  • Melasma? Hormones & Skincare. Recently, I discovered that I am a carrier of Factor V Leiden (a blood clotting disorder) and so I had to change my contraceptive. Over the past 2-3 years I have been struggling with my skin - dark spots on my forehead, on my cheeks and acne. I thought it was possibly due to changing and then going off of birth control. I thought the switch might have caused my melasma and acne but it also adversely affected my mood, and so I started trying to learn more. I reached out to Dr. Hirsch, a plastic surgeon, to learn more about melasma, my skin, and to find ways to treat the melasma and then Dr. Mama Jones, an OBGYN to learn more about birth control and the potential role it could play in the onset of melasma.

I applaud you for talking about conformation bias and importance of digging deeper, when people self-diagnose and/or get diagnosed by professionals. This should be discussed so much more! Being critical is important, but being critical means you consider the information that disagreess with what you know and you don't just disregard it.

I have extremely light skin and no moles (not even one) so I’m always getting comments and warnings from my dermatologist like you got in this video. If I go in the sun I look ridiculous with the gear and the thick white sunscreen, but it’s worth it! I spent my childhood with severe sunburns because we lived on a lake, and between that and hormonal issues after a hysterectomy, I am definitely benefiting from this series. Thank you! Can’t wait for more!

I can soo relate to going to see a well known Dermatologist for my mild acne and melasma and having her tell me to go on Accutane for two years was not what I wanted to hear. It can be soo disheartening dealing with some "professionals" who give you no answers and having it be so damaging on our self esteem! Looking forward to this series! 🙌🏻

I worked so hard to get my cystic (hormonal) acne under control, and introducing birth control into the mix more recently has been a struggle. I actually stopped it because I couldn't tolerate the cysts starting to come back after all that time; they caused me so much emotional pain and I never wanted to be in that spot again. :( Thank you for starting this series.

Thank you so much for this. I'm turning 36 this year and just stopped taking estrogen birth control last year. A lot of stuff in my body is changing and it's driving me insane. I guess that's the whole point here, it's having obvious noticeable physical effects on my body but it's mostly effecting my mental ability to cope with daily life simply due to that fact. Nobody else on UA-cam has talked about this yet that I have seen, so thank you.

Thank you for sharing your experience with your dermatologist! I have alopecia, which isn't life threatening but it absolutely affects my self-esteem. I've gone to multiple dermatologists that have told me I'm fine, I'm not dying, it's no big deal, it's just hair, and hearing that from them had been so disheartening. I understand that there's nothing medically that can be done but cosmetically is what I'm concerned about! I'm glad that you were able to find a suitable, capable professional that not only can help you but who will also listen to your concerns ❤️❤️.

I also have Factor V Leiden. Found out after my sister had 2 miscarriages and had to give herself shots of heparin & Lovenox in her stomach everyday to be able to have my neice and nephew. I and several other family members got tested and are all positive. I would have to give myself shots in the stomach everyday too, if I ever want kids.
I also don’t get a period because I have PCOS. So, I’m on 2000 mg of metformin daily and just started taking progesterone too. So far, so good. I always have to remind doctors all my issues, kinda annoying. A hematologist is good to have.

It's caused by big fluctuations in hormones not solely an increase in estrogens. Remember when you consult with someone who went to school and is an expert they will see the problem through the restrictive lens of their individual expertise. An obgyyn is going to see a lot of estrogen related melasma, its common during pregnancy.
An endo would have told you other hormones can have this effect, and not just sex hormones - eg thyroid hormone. Where, when, and how quickly it comes up are good clues to find the cause of an individual's melasma.

This sounds interesting as someone who's had to go in and out of hospital all my life for physical health issues as in my views have always been to do my own research(cos the 80s and 90s NHS) and ask/listen to specialist and not listen to hearsay. I just don't understand how people find it so easy to listen to non-medical advice but it's interesting to see I'm probably more conditioned by circumstances so it sounds odd to my ears.

As a kid, I remember seeing the melasma on both Grandmothers. They called them "Age Spots," and "Liver Spots." :( They felt badly about them, as if it was an indication of their liver health. My face and arms started "revealing" these spots in my 20's. I don't let it rule my self esteem, but I have decided this year I am going to do something about it (photofacial? microdermabrasion?) not sure what. I look forward to learning more through you, Kati. Thank you for all you do 💖

I have one type of the Factor V Leiden disorder. Did you see a hematologist about yours? You said you were a carrier, not that you had the disorder, so I'm curious. My hematologist told me there are actually 3 ways to "have" it. You can be a carrier, you can inherit the disorder from one parent or you can inherit it from both parents. So it gets progressively worse. I have the disorder where I inherited it from my mother. However, I have 3 older brothers from my mother's 1st marriage and they all inherited it from both of their parents. So they have the most severe form and have suffered from blood clots. So you might want to verify where you fall on the spectrum. As far as birth control, because I needed to be on something because of a severe medical conditions that could cause my death if I became pregnant and a weird allergy made condoms not an option. My OB/GYN ended up putting in a Mirena IUD. It has a lower dose of estrogen. I was on it for 10 years (each IUD lasts for 5) and never had an issue. So you might want to investigate further because you might actually have some options that you didn't know about. Love your channel!♥️

This is 100% from progestin only bc. It stops all production of your normal progesterone which leaves you estrogen dominate. I can't believe that Dr didn't know that!!
    Progestins are not progesterone.
Also you have clotting issues in your family and melasma is also a vascular issue.
    This all makes sense.
Trouble is its triggered but no off switch for it when its birth control induced. I've had mine for almost 6 years....all of my 30s. Nothing works very long and it's always still there

I suddenly developed melasma when I had the flu and went out into the sun for on hour and a half. I would have preferred not to have gone out but my boyfriend refused to take no for an answer. The condition appeared as a mask across both cheeks and my nose. When it did not go away even after months I went to a dermatologist for laser treatments. This only made matters worse because the laser treatments were unevenly applied so now I have paler areas among the melasma. The melasma eventually left my nose and most of my right cheek but persists on the left cheek. Of course I have been upset ever since. I did not realise that exposure to the sun makes it worse because I thought it was hormonal.. I did not even know it was called melasma until watching this video. Now I wonder if the medicines I took for my flu was the cause of this unexpected skin condition. Also it seems like I was again the victim of an incompetent and careless medical system here in the Vancouver area of Canada. I feel frustrated that I always have to wear makeup to go out during the day. Anyway, Kati, I have always admired your creamy skin. I had no idea you were experiencing issues and sorry to hear about the difficulties you have been going through. I hope the problem eventually resolves.

I started the carnivore diet 10 days ago - I think the only cure is changing your diet.
so far, I've lost five pounds and stopped breaking out. well see how it goes.

Well, this is embarrassing! Sean & I incorrectly gave Dr. Jones an incorrect last name in her lower third. I’m sorry for any confusion this may have caused!

    Katie,
I saw an irreg ular spot on my left hand this past August, and totally panicked that I had cancer. I'm 51 and still in perimenopause. I called several dermatologists until I could get into see one. Actual, it was the physician's assistant. She immediately diagnosed it as Malasma. No, you don't have cancer. I almost always wear sun block and have plastic Rx sunglasses. I went off of my burth control a few years ago (was on the Depot shot for the max, 5 yrs). If it's hormonal, could it be because I'm nearing menopause?

    Hey Kati,
You unfortuanately wrote the wrong name of the OB/Gyn Mama Doctor Jones.
She is called Danielle Jones (born Brown).
Dr. Danielle Belardo is a cardiologist :D

How about Tamoxifen? It suppresses hormones.
I was on Tamoxifen for 3-4 years because I am at high risk of breast cancer.

Is there a blood test to check your hormones to see what is the normal amount so you don't get it?

hi Katy, i've been struggeling with depersonalisation/derealisation for a few years now. I've never been on birth control (or used any other medication). lately i've been thinking about it but i'm so so scared the hormones will worsen any of my mental illnesses (also my depression/anxiety) but especially my depersonalisation. I'm too scared to try out any medication since i'm scared it will trigger anything (since it has been kinda stable for a while now). Do you know more about this topic? Is it possible hormones can affect depersonalisation/derealisation disorders?

Funny how a professional recommends staying out of the sun, we need to be in the sun as humans we are we need daily dose of sun. Most peoples hormones are out of wack due to standard American diet, and lack of sun causing extremely low levels of vitamin d. Then we ask why all these problems and go to dermatologist. You can ask anyone with melasma and they will tell you just how hard it is to treat it, surely dermatologists will take your money.

I've had it for the past few months and it's definitely my hormones just from the research I've done online but I went last week to get everything tested from a doctor and they told me my hormones are completely out of whack so I don't know what to do because they didn't offer any treatment they said just to get sleep eat well and it might correct itself

I get a little of that along my hair line in summer, but I'm not quite sure if that's just normal freckles or melasma. But does anyone know of a physical sun block that isn't very thick? We used that on our kids when they were small (that's until maybe 7-8 years ago), because back then it was the only sunscreen I could find without any parabens/perfyme/colouring/other harsh chemicals. But it was such a hassle putting it on, and it rubbed off on everything they touched, making marks that couldn't be washed off.

i havent had time to watch these videos because my health issues got so bad and now things have gotten worse and i am fighting to make sure that the 600 dollar fine that DC has for not having health insurance becomes obsolete
my health issues got way worse and i heard noises from my closet and i dont know why considering i been so stressed and not sleeping that i might be on the verge of dissociation
i am trying to get rid of the leesa mattress because it caused my skin to be blue and i know CTFxC also dealing with that issue so i am trying to figure out what i might be able to come up with to help them
since i already created a planet for demons i dont know what else i can do for them or what they want other then to make me suffer more then i already am

The melasma sucks!!! You can fix it at home with corrective peels. Glycolic acid 50% or if daring a jessners. My skin is prone to hyperpigmentation, so I did a major one and I do booster ones when needed. While I am a professional cosmetologist and educator working on my PhD in Ed psych (masters forensic). You don’t need to be a pro to do them just follow the directions. Other option is azaleic acid in the AM and retinol in the pm but takes for ever. In laymen’s terms the AHAs OTC work but take weeks to years.
