Rexulti

  • Rexulti improves symptoms of schizophrenia but is principally advertised as an add-on therapy for lingering symptoms of depression not satisfactorily improved with ongoing antidepressants. Rexulti appears to be a successor to Abilify - another similar atypical antipsychotic drug.
Questions exist regarding the wisdom of prescribing atypical antipsychotic drugs for treatment resistant depression. Weighing the potential benefits against the very real risks of significant harm leads some to doubt any advantage of Rexulti. The drug causes tardive dyskinesia - a movement disorder that typically persists long after discontinuing the drug. It also leads to weight gain, diabetes, increased cholesterol, falls and impulsive behaviors. Both fatal and non-fatal stroke may occur.
The Canadian health authorities appear unimpressed with Rexulti and recommend against its use in typical depression. Similarly a Veterans Administration study demonstrated relatively equivalent results with adding another antidepressant or switching to a different antidepressant.
The cash price for Rexulti exceeds $1100.00 a month compared to less than $25 for the generic versions of Abilify, Zyprexa, Resperdal or Seroquel.

Wow, my doctor gave this to me to take with Cymbalta which was prescribed for Fibromyalgia. I started getting depressed because I lost my job due to slow recovery from obstructive sleep apnea and fibromyalgia. He said that if the depression wasn’t manageable to take rexulti. He gave me a starter pack. I’m glad I saw your video before taking it.

I was on it for 2 years for Schizoaffective disorder. I loved it because it was the only anti-psychotic I've been on that didn't have any side effects on me. But unfortunately it stopped working after a couple years. Now I take Invega, which has it's pros and cons. Do you by chance have a video on Invega?

It is interesting that you had a severe akathisia reaction while on Abilify and 2yrs on Rexulti no such reaction although it is supposedly the same chemical formula. What if this drug gives its specific positives only to a subset of patients (like all APs), but who would otherwise have a bad reaction on Abilify (1 in 4 patients on 15mg Abilify ends up in emergency for akathisia). Maybe only those patients are the target for Rexulti: try Abilify first, but severe reaction => try Rexulti.
While on Rexulti for 2yrs, what was your LDL, HDL, Total, Triglycerides, HbA1c (ideal HbA1c level is 48mmol/mol (6.5%) or below. If you're at risk of developing type 2 diabetes, your target HbA1c level should be below 42mmol/mol (6%)) at the end of 2yr period?

I experienced fairly significant impulse control issues while on Rexulti and needed to stop the medication. What is the underlying mechanism of this as I know it is an adverse side effect of its close cousin, abilify

    • @Kris_USA90

Not sure if you're still interested in this, but it's because of the activation that the Dopamine D2 receptor, we'll just heavily involved in pleasure seeking and enjoying activities. Abilify and Rexulti both act as Partial Agonists at this site, unlike almost every other antipsychotic which are D2 antagonists. Antagonists bind to and fully block the action at a receptor site. Agonists Will bind to and stimulate action at receptors similar to the way free floating dopamine well touch that detail receptor and give you the "feel goods".
These medications have the Goldilocks Effect because they are partial agonists meaning that they can theoretically lift low dopamine levels and reduce overabundant dopamine levels.
They bind to the site and in the presence of low dopamine transmission (depression), will boost dopamine activity in their own right. In high concentrations of free androgenous dopamine floating in the synapse (schizophrenia), you are still getting some activation because the medication is sitting on the receptor and causing some sort of intrinsic activity, but still blocking/reducing levels overall.
All that being said, typically it tends to be very balancing for many, but for those sensitive to the acts of dopamine and dopaminergic transmission, in both control and dopaminergic transmission, impulsivity and hedonistic activity can increase.
      Hope that helps! :)
