Depends, you'll always be taught to throw it with your pinky facing the floor, mainly when you're on the inside. But you'll find that in fights, most guys throw it with their hand facing the opponent. Its completely up to you, as long as you know the correct technique to throw a left hook, you'll be fine either way.

Because moving to the left is best for avoiding the right straight power shot. Going left circles away from the shot, going right circles into the shot and leaves you open.
@indicaDreaming is correct. If you move right, even if you evade the first one, they can double the right up - you are still in front of the right. Move left you are standing on the back side of their right shoulder - they are left with no offense except to move their feet. If they turn to you they should be met with the big left hook, otherwise they have to step out and then you win the exchange...just as Freddieson said. The ultimate goal of stand up is to utilize your footwork and technique to get in a position where you can unload on your opponent from an angle that he cant return fire. Getting outside of your opponents shoulder and/or lead foot is how ya do it. If you are doing MMA you need to use less ducking and low head movement and need to learn to fight from both stances so you can use stance switches to help get good angles. Even if you are in MMA looking for takedowns, you need footwork against a good striker because you ideally want to get the take down off an angle. SO at the end of the day you should always be working to get the superior angle. It is the only way to take big shots with minimal risk, no matter what kind of fight you are in.
Not if you start losing your technique. Hit it for as long as you can maintain good technique. Try to time it. Then start doing rounds that are about that long, with rests in between. Try to build up your stamina over time as well.

I do that sometimes. It's just to control the bag without having to fully stop hitting. When you really start hitting the bag hard it starts to move a lot. Sometimes if it's coming at you a bit too quick, you can risk hurting your wrists if you hit it while its coming at you hard. So just a quick little elbow to slow it down, or control it, and that's it.
