.
    Do watch it. It is available in UA-cam for just a couple of bucks and it is a VERY GOOD movie.
    Especially for the aviation fans, it has excellent real footage of airplanes taking off, flying and landing, taken both from the ground and from the cockpit.
    I watched the movie at a special event where Piñeiro was available for Q&A after the show. One of the attendees asked him "How did you manage to capture these close-up shots in a live airport, and with yourself sitting in the cockpit and handling the controls of a real 737 in flight?". His answer? "I plead the 5th".

It is in Spanish and it does have subtitles in English (the paid version in UA-cam movies which is just a couple bucks, because there is also a low quality "pirate" version as a general UA-cam video that doesn't have subtitles, I don't recommend the free version for several reasons: It doesn't help the guys who did the movie, it doesn't have subtitles, and you will missing the top-quality excellent aviation scenes.

    There's a good movie called Whisky Romeo Zulu, acted and directed by Enrique Piñeyro who was actually a LAPA pilot and had denounce all these irregularities, for that he was sanctioned by the airline and ended up quitting. The movie is actually about this accident, showing how and why it happened, It's very interesting to see how bad things were at the time. He also directed Fuerza Aerea Sociedad Anonima, a documentary about the (lack of good) air control in Argentina in the late 90`s.

    I am not even joking, I am not a pilot, but I knew from casual x-plane simming, that the master caution, normally means flaps not set correctly,
    it is beyond obscene that pilots were instructed to ignore warnings.

      I always sit over the wing and watch the flap settings like a hawk. What would happen if I noticed no flaps just before takeoff? Die with everyone else of course (imagine the flight attendant humoring me upon pointing it out).

      That's because you are not from Argentina. Unfortunately, that's how we live, been living for a long time now. Corruption is an understatement. People's lives are just "contingencies" in a paper. If it's cheaper to not mmaintain and pay damages than to maintain, that's the way. And we are so used to it that we don't even question anything anymore. I'm sure the pilots were used to it too. Those 7 who were convicted were just the scapegoats. Nothing has changed. 🤬

      At the time false alarms were very common atthat airline because lack of proper maintenance. That's why they ignored it thinking nothing was wrong.

    This is one of those crashes that makes you shutter at the arrogance of two people who don't take their jobs seriously. I'm not a pilot but I know it's one of the most basic elements in takeoff....FLAPS SET.

    So much negligence. From the pilots smoking and ranting instead of performing their duties, to the airline instructing pilots to ignore alarms and dispatching faulty aircraft, I am disgusted by all the circumstances. Thank goodness LAPA is no longer in business.

    Reminds me a lot of Eastern Airlines Flight 212 where the pilots talked about things from politics to used cars and weren’t concentrating on their jobs. 72 people were killed including Stephen Colbert’s father and two of his brothers.
    Edit: please be respectful. It’s okay to dislike Stephen Colbert, but no one deserves to lose their family members in tragic accidents like this.

    Thanks for providing the cockpit recording. It explains so much! Listening to the recording, it sounds as if the pilots & other person I heard on the recording (woman...purser? or stewardess?) were at happy hour having drinks. It's so sad, but those two pilots were an accident waiting to happen.

    Just WOW, is all I have to say to this. All those innocent people died...and didn't have to. I'm not a pilot or ever been trained in the field, but even I would know, when the configuration horn went off, to immediately look at the flap settings. So many aircrafts and people have fell victim to this over the years.

    Very sad, RIP to the poor passengers, cabin crew and the two ground casualties. I do hope the airline doesn't operate anymore... Even sadder that Spanair 5022 crashed with exactly the same problem (takeoff with no flaps) several years later.
    Minor thing but, I'm appalled smoking in the cockpit was still allowed somewhere in what's basically already 21st century.
    Keep up the good work, these little known incidents/crashes are just the type of content I enjoy seeing here, especially when some other well-known channels haven't covered them yet!

    Doesn't matter the airline told them this or that about warnings in the cockpit. Fly the plane with diligence. But you can't really sue dead pilots estates without blaming the airline culture. They were impaired, distracted, and too anxious. Pretty close the most bizarre story yet.

    Even with all of the unprofessional behavior, one would think that an unidentified alarm in the cockpit would make them reject the takeoff.
    I’m anal about following the checklist, and I only fly a 172! How they became and survived as long as they did as commercial pilots is absolutely astounding.

      The implication in some reports is that the company had such a bad culture for maintenance and procedure, that alarms were entirely common, and so ignoring them became second nature.

    There must be at least 10 incidences exactly like this one presented on various air crash channels on yt. I keep asking why there isn't a mechanical or electronic lock that prevents advancing of the thrust levers to take-off position while the flaps are retracted. Advancing the thrust levers to take-off power with the aircraft on the ground only happens when a take-off is imminent. Therefore it should be easy to install such a lock. All you'd need are two sensors (both of them already installed 1) Is the aircraft on the ground? 2) Are the flaps retracted? As long as both are positive the lock is activated. (Take-off power setting is very different from the power setting used for taxiing so there is no chance that this lock would prevent normal taxiing). This is a much more efficient solution than a bleeping take-off config warning horn, that might simply be ignored. I'm asking myself whether such locks haven't been installed because doing so sending a clear message to all pilots: look, we trust you but not that much. So we make sure you can't f up. A message that might bruise the ego of some pilots.

    With stories like this and the 70's Potomac crash in my head, I settled into a 737 dometic fight in my country Nigeria which after boarding was completed proceeded to taxi out to the runway with the flaps still stowed and not extended as usual. Lucky that I make a habit of sitting behind the wings and immediately noticed this. A steward was busy nearby and I quickly signalled him across, and said in the most authoritative and knowing voice I could muster that he should inform the pilot he was yet to deploy his takeoff flaps.
    Thankfully, the man did as I bid using the phone intercom at the back and shortly after came the grinding sound of flaps being deployed. We had come to the holding point by this time and there was no telling whether the pilots had forgotten this critical step or not. I was only thankful that I took the step I did and did not just sit back to watch.

    This accident is similar to that of Delta 1141,although the LAPA one turned out to be more lethal because,at the end,it ended up colliding with a natural gas box.

    LAPA is as much at fault here for allowing these two pilots on an A/C knowing their personal issues and deficiencies as well as "ordering" pilots to ignore vital safety warnings emanating in the cockpits and supporting substandard maintenance practices. As much as the US has some aviation issues I see and hear more things like this with foreign carriers. So sad this many innocent folks perished in such a horrific way because of incompetence.

    Besides fuel on board, aren't the flaps settings the most important checklist item when taking off? How could any pilot forget the flaps if they want to FLY, although it seems to happen often.

    This is one of the best examples of how a bad company culture wins over good personal culture. An airline willing to operate irresponsibly will always find pilots willing to play the game. The good, professional pilots are excluded by this culture, either because they leave or are fired or because they adapt to the company culture.

    The first reason was that flaps weren't set but the real second reason is LAPA told their crews to ignore warnings because they were negligently not maintaining the aircraft.

    This is one of the two crashes that make me really angry. I've flown into and out of Aeroparque, the airport in this accident. Aeroparque is a small and congested airport only alive because its in the middle of the city.
    The airline did not survive the crash. Only good part of this accident.
    Entirely avoidable crash.

    There’s a saying in aviation, “there are no new accidents…”. This fits that saying. August 1988, Delta 1141 crashed on take off because the crew didn’t set the slats & flaps. They were too busy talking about topics non-related to the flight, violating the sterile cockpit rule, and just gave lip service to the checklist.

    Northwest Airlines Flight 255 crashed at Detroit Metropolitan Airport in 1987 for this same reason: failure to set the flaps for takeoff. The pilots were not distracted, and were focused on their check-list but nonetheless they overlooked that item.
