Portal hypertension refers to high blood pressures in the hepatic portal vein - the large vein that carries blood from the digestive tract, spleen and pancreas, to the liver. Within the liver, blood runs through tiny channels where it exchanges contents with the liver cells. This is where nutrients are processed, toxins and pathogens are removed from the blood before it exits the liver, via the hepatic veins, to enter the general circulation.
Portal hypertension usually results from an obstruction, or an increased resistance to blood flow, which may occur before, within, or after the liver. Most commonly, it happens within the liver, due to liver damage, in diseases such as cirrhosis and schistosomiasis. Cirrhosis is by far the most common cause in developed countries, while schistosomiasis, an infection by parasitic flatworms, is more prevalent in children from developing countries. Cirrhosis is the widespread scarring of liver tissues caused by long-term alcohol abuse, chronic viral hepatitis, or fatty liver.
Clinical presentation of portal hypertension may include:
- Abdominal distension, known as ascites - a result of fluid leaking into the abdominal cavity under high blood pressures.
- Enlarged spleen due to congestion of the splenic veins.
- Formation of so-called collateral vessels as alternative routes for blood to bypass the liver. These new blood vessels connect some digestive organs directly to the general circulation. By doing so, they reduce blood flow through the portal vein and relieve portal pressures, but they may also cause serious complications.
- As blood pressures increase, these small vessels can become engorged and form varices, which may rupture and bleed. Variceal bleeding can be massive and life-threatening.
- Because collateral vessels direct blood away from the liver, toxins that are normally removed by the liver can now reach the general circulation and pass into the brain, causing symptoms such as confusion, drowsiness, tremor, or even coma, in a condition known as hepatic encephalopathy.
Treatments aim to prevent and manage digestive tract bleeding, and include ongoing monitoring with endoscopy and a variety of drugs. If these fail, surgery to reroute blood flow may be performed. Liver transplantation may be needed in case of severe liver damage.

A tip to anyone in nursing school- watch videos like this when the book doesn’t make sense. Also, find the same topic explained by different uploaders (minimum 3), that way, the material sticks better when you hear the same thing from different people. Also important, don’t watch long videos With lengthy explanations! 5 minutes or less! I promise you you’ll retain it.

Their professors never worked in the profession and many of his students were lost and begging for explanation. Rather than making my own videos, I share ones I find helpful.

Im 22 and have portal hypertension due to a prehapatic obstruction, not cirrhosis. I had a blood clot in the portal vein it caused scarring in the portal vein making it narrow, making it hard for blood to flow without a problem. my liver is firm and is fine, My symptoms are feeling lethargic probably due to the low platelet count from my enlarged spleen. I’ve had three GI bleeds in my life from the esophageal varacies rupturing. Im taking nadolol to help alleviate some of the pressure and currently looking into getting a stent.

@@muhammadwaseem1230 no it is not. I am still on nadolol. My doctor checks every 6 months if my esophageal varacies have changed or not. so far I haven’t had any GI bleeds. They don’t have any plans on placing the stent.

I'm 32. Diagnosed with chirrosis in August 2022, had the tips procedure shunt placed in October 2022. Anyone having problems should definitely look into the TIPS shunt. I have an enlarged spleen also so I have low blood counts. Drowsiness, nose bleeds, sleep disturbances. Swelling in ankles/feet/legs and sometimes stomach. I have an amazing liver team at UPMC Pittsburgh and also a hematologist, cardiologist, ENT for side issues stemming from liver. MELD score of 19 back in October 2022, now as of May 2023 it is at 13 or 14.

The problem is with my wife she have partially blocked in portal vain, there is a clot and the spleen size is also 20 and the plates count fluctuate some time 35k, some time 50k

I had an acetaminophen overdose a fee years ago. I also have cystic fibrosis and had a thrombosis of the pulmonary artery that took over a yr and a half to dissolve. My overdose cause severe liver damage which I’m guessing caused cirrhosis but my doctors were useless and hardly diagnosed anything. Now i think i have this as my spleen/liver enlarges with each chest infection i get. This time is worse tho as both have enlarged causing a dull ache which hasnt happened before.
How/ why did you overdose on Tylenol?

Hii.. I am 25 yr old Suffering from portal hypertension..i had 3 episode of blood vomiting . Last time was in November 23 ...since then i have to to endoscopic banding in every month..
Some doctor said .. operation not required some said do have to do endoscopy every month ..and i have to take propranolol tablet lifetime..so i am doing every month endoscopy and taking propranolol 40 every day...
Rest is I don't know..what will happen next..😊

Hi had liver transplant 2013 . Had first fibroscan 3mts ago , it read 33.7 pka . Had endoscopy last week and hav 2small varicies and one medium . I'm 65yrs , would you be able to tell me what stage of liver disease I'm at please ? Thank you . Teresa
