This happened meters from my house. I live right under the flight path to East Midlands. My neighbour remembers seeing the plane coming over on fire and she thought it was going to crash into her bungalow as it was that low. Every year kegworth comes together to remember ❤️
Did you ever meet the young squaddies from the nearby Garretts Hay barracks who helped at the site? I was posted there at that time, but was put on guard-duty whilst the rest of the camp did what they had to do. They had horrifying stories to tell... (I had to display my rifle to block reporters from pushing past and entering the camp.)

@@sharpemang Well, yes and no. Boeing doesn't make the engines. CFM made those engines. Boeing chose them for this plane, but did not make them. However, when it comes down to it, these big corporations don't want to own up to their mistakes, and so Boeing, CFM, and the airline are going to shift blame away from themselves and pin it on the poor guys flying the time bomb.

They did their best to land that huge bird in one piece.
In light of the investigation, both pilots should have been awarded a hero's medal, and returned to their careers with apologies from Midland AND Boeing. In my ideal world. :(
The captain should have sent the flight engineer or co-pilot back to look out the windows at the engines. A very basic move.

I was actually meant to be on that flight returning to RAF Aldergrove after a weekend in London. Luckily, due to being too late to the airport (massively hungover) I had to catch the BA flight instead. My friends on the flight all made it off, one of them never flew again. Can’t say I blame him.

Shifftee they were sitting in the rear of the aircraft in the last bank of seats so couldn’t see the engines, wings or anything of importance. They were also asleep having been drinking in London’s finest pubs most of the previous day!
This might be a silly question, but why doesn’t the flight crew ask for a visual confirmation from the flight attendants about the engine fires?
I don't know, but all the information they were given pointed to the right engine so they didn't think they needed to?
As said the information they were given told them it was the right engine and calling for the crew would take too long because you have to shut the engine down before it destroys itself

It’s always easier to blast a fuse than to admit that something in the whole organized system, or the system itself, went wrong. I work in a hospital and know a deal about near-miss’s and cascade of events...
The fact that the pilots were blamed makes me so mad. Wasn't their fault, it was the damn company that messed up.
CFM for not proper engine testing, Boeing not training them properly and a little bit of pilot error
Even Captain Sullenberger underwent intense scrutiny and questioning following his landing on the Hudson. The plane was ruined....US Airways and the insurance company were none too happy.
As I noted in another comment, I strongly feel that if you are not 100% sure which engine is having issues, such as flying a new plane type you’re not familiar with, a visual inspection of both sides of the plane, both engines, through the window would be a good thing to ask cabin crew to do and report back. If this had been done, the left side engine properly identified as the one malfunctioning, this flight could have landed safely.
WRONG, it was the mistakes of the pilots. You cannot blame shutting down the wrong engine on the company. It was surely their fault that the plane crashed and the people killed because of their incompetence.

If I recall correctly, another contributing factor to the pilots making the wrong choice of engine to shut down was that the B737-400 also had redesigned cockpit gauges that were not as easy to read, or rather their design was conducive to improper interpretation, as they were a little bit different from the design of previous generations of the 737. Thus, the pilots both did not correctly interpret what their instrumentation was telling them about which engine was the problem.
Calyo Delphi yes but unfortunately TFC has used the wrong cockpit to show this. The LED indicators were tiny and the FO struggled to read them when the plane was vibrating
The indicators were did not have a high contrast and were on the outside of the dials so it was indeed harder to read
Yes they reinvented the wheel. Nothing like propper dials with pointers. Also the two parallel dial layout was criticized.

Another very well done documentary, TFC. You could feel the tension as the plane got closer to the landing strip. A respectful memorial to those who lost their lives.

I believe the Air Accident Investigation Report recommended that pilots should be able to see the engines, either directly or via cameras. It's kind of an obvious one. Has this recommendation ever been put into effect . . . ?

Uh....they could have actually seen the flames shooting from the nr 2 engine, however passengers should have gone to the cockpit and told the cockpit....sorry folks but you have a DUTY to speak up when you see things like this...no excuse, I consider this the failure of passengers basic common sense!!!! ROSEMARY DUDLEY planes don’t need mirrors because they don’t reverse on own power and won’t survive being blown by 500-600mph winds. Also it mirror broke off it would likely get sucked into the engine. Cameras on vehicles are also mainly for reversing. And if you see something wrong you can stop and get out
A number of modern airliners have a camera on the tail pointing forwards, it’s accessible in the cabin as well as the cockpit.

It seems the instruments didn't show any problem with the engine, resulting in the crew making a wrong decision about the source of the smoke and vibration. Why didn't the instruments tell them there was a fire/a problem with the left engine?
bradc002 they did. But the 737-400 cockpit engine instruments only had small LED indicators that ran around the outside of the gauges, not a big white needle on the inside like older variants. The vibration combined with the smoke made them hard to read. The cockpit display on this video is incorrect
The instruments did, however the captain decided the instruments were wrong and shut down the other engine instead. The reason for this is that he wasn't used to flying that model of plane and hadn't read up on it properly, in older versions the smoke surrounding him was indicative of a right engine failure but in the newer model it could be caused by either engine.

@@joekrim6557 unfortunately non of the passangers spoken up they just let the wrong engine shutdown and stayed complacent and stayed quiet (no bloody way I be quite if that ever happened if I was on the plane, I would of been B-lining it it for a cabin crew right away until I was sure they told the cockpit crew on the phone right in front of me, I probably call police at same time as be close enough to ground to make a call)
Why is there a culture, in the UK and US, of blaming pilots who have seconds to make life-affecting decisions, without the correct information? I suppose they are individuals, thought to be scapegoats by the airlines and manufacturers.

Its either getting to the point on some or past it on others, where the detail is just ludicris. I dont know of youve watched half of them (and actually watch it all = eyeballs attached to screen) in TWA FL800, there was serious metalurgical trama going on through the airplane. Thinking about the blood droplets being visible and or the gas being heated to 315deg + and ignited by a "very real spark" as that plane sat on a runway in NY in the middle of Summer, with the HVAC cranked (as it sits under the fuel tank that's 90% empty)... on a "newer" version of software that "runs" a ever "higher detail" and more capable graphics card and related.. is just fucking morbid.

Very good reconstruction. I believe those flight crew were scapegoated by the authorities and by British Midland. No one seems to mention CFM and the 737-400 training system. If someone can prove to me these guys acted negligently, I’d be interested to know. And now, back in 2019 we have the MAX.

My grandad was a firefighter in Nottinghamshire when this happened, he was there at the scene responding but has never talked about what happened or what he saw. I respect his decision not to bring up probably painful memories, but I think of him whenever I see a video of this accident.

I remember this incident well as i was working for leicestershire county council at the time on the motorways division and i was on duty that night driving a snow plow/gritter I was based. at LFE depot and my route only took me as far north as jct 22 were i turned round a went south to Northampton but i believe the driver based at Shepshed was one of the first at the scene and helped to rescue several people

How sad I always want everyone to live. I think hindsight is something we don’t always have the luxury to have . In moments of extreme stress your brain doesn’t process things in a normal way it sort of goes into fight or flight mode . I don’t think the pilots should have lost their jobs every indication pointed to them doing the right thing. I think the failure to test out the equipment on a real flight instead of some in-house tests were to blame entirely. Just like Max 50 mins IPad training. No sim training and not even in their handbooks. In these cases Boeing is to blame 100%
My heart is with the families of the dead and in my view innocent pilots. Pilots in all cases their only desire is to land their planes and passengers safely . There a unique bunch of guys ❤️🇬🇧🇺🇸😥✈️

"BLAME THE PILOTS" is the popular stated cause of every fatal crash until thorough investigations are completed and often AFTER investigations are finalized. Seems Boeing has a long history of failure to properly research their changes and failure to educate and train pilots properly about their "small" changes. The FAA also closes its eyes to mandating airlines to make pilots take more expensive training for every change in the position, functioning, and effects on performance of manufacturers "minor" changes and updates. This is one constant in every crash where pilots have not been adequately trained to handle anything out of the ordinary. A large part of this is the airlines rush to fully automate airplane flight and transfer control of flying from the pilots to computers that can't understand anything more than they have been programmed to understand - by non-pilots.
Boeing:: Here pilots, you're good, solid 737-300 pilots. Here is the 737-400 aircraft that works EXACTLY like the 737-300 aircraft.
Then a crash happens because the 737-400 doesn't behave EXACTLY like the 737-300 in a crisis. But let's blame the pilots who were told the 737-400 behaves identical to the 737-300 they are used to. Bloody Hell!

Jyral, Exactly! Same thing happened in New York in October 2001 with American Airlines. Giving Boeing Pilots an Airbus to fly and the pilots fly the plane apart because the rudder on Airbus is much more sensitive to input than the Boeing aircraft.

Thank you for another brilliant video. Heartfelt sadness for those lost in this incident. I believe that most of the blame is to be placed on the company for inadequate training on the new airframe and the changes that made it different. I think if the training was accomplished as it should, the correct engine would have been shut down.
And if you’re flying at night and you see a turbofan high bypass engine belching flames, start getting the crew’s attention. Those engines NEVER EVER belch flames. If they do, something inside the engine has gone wrong and will NOT improve. The engine is coming apart and needs to be shut down.

Like many people in the UK who watched this disaster on the news, I remember it largely because it happened just a couple of weeks after another massive aviation disaster which hit the UK and from which we were still reeling: Lockerbie (Pan Am Flight 103). I recall that this crash somehow linked the towns of Lockerbie and Kegworth and that residents of Lockerbie, despite still being numb from the night of December 21st 1988, reached out to Kegworth and offered support in a selfless generous way. Sometimes this kind of tragedy brings people together in unexpected ways.
This video was wonderful- the different music, the graphic pictures of the fire actually had me holding my breath! If only the passengers had spoken up! The clarity of the engine fire & the lights below were so clear. It seems ashamed the pilots had to take all of the blame. RIP to the passengers who died.
Shannon Rutledge it’s also not that accurate. The cockpit is the wrong type and doesn’t show the gauges that were hard to read and during the first “fire” it wasn’t a continuous flames it was engine surges and backfiring. The engine only caught fire on approach

Reading the pilots's experience, it's hard to imagine non experienced pilots guiding a plane into a 110 ft structure.
We now have another situation, where proper training for this model and upgrade type were not fully completed.
I agree, when new or different type planes are brought in, it is the responsibility of the airline company to train them(not the airline manufacturer)

I remember seeing Capt. Hunt turn up to the Inquiry in a wheelchair - looking a tired, broken old man (he was only 46). He and the F/O were made to carry a disproportionate amount of blame.
Boeing should have shared most of the blame for the redesign that changed the airflow dedication from one engine to another, apparently without mandating pilot training so they could understand the change. Why would Boeing make this change in the first place? ‘Upgrades’ are not always UPgrades. Some make things worse. I experienced a lot of software upgrades that made it far worse.
I remember going to my Nans house about 3 weeks after this crash . We went on the M1 as she lived in Rochdale and as we passed the site of it we could see the blackened ground of the banking at the side of the motorway. Really brought it home to me then how bad it must have been.

Another amazing video. Great work once again. A true tragedy with literal feet meaning the difference in loss of life. I’ll certainly have to read more about this, but it appears the pilots did what they thought was correct and actually worked at the time. If the left engine had give thrust for literally a few more seconds this would be a very different outcome. Heartbreaking. As a footnote. I don’t care how similar a new engine is or if it has been lab tested. It shouldn’t have taken this accident to make flight tests mandatory. There are so many parameters that an engine experiences in flight that cannot ever be replicated on the ground or by simulators, models and computer programs...especially in the mid 80s. This did NOT need to happen to save a few R&D dollars. Ridiculous.

This is *stupidly* close to the 737-MAX issues. It's like we are replaying 1989 all over again. Small, seemingly insignificant changes to engines result in non-standard behaviour that pilots have no idea how to deal with.
They didn't get confirmation from cabin crew witch engine was at fault
I think you should do a video on one of the de Havilland Comet accidents. First commercial jet airliner. The investigations into the disasters paved the way for safe modern flight.
