    I'd had high cholesterol for many years. Initially my GP recommended a low fat diet that I stayed on for six months - to very little effect. He then recommended statins that I took for a number of years despite suffering side effects (and changing statin). By the age of 50+ I told my GP I didn't want to suffer the side effects anymore and come of the statins. I was referred to a consultant at the Royal Surrey CH in Guildford who proposed a calcium score heart scan (that cost me £500 as it was not covered by the NHS or my private health plan!) He said if the score was low he'd be OK with me coming off the statins. It was zero! So no more statins. He did recommend I took a further test after five years so in 2012 I took another test - zero again! He also said that if the NHS funded the calcium score heart scans it could save itself £millions in prescribed statins!
    I recently had an NHS Health Check (including blood test) and was called in to see my [new] GP. "You have high cholesterol and you need to consider statins." I showed him the report from the heart scan (that showed that although I was 59 I had a heart equivalent to a 39-year-old!) So I said no thanks to the statins.
    What's worrying is that the GP had never heard of the Calcium Score Heart Scan! How many other GPs are ignorant of it!?

      CAC san only shows up calcified plaque, soft plaque can be missed which can also clog arteries.

    Just last week I had to politely inform my GP that my slightly raised total cholesterol did not matter to me. I mentioned my low calcium score and my risk as per the MESA score. My triglyceride matters as does my HDL. The ratio to me is important. I'm improving that via low carb, lots of fat with the protein.

    I tried pouring some leftover vegetables down the drain in the sink. They plugged it. Proof that vegetables cause heart.

    i wish Kaiser Permanente would listen to this.I am 55 year ols male with total cholesterol of 318 and Zero CAC score no insulin resistance low BP low CRP ferritin of 28 ETC ETC yet my GP thinks I NEED to be on a Statin which Im glad im educated enough not to take.Keep educating peiple Ivor

      Hi Jerry, I am a retired Kaiser Permanente emergency physician. Kaiser Permanente is a large medical group with physicians who have different points of view. You might show this to your doctor: www.thepermanentejournal.org/issues/2015/fall/5945-insulin-resistance.html
      That might help her/him understand why you have chosen not to take a statin since it is a Kaiser Permanente article. Your doctor may recommend a statin but that is a recommendation, not a commandment. Most Kaiser physicians are pretty easy going in my experience and will be fine with your decision not to take a statin. But if your doctor still pushes back, it's easy to switch docs in the Kaiser system. My wife sees a Kaiser doctor as I do, and her doctor recommended a statin. But she had no problem when my wife declined. Hope this helps.

      Modern medicine has become so institutionalized that doctors are now essentially highly trained technicians. I call them "McDoctors".
      Sure, they sometimes get to exercise some clinical judgement, but most of they time they're just trying to fit you into a box so they can write an Rx and get you out the door. For this they rely on -wrote- rote memorization of the guidelines and decision trees, not critical thinking.
      This is why their eyes glaze over when you give them a complex history. What they want and what they expect is a short bullet point list that fits the model of an exam question on their medical licensing exam. Anything more just confuses them.
      So yeah, you want an engineer because engineers actually solve problems.

    Great presentation. I'm frustrated by the mainstream's insistence that we use the term "in remission" instead of "cured", for people who just don't have Type 2 Diabetes anymore.
    Imagine going to the doctor to follow-up on your recovery from a broken leg bone, and, instead of the doctor saying the normal thing -- "your fracture is healed" -- he said "you're only in remission, because if you fall off of a roof again, you COULD get another leg fracture". You'd think the doctor was nuts.

      Here's why I would consider the term in remission to be accurate, for me. After 4 years of keto, I'm still insulin resistant even though my a1c is very good. It's good because I manage the sugar. Even sugar substitutes raise my blood sugar, my metabolic system may always be damaged, for me. It's not falling off the roof, any sugar, including an apple throws my blood sugar off and it takes hours to return to pre food levels. Not everyone is the same, but many people are. If your blood sugar returns to pre food levels within and hr or two consider yourself fortunate. To be cured, get a sugar test - forgot the official name. And get insulin tested after you have had sugar water, see how long it takes for your body to return blood sugar and insulin to normal levels. Now that's the definitive test for diabetes. Best wishes.

    Ivor, thank you for posting this excellent presentation. Some of the slides list vitamin/mineral deficiencies as possible risk factors. Those in orthomolecular medicine (i.e. mega dose vitamin therapy) like Dr. Linus Pauling and Dr. Thomas Levy postulate that a lack of Vitamin C makes the arteries weaker and more susceptible to the disease process which leads to plaque deposition. Have you seen any such research about the effectiveness of Vitamin C either in preventing CVD or reducing the extent of plaque once it has been deposited? Thank you.

      Not as simple as that.
      There is a school of thought that say atherosclerosis is a symptom of scurvy, particularly low level scurvy over many years.

    Having been on a keto diet for about 6 weeks or so, and having lost about 6kg, I'm concerned about the part at the end (APOE4), and what he just briefly mentioned about cheeses and high fat intake. Not sure what that entails, exactly. My dad was diabetic, mom had rheumatoid arthritis, and my brother has progressive MS and is in a wheelchair.

      There is no conclusive data on this.As an a APOE4 my focus is to maintain insulin sensitivity and keep the insulin low as this is what provides the greatest protection.I do not eat dairy or grains as I am concerned about gluten and casein sensitivity and the hormone load with dairy.I largely eat what was eaten pre 10k yrs ago and the only fruit I eat is berries.I only drink distilled water and add electrolytes as required.Having got used to this way of living I largely ignore my APOE4 status and let the cards fall as they will.

      @@tonynoonan3723 Interesting. So, all dairy, including butter and cheese, or just milk, ice cream, and yoghurt? I've watched some videos that say cheese is in a different class than milk (can't recall the exact reason) as well as butter, being a fatty oil. Since I posted, I've gone about 99% carnivore, and my strength has gone up, rebound after exercise, and I've dropped another 1.5 kg.

      @@canadianroot Nutritional Anthropology and studies of the few remaining hunter gatherer societies clearly show that humans can be healthy on animal based or plant based diets.No human can be healthy on processed food.There are camps on both sides claiming the superiority of their choice.The APOE4 easily become inflamed thus an anti inflammatory diet is preferred.Gluten and casein are inflammatory especially as we age and the casein is in all dairy products.We are the only mammal that consumes the milk/products of another mammal and also the only mammal that consumes the milk of any mammal as adults.There is no doubt that some people do ok with dairy but the APOE4 need to be careful and I avoid it as I am at high risk of developing Alzheimers due to my status and family history.Coronary artery disease is in essence an inflammatory disease of the blood vessel so again,an anti inflammatory diet and a low insulin target makes sense on many levels for me.My approach is zero processed food,dairy, seed oils and grains.Other than that I eat all animals,above ground plants,berries and some nuts.Best of luck with your health and dietary choices

      @@tonynoonan3723 Thanks, my friend! Same to you. I'm curious about the talk of plant toxicity (along with words that are somewhat new to me: oxalates, phytotoxins, lectins, etc.). The fact that nutrients aren't readily absorbed from plants and fibre. All I can go by is how I'm feeling now. I've been vegan in the past, and never got toned, always had trouble with weight management (even while keeping processed foods to a bare minimum....that damn carb addiction). In my mind I wish I didn't have to eat meat, but it is what it is.

    Problem is, I have a constant battle with GP's and even yougn hospital doctors (I went in because I had a bruised lung after a fall) who insist they 'know best' and that somehow saturated fat/ high cholesterol is the devil. The fact I have a low sugar/carb diet and nice low triglycerides, am a healthy weight, and no signs of metabolic syndrome etc completely passes them by. They are obsessed. I live in France and the shortage of GP's mean a lot of old doctors are still in place or have come out of retirement. Heck, one of them even has the bloody FOOD PYRAMID on the waiting room wall. Informing people that the best thing they can do is cut out butter, eat margarine and seed oil instead ...along with 'healthy' whole grains. I doubt any of them have read a study since 1990. Of course, since the 'healthy' advice to cut back on meat and healthy fats and eat more carbs and 'stuff from the stuff machine' France is catching up on the rest of the West in terms of obesity.

    I’ve eat butter olive oil and meat 6 times a day for 28 years I’ve had a 2 full heart scans with both with contrast one with exercise the other without no blockage and I’m 45 year old male all my other organs are perfect as well

    Saying you can't eat food with cholesterol because it's bad for you is like saying you can't grow plants with carbon dioxide because it will kill them. Is it because humans are simply that stupid or is it because those we depend on in the health community fail us by falling pray to government and corporate wishes whether they be profit or resource driven. I just started a carnivore diet and I can't wait to see the results. Thanks Ivor for what you do.

    Have you heard of mixed-meal tolerance test (MMTT) , using a protein/fat/glucose drink, then test hourly. Which tests seem most benficial (besides the CAC (calcium heart scan)), you mentioned angiotension2 , and list see at 25min. , HOMA (if TG/HDL is good then no IR?), vascular function (is that BP or ??). Thank you for info. Was looking at the book Beat the heart attack gene (like 9P21 and 4Q25) by Bale and Doneen , mention Lpa, and a LPlak2? . Thanks for info

    My LDL tests over 300 mg/dL, but my other markers, like fasting glucose and insulin, A1c, Crp(hs), insulin resistance and bood pressure all measure normal/low. Tests were done after months of low carb/keto diet. Also, a NMR cholesterol test showed most LDL is large pattern A, with very low pattern B small. However I did score high on an oxidized LDL test. Confusing. I'd like to get a CAC score, but no place near me does a heart CT scan without a doctor's prescription, and I don't have a doctor.

      a cac will be prescribed by an in house dr in the establishment usually. an ldl of 300 needs to be sorted

      High LDL is only a problem if you have some of the other health markers, such as high blood pressure, high glucose/ insulin levels, and chronic inflammation.

    If i eat a Keto diet for months or a HCLF diet for months,my TC says at close to 300.My doctors want me on meds.my lifestyle,doing eveything recommended,Ie....HITT,IF SLEEP SUNLIGHT,ETC.Should i be worried?Any advices or experieces of others?

    If the fat hypothesis were true losing weight would be deadly as all that stored fat is the dangerous kind and would have to pass into the blood yet no one seems to die during weight loss in fact heart disease reverses, just the opposite expected result.

    But is a high LDL ideal? Could it be more wise to strive to attain both low LDL and low insulin resistance or do you believe the two are mutually exclusive?

      High LDL levels are commensurate with lower all cause mortality. In other words, the higher your cholesterol, the longer you are likely to live.
      Yes, apparently LDL goes up on a LCHF (Low Carb High/healthy Fat) diet.
      You do not want high insulin!! 😁👍👍

      Ldl is a lipoprotein not cholesterol. Fat glides through your body , sugar sticks in your arteries

      Saturate fat is liquid in the refigerator and solid at room temperature and solid at body temperature?! And, of course fat doesn't
      get into the blood stream or you got BIG problems. What circulates are lipoproteins. The ad was put together by liberal art majors
      of the artsy fartsey type. You know the type that would fail bonehead calculus.

      Steph keto on you tube look her up 55 and looks about 35 solid muscle been keto 15 years . I like her analogy she said if you spill fat on the floor what happens? You'd slide on it and slip up . Drop sugary drinks on a floor and you stick to it when walking on it , same in our bodies .

      Most studies hint to that correlation: the more saturated fats, the worse for most people's health. Ie eggs are not beneficial for your arteries.

      It's BS. In America when people eat eggs for breakfast what do they eat with it? Biscuits,jelly,toast,orange juice, and so on all spiking blood glucose and damaging glycocalyx and causing insulin resistance leading to diabetes.
      All the studies that demonize one food never can account for the absolute garbage we eat with it. It's implausible to lock people in a room for years and only feed them plain eggs to test. So they use questionnaires with mostly improperly done epidemiology.

    I agree fat is good and carbs need to be limited greatly but Ancel Keyes died at age 100. He was doing something right for himself anyway.

    Implying nobody knows where the countries chosen for the 7 countries study comes from is disingenuous. It's is well known and is published in multiple places in the literature unfortunately it doesn't align with the preconceived bias of speaker, therefore is ignored
