I have 4 implants and a zerconia bridge in the front of my mouth. My ex was a dentist and crowned my front teeth and did bad job because he decided to root canal virgin teeth. Long story short, I had my work done at a university dental school, still not inexpensive. Had bone and tissue grafts, 16k but the work is beautiful. Warning, you still need to have good oral hygiene. I never forget to clean my mouth before bed, water pic is your new best friend. Don't think you can ignore your mouth once the work is complete.
Best dentalxare ever: ALWAYS BRUSH YOUR TEETH WITHIN HALF AN HOUR AFTER EATING . It feels good and your teeth will last your lifetime as bacteria cannot grow without food stuck on your teeth!
I really enjoyed your video. It explained a lot. I had started the implant process in my lower mouth and it failed. I spent over 20 grand and I don't have the money to start over. I started this about 5 yrs ago and now I don't have enough bone to start over. I am so disgusted with the way it went down and I doubt I will continue the process now. I was made promises that were not kept and it was really easy for the prosthodontist to tell me I will have to start over. So to say the least it left me in a really bad financial position. If you don't have money this is not the procedure for YOU!
I've done my zirconia bridge and all-on-4implants on my down jaw. I can only say that the only case is who has done work for you. My doc is amazing. She does an incredibly quality job. I've done them in Croatia. The cost was 8k. I must say that Croatia has quality dentists.
I cannot understand how some people do not understand this.....I am not a dentist but it's PERFECTLY LOGICAL!! to me that 4 implants is not enough and does not distribute the load correctly, the jaw exerts a very high amount of biteforce so it's obvious the more implants the better the load is distributed so therefore a far heavier bite force they will withstand and considering how extremely expensive this procedure is THERE IS NO EXCUSE FOR THEM TO FAIL!! any dentist worth their qualifications will DO RIGHT BY THE PATIENT!! and NOT BY THEIR PAY PACKET!!
You explained so accurately in laymen terms why I should opt for on on 6 and not all on four which concentration is focused at the front of the mouth instead of even distribution where the mollas take the pressure. I'm sold!! Thank you so much.
Great information. 100% true. Just remember with "All on 4," the downside is "none on 3," if one implant fails. If you are interested in the procedure, spend the extra money for a couple of more implants and the zirconia, it is totally worth the extra money.
I went through domestic violence and I'm trying to get new teeth I've had dentures 8 yrs they never fit I'm terrified to walk around like this forever mu bones recided so far back I found a local doctor who willing to help me and do lower implants he heard my story of childhood trauma never ever had any dental car till foster care and then ultimately domestic violence unfortunately he can't do my upper teeth do too needing bone grafts and he said it's too big a job for them basically I'm so grateful but I'm scared to go forward I pray that with my UA-cam channel 1 day I can get a whole new smile
You deserve it. Hopefully very kind oral surgeon will gift you with bone grafting you need. I am getting dentures in couple of months. I pray both of us will be smiling again soon.
I was referred to a local oral surgeon with the intention of getting some implants to replace my lower molars. The surgeon talked me into the all-on-4, and it was one of the most painful things I've ever been through. They referred me to my regular dentist for follow-up be she was clueless as to what to look for. When I insisted on seeing the surgeon, the implants had failed, probably due to the horrific bite of the hybrid in combination with the lack of follow up. They took out those implants, replaced them with a different type, after bone and tissue grafting. Then, when they were uncovered, one of them fell out. I'd had enough and went with a snap on denture. Now the tissue graft has failed, and they have washed their hands of me and sent me to a periodontist to spend more money fixing their work. I'm not a happy camper. I'm going to need work done on my upper arch soon, and they won't work with me because my implants failed.
It’s patients like your case that drive us and our message. Implants work!! But each case has to be done with exactness, experience and training is key to success. Of course we’d be happy to help and can ensure success. Sorry you had such a bad experience, we see failed cases weekly, and fix them.
My mom had implant failure the dentist did them again. They charged my mom the full price. They are fine now. Risk of failure is possibility someone didn't do their job. Which was explain that to you before they did the implants. So they should fix it for free.
They don't do them again for free. My Mom has individual implants. The dentist explained that failure is possibility. That if failure occurs my mom was responsible. For the cost of doing the implant again. Unless you have a lot of money. When complications arise you will be disappointed. Out a lot of money. Those commercials of people smiling and bite into apples. Is for selling implants. The idea that replacing missing teeth with implants. Comes with no complications and no added costs. Is simply not true. There is no one and done. They require a lot of maintenance and added cost. I don't have enough time or money for that.

People like me can only dream of having teeth that are not a nightmare. I’ve spent thousands while uninsured only to have the teeth they worked on get infected and removed. I had all my teeth when I went to him. Now my upper rear molars in the back are gone on both sides. He will no longer help save my existing teeth ? He wanted to pull all my good teeth as the only solution and get dentures ? I’m 64 and I realize I’m not worth helping in this world. It’s heartbreaking knowing that no one will help you , but the life of the poor.
I had temporary upper denture put in 5 years ago and it improved my confidence 100 percent - I will stay with denture upper and deal with lower missing teeth with implants and take out a loan
I am poor person myself. I found low income dental clinic. When the dentist came to talk to me. I already knew that my teeth couldn't be saved. Three are, infected one of the teeth the infection spread. The two were cracked at the gum line. I will be getting dentures in the next few months. If you don't get these teeth pulled. Eventually the infection will spread. If travels to your heart or your brain it becomes life threatening. I had two choices. I could get the teeth pulled and live. Or keep the teeth and lose my life. The teeth are replaceable. My life is not. Please get them pulled.
Unfortunately I live too far away. I am way over in west central Texas. I would not be looking for all on 4 or all on 6. I would be looking for single implants in multiple scattered locations.

It has taken me years to save up to do this that I'm now having a lot of health problems. I can't wait & have to settle for the All-on-4. Is it possible to add the Zygomatic Implants later? I really want that reinforcement & better distribution of bite force.

I just want to know if they can put the 4 implants in and leave space for zygoma implants. In the end I'll have 6 total. If only I knew about these sooner. I really think they're the way to go. It would've been about same cost. Less invasive, more secure, better quality, no grafts...1 & done. Why aren't they more popular? They seem like the next best thing to real. I found two wonderful Clinics in Mexico City and am going next week. All on 6 top and all on 4 total price is $15,000 . Look up a you tuve video of Kemm Dental. The Doctor studied at University of Pennsylvania. All high technology and use the DIO method which can be dound in the U.S. but with three times the price
I admire the fact your trying to give the best smile they can afford at this po8nt. Your upfront n honest which is rare. My dentist removed all mine n gave me dentures. I got sick of scraping out glue all the time. I now know if he did his job they would have fit tight . So I spent 25,000 cash only got from a trust. We did all on 4s. Mind you my dentist office for 13 yrs. I had to travel a d pay all expenses for everything. Teeth kept breaking and just replaced denture again. My top snap in broke. They wanted me to come back down for two weeks. That’s a 12k price tag for everything. So I went to place here to see if they could glue it back together " my other place also said they would have to start over. After 6 yrs of this non sense. As soon as I saw 3D X-ray of my mouth I started cry8ng. What a mess. The screws were not placed right , some were moving so now I would need 5 screws replaced and two new bridge placements also with some bone graphs done. So now it’s 18k total. No way I can do this right now but I have all exposed metal bracket on my top that is all uneven n 5 pc’s like why? So I needed quick fix so he was honest with me and said I know it’s not ideal but we could do molds and make you a fitting denture to go over you metal and fit securely n tight so you can have some kind of comfort. I went in the next day they had these wax teeth to check placement fitting n all. Then had me come back 2hrs later and it was done! It was beautiful they matched it exactly like my real teeth on the other. I don’t like the fact it’s a reg denture but it looks amazing and the entire process. 2 hr consult, 3D X-ray so cool all moldings multiple wax mock ups and finished denture. $ 389.00 yes 389.99 when we do the bottom next month it’s 240 more that’s all. So that was the most affordable way to get something for now.
One thing: as a general rule, zirconia bridge can not be longer than 3-4 units. There is a big risk of breakage. But on your video I see full arch prosthetic restorations without any kind of frame inside..?
The prosthesis needs no framework with proper implant distribution and little to no cantilever, proper occlusion and fabrication is also critically important, I have done over 5k zirconia full arch fixed cases and have had less than 6 fracture. Two were from accidents the patients had.

@@youafan259 if there were failures as you mention my practice would not survive being that I don’t sit around in a university clinic all day long I have not had time to collect the data, as I am running and growing my practice. One day I may publish my results in JPD or IJOMI, for now I am concentrating on getting practitioners such as yourself to stop doing all on 4 and do what is best for the patient. Again when properly designed zirconia is the most ideal material, for now at least. if you use it on a traditional corporate all on 4 case, it will definitely fail. But you would have to learn how to graft the sinus properly, and how to plan a case a place implants properly in order for that to happen. In other words go back to school and get proper training.

curious about something. The panoramic x-ray you show around the 8:45 mark.....there appears to be a very significant bone loss around the maxillary implants. But this isn't addressed at all. Sure there are 6, but most are 50% covered by bone at best. Is this the same patient as the pano below it? If so, did you place and restore the maxilla?
Definitely no bone loss on pano at 8:45, it’s a radiographic artifact, implants are stable and prosthesis secure after 7 years. With proper implant distribution, surgical technique and prosthodontic techniques these are life long procedures unlike all on 4.
Thanks for this video. I can imagine the all on 6 will be more secure and it is better in slowing down bone loss with 6 implants rather than 4, but I understand that cleaning becomes more complicated with more implants. Is that true? Thanks.
Ok. The all on 4 case you showed failed because there was not enough inter occlusal space ie bone reduction done at time of surgery. All on 4 with first molar occlusion done properly is a good restoration. Zirconia is not without problems too (the metal cylinders can debond from the zirconia making the prosthesis loose, ). Problems can occur with any case whether it be all on 4 or 6 or whatever. Good pre op discussions with the patient and planning will help minimize complications.
I have hundreds of other failed all on 4 cases this is just one, it’s poorly designed and prosthetically unsound the placement of the implants in relationship to the prosthesis. Also it was developed to dumb down implant surgery and implant prosthodontics. It is the patients who suffer buy putting their trust in these poor doctors who buy into the procedure without having training or experience and end up with a practice filled with failing Prosthodontics and or failing implants.
I don't care how good you are or how long you've been doing this, no one can fix my mouth!! They could have, at one time but for some reason chose not to. No doctor or dentist is ever having access to my mouth and thanks to my last experience I won't even go to a doctor anymore. Because of the last dentist I had.
I am really sorry that you didn't have good experience with the dentist. But if you are having a lot if trouble. Like I am you could have infection in your teeth. That you may need antibiotics and having them pulled. I don't want you to get sick.
$46,000 seems to be the going cost in FL. That includes pulling ALL teeth, temp set of implants for 6 months while healing, then permanent implants after that. They use Titanium implants with Zirconia teeth. The place is called "New Teeth Now". Anyone have personal information about them or the implants? Doc?
I am going next week to Mexico City for my top all on 6 and all on 4 bottom. Total cost is $15,000 with Zirconia teeth. They use the DIO method which is found in the US and top technology but Price is soon different. I did my homework and this is the best so far! I can not afford the U.S. price
I have 8 titanium implants which supports a full upper porcelain bridge for the last 15 years however the bridge is chipping and needs to be replaced. I have a great dentist and he will redo the bridge for me for a good price but I want a zirconia bridge rather than the porcelain. My dentist used the old technology of cement rather than screws. He is also installing 6 implants for the bottom bridge (currently using a partial on the bottom). I am wondering if ziconia is too heavy for the cement install. I use a HydroFloss, flossing and frequent office cleaning to keep them clean. Please let me know your thoughts about the weight or should I do ceramic or porcelain this time. I like the zirconia.
Hello great vid just found your channel. I yoo have 4 or 5 implants with my problem being unable to place implants to my upper back part of my mouth. I am told I need sinus lifts. I notice all the xrays I seem in the vid none of the pts had or needed sinus lifts? Were they all blessed with plenty of bone in that part of their mouth? They are even older then me?
I had a root canal fail at tooth 12 and had an implant put in. I asked the oral surgeon at the time if there was a certain type or way to do the implant so that in the future when I will need dentures this implant post could be used. He said they would just work with this implant. I know in the next 10 years or so I will probably need a full upper denture implant and want to plan future implants to work.
I’ve been quoted $34k for my full mouth restoration. Its been so depressing trying to figure how I’m supposed to afford that just to have a healthy mouth again on top of just wanting to eat anything harder than bread and….smile again 🥺 How do average people afford this?
I am in NY and was quoted $180,000 for zirconia bridges top bottom - I asked how I’m supposed to pay for this and response was to take out a home equity loan against house, sell the house, or hopefully get an inheritance ….
I got a price of $26,000 per arch but can get a senior discount and a discount if paying all up front. So a total of $46,000 after discounts but includes everything except the 6 month maint visits which will be $190 per visit. Hope that helps.
Hey Doc I’m a retired Army Combat Medic of 22 years … I was approved for fill mouth implants from the Army DENCOM Command … But the Periodontist as well as the Prosthodontist all PCS and my case fell through … I’m now retired after 22 yrs of Federal Active Service and I’m 100% VA disabled with Tricare Health … VA Health as well as Medicare Part A /B … I wanna do this right the first time even if I have to come out of pocket to do it …. My Teeth 🦷 have continued to deteriorate over the years after getting my mouth in a healthy field and state … But the Army failed Me! … All I did was deploy! To Iraq 🇮🇶 and Afghanistan 🇦🇫 multiple times on combat tours and they pulled teeth while was down range fighting America’s wars 🇺🇸 three 15 to 16 month deployments back to back … I have the letters of approval to verify … Can you please! Help 🤷🏾 doc? … Tell me what must I do?!
Definitely not but the occlusion and design is key, it’s critical that there be sufficient implants to support the prosthesis. All on 4 is a bad idea with zirconia prosthetics because cantilever is too big on the posterior or back of the prosthesis (where the molars and most load is) need at least 6 implants and it works great 👍

I fell on ice when I was 10 and I have been living a nightmare for 25 years after that. Never even been In a relationship sadly. I probably have the worst teeth in the US. Don’t lose hope my friend if you can get us an x ray, panoramic at a minimum I can see the extent of what you need and can give you a budget. I can promise you, if we are able to get your smile restored, it actually restores so much more. I see my patients confidence grow immediately, just take the steps and you will make progress. Do not lose hope.

I wish I knew about zygomatic implants before now, but it is what it is. So I'll settle w/ all on 4 until I can get zygomatic implants. My physical health has declined too much now to wait another minute. Which doesn't help with what's left of my mental health. My grandmother actually died because of a couple of bad teeth that kept giving her infection. Funny you mentioned the masks. That was the ONLY good thing about them. Anything will be an improvement. I am devastated after selling everything i had to get all on 4 done in Turkey by medlife group. 16 /24 teeth broke and the company are just ignoring me. I now also have TMJ problems. They quoted me for zirconium and gave me ceramic and increased 'worse case scenario' by 1000 GBP.
That’s horrible all on 4 is not for anyone as far as I’m concerned, it’s used only by untrained get rich quick dentists, period requires no surgical or prosthodontic skills.
I am really sorry. When you go to another country to have the implants placed. They don't have the same standards that we do in the US. If you run into complications you won't have any help. My suggestion is to try and find someone who can fix it or remove it. Hopefully they left enough bone for either dentures or implants in the future. I hope you get some help.
I work for a prosthodontist and this is why we hate The All in 4 approach, patients come in asking for it and we have to use this very example to talk them down and some still want them
Prosthodontist does removable dentures also right ??? The reason for my question is when. I am ready for my permanent dentures. The size of my mouth is extremely small. So I know that it will be hard to fit me. That using general dentist might be bad idea for me.

Zirconia prosthesis needs good support and four implants especially as Paolo Malo the developer of all on 4, teaches to place those implants, is not enough to provide proper support. There is also a large cantilever or lack of posterior support one of the two, but actually usually both that’s why I am not a fan of all on 4
Hello Doctor I'm having fixed bridges with 4 implants or 6 implants. If I don't like the temporary fixed bridges can I have snap on dentures with the same implants ?
Of course you can but would never recommend snap in dentures a properly made zirconia fixed prosthesis is the best way to go, especially if you already spent the money on the implants. Now if the implants are in a less than ideal position for this type of prosthesis that is a whole other issue. Good luck
Snap in dentures are a maintenance headache, the attachments constantly need to be changed and it’s very old technology we have not done attachment prosthetics for 15+ years
I am in NE Oregon and will need a full mouth, I have dentures but can not keep them in my mouth due to an autoimmune condition. Is there a way to get scans here where I am by sending an order to hospital for the scans? What time frame would need to be done? All in a day, or would there be several visits? I have medicare insurance which I know will pay for somethings. I completed the contact questions/comments but have not yet gotten an email. Thanks
Medicare does not pay for implants....or dental work..the ads for Med Advantage ar very misleading meaning any high expense work like crowns or root canals can be 2500,crowns 1200.....etc. just ask the rep. on the phone I suspect Medicare only covers cleanings .....most dentists will not accept Medicare there has been no coverage for dental for seniors for years and is a problem......unless you have dental insurance....some Dentists will not even take the Delta Dental for an 8000 root canal and pulling of a molar......

With severe bone loss of upper jaw, I was told that quad zygomatic implants were my only alternative. From what I understand, this is a form of all the on 4 process, So, are we talking the same thing? Somewhat confused.
I’d love to see a ct scan to properly guide you, zygomatic implants are just ok, but for those who have no bone, and are being treated by a surgeon not versed in proper guided bone regeneration, that is the only tool they have to offer you a fixed solution. Hey bob, i am also a dentist. I don’t want to put words in his mouth, but although both “all-on-four” and “quad zygoma” both include 4 implants they are very different protocols. Bone grafting is sometimes a better alternative to quads
I just had the top teeth pulled with having the 4 implants put in, and my dentist did put 1 on each side where the canine teeth were, then put 1 on each side towards the back where the second to last tooth was in the back, do you think I could be okay with that or should I ask my dentist to add 2 more (one on each side)?
$30.000-$70.000 would be the ballpark figure according to Google. That increases if you need bone grafting. Additional $10.000 for that. You also could have complications including implant failure or infection. In which case you would be charged the full price if that happens. Unless you are extremely wealthy. I would be looking at the best dentures you can afford. This procedure is not for middle class people period.
Dear Doctor Firstly thanks for all your help and I'm sorry for been a nuisance . I was told that it is £250 GBH for 1 hour to have conscious sedation. I have 3 teeth and 1 root to be extracted . How long will the procedure take to have 2 fixed bridges and will it hurt in the procedure ? Would it be okay to have conscious sedation for the hour to have the worst of it over ? One thing you should've mention too, is that all types of implant retain prosthesis should have a minimal thickness to avoid fractures,(4:44) of course, with the help of NG. Also should've mention about flanges is this types of prosthesis should be avoided, (12:51)that helps the ptt. to have a better hygiene. Maybe the waterpick was not used by the ptt like it was recommended. That bar used in that prosthesis...looks like it casted not milled. I'm just a tech....thanks for ur video.
Thanks for the comment, properly fabricated all on 6 zirconia prosthesis with perfect occlusion will not fracture, and alveoloplasty is unnecessary. Smooth and contour ridge but no need to reduce 5-10mm of bone to ensure proper thickness for the plastic in a hybrid. All on 4 concept has so many flaws.

Still think having these are still bad designed. What can they not put these under the natural gum? Just have to make the tooth longer and add a very fine nurl , so the gum grips the new tooth.
HELP Question for whoever sees this..I'm a disabled veteran. My rating is 90% and in order to qualify for dental you must be 100%..After a recent car accident that left me with about 11 teeth altogether, my mouth health is affecting EVERYTHING now. I cant eat so I"m losing weight ..I can't smile, I cant go to job interviews and it goes on and on. Does anyone know of a Pro Bono dentist....Or a dentist that provides donation dental for simple dentures ?? Of course implants would be better but beggers cant be choosers. I know your question was few years ago. Did you contact any Veterans organizations??? They may be able to get you the dentures you need. The only other one I have is try. Googling low income dental care the town. You live in. There should be dental clinics to call. They charge based on your income. They can pull the teeth. But they don't do dentures. That is the best f can can do.

I have 4 implants and a zerconia bridge in the front of my mouth. My ex was a dentist and crowned my front teeth and did bad job because he decided to root canal virgin teeth. Long story short, I had my work done at a university dental school, still not inexpensive. Had bone and tissue grafts, 16k but the work is beautiful. Warning, you still need to have good oral hygiene. I never forget to clean my mouth before bed, water pic is your new best friend. Don't think you can ignore your mouth once the work is complete.

I really enjoyed your video. It explained a lot. I had started the implant process in my lower mouth and it failed. I spent over 20 grand and I don't have the money to start over. I started this about 5 yrs ago and now I don't have enough bone to start over. I am so disgusted with the way it went down and I doubt I will continue the process now. I was made promises that were not kept and it was really easy for the prosthodontist to tell me I will have to start over. So to say the least it left me in a really bad financial position. If you don't have money this is not the procedure for YOU!

I've done my zirconia bridge and all-on-4implants on my down jaw. I can only say that the only case is who has done work for you. My doc is amazing. She does an incredibly quality job. I've done them in Croatia. The cost was 8k. I must say that Croatia has quality dentists.

I cannot understand how some people do not understand this.....I am not a dentist but it's PERFECTLY LOGICAL!! to me that 4 implants is not enough and does not distribute the load correctly, the jaw exerts a very high amount of biteforce so it's obvious the more implants the better the load is distributed so therefore a far heavier bite force they will withstand and considering how extremely expensive this procedure is THERE IS NO EXCUSE FOR THEM TO FAIL!! any dentist worth their qualifications will DO RIGHT BY THE PATIENT!! and NOT BY THEIR PAY PACKET!!

Just remember with "All on 4," the downside is "none on 3," if one implant fails. If you are interested in the procedure, spend the extra money for a couple of more implants and the zirconia, it is totally worth the extra money.

I went through domestic violence and I'm trying to get new teeth I've had dentures 8 yrs they never fit I'm terrified to walk around like this forever mu bones recided so far back I found a local doctor who willing to help me and do lower implants he heard my story of childhood trauma never ever had any dental car till foster care and then ultimately domestic violence unfortunately he can't do my upper teeth do too needing bone grafts and he said it's too big a job for them basically I'm so grateful but I'm scared to go forward I pray that with my UA-cam channel 1 day I can get a whole new smile

I was referred to a local oral surgeon with the intention of getting some implants to replace my lower molars. The surgeon talked me into the all-on-4, and it was one of the most painful things I've ever been through. They referred me to my regular dentist for follow-up be she was clueless as to what to look for. When I insisted on seeing the surgeon, the implants had failed, probably due to the horrific bite of the hybrid in combination with the lack of follow up. They took out those implants, replaced them with a different type, after bone and tissue grafting. Then, when they were uncovered, one of them fell out. I'd had enough and went with a snap on denture. Now the tissue graft has failed, and they have washed their hands of me and sent me to a periodontist to spend more money fixing their work. I'm not a happy camper.
I'm going to need work done on my upper arch soon, and they won't work with me because my implants failed.
It’s patients like your case that drive us and our message. Implants work!! But each case has to be done with exactness, experience and training is key to success. Of course we’d be happy to help and can ensure success. Sorry you had such a bad experience, we see failed cases weekly, and fix them.
I would sue. I can't tell you how many people I know of who have had similar experiences and were out a fortune and implant failures they won't fix.

My mom had implant failure the dentist did them again. They charged my mom the full price. They are fine now. Risk of failure is possibility someone didn't do their job. Which was explain that to you before they did the implants. So they should fix it for free.
They don't do them again for free. My Mom has individual implants. The dentist explained that failure is possibility. That if failure occurs my mom was responsible. For the cost of doing the implant again. Unless you have a lot of money. When complications arise you will be disappointed. Out a lot of money. Those commercials of people smiling and bite into apples. Is for selling implants. The idea that replacing missing teeth with implants. Comes with no complications and no added costs. Is simply not true. There is no one and done. They require a lot of maintenance and added cost. I don't have enough time or money for that.

People like me can only dream of having teeth that are not a nightmare. I’ve spent thousands while uninsured only to have the teeth they worked on get infected and removed. I am poor person myself. I found low income dental clinic. When the dentist came to talk to me. I already knew that my teeth couldn't be saved. Three are, infected one of the teeth the infection spread. The two were cracked at the gum line. I will be getting dentures in the next few months. If you don't get these teeth pulled. Eventually the infection will spread. If travels to your heart or your brain it becomes life threatening. I had two choices. I could get the teeth pulled and live. Or keep the teeth and lose my life. The teeth are replaceable. My life is not. Please get them pulled.

I admire the fact your trying to give the best smile they can afford at this po8nt. Your upfront n honest which is rare. My dentist removed all mine n gave me dentures. I got sick of scraping out glue all the time. I now know if he did his job they would have fit tight . So I spent 25,000 cash only got from a trust. We did all on 4s. Mind you my dentist office for 13 yrs. I had to travel a d pay all expenses for everything. Teeth kept breaking and just replaced denture again. My top snap in broke. They wanted me to come back down for two weeks. That’s a 12k price tag for everything. So I went to place here to see if they could glue it back together " my other place also said they would have to start over. After 6 yrs of this non sense. As soon as I saw 3D X-ray of my mouth I started cry8ng. What a mess. The screws were not placed right , some were moving so now I would need 5 screws replaced and two new bridge placements also with some bone graphs done. So now it’s 18k total. No way I can do this right now but I have all exposed metal bracket on my top that is all uneven n 5 pc’s like why? So I needed quick fix so he was honest with me and said I know it’s not ideal but we could do molds and make you a fitting denture to go over you metal and fit securely n tight so you can have some kind of comfort. I went in the next day they had these wax teeth to check placement fitting n all. Then had me come back 2hrs later and it was done! It was beautiful they matched it exactly like my real teeth on the other. I don’t like the fact it’s a reg denture but it looks amazing and the entire process. 2 hr consult, 3D X-ray so cool all moldings multiple wax mock ups and finished denture. $ 389.00 yes 389.99 when we do the bottom next month it’s 240 more that’s all. So that was the most affordable way to get something for now.

One thing: as a general rule, zirconia bridge can not be longer than 3-4 units. There is a big risk of breakage.
But on your video I see full arch prosthetic restorations without any kind of frame inside..?
The prosthesis needs no framework with proper implant distribution and little to no cantilever, proper occlusion and fabrication is also critically important, I have done over 5k zirconia full arch fixed cases and have had less than 6 fracture. Two were from accidents the patients had.

@@youafan259 if there were failures as you mention my practice would not survive being that I don’t sit around in a university clinic all day long I have not had time to collect the data, as I am running and growing my practice. One day I may publish my results in JPD or IJOMI, for now I am concentrating on getting practitioners such as yourself to stop doing all on 4 and do what is best for the patient. Again when properly designed zirconia is the most ideal material, for now at least. if you use it on a traditional corporate all on 4 case, it will definitely fail. But you would have to learn how to graft the sinus properly, and how to plan a case a place implants properly in order for that to happen. In other words go back to school and get proper training.

curious about something. The panoramic x-ray you show around the 8:45 mark.....there appears to be a very significant bone loss around the maxillary implants. But this isn't addressed at all. Sure there are 6, but most are 50% covered by bone at best. Is this the same patient as the pano below it? If so, did you place and restore the maxilla?
Definitely no bone loss on pano at 8:45, it’s a radiographic artifact, implants are stable and prosthesis secure after 7 years. With proper implant distribution, surgical technique and prosthodontic techniques these are life long procedures unlike all on 4.

Maintenance is key regardless of number of implants, so no it is not more difficult to maintain all on 6 vs all on 4.
Ok. The all on 4 case you showed failed because there was not enough inter occlusal space ie bone reduction done at time of surgery. All on 4 with first molar occlusion done properly is a good restoration. Zirconia is not without problems too (the metal cylinders can debond from the zirconia making the prosthesis loose, ).
Problems can occur with any case whether it be all on 4 or 6 or whatever. Good pre op discussions with the patient and planning will help minimize complications.
I have hundreds of other failed all on 4 cases this is just one, it’s poorly designed and prosthetically unsound the placement of the implants in relationship to the prosthesis. Also it was developed to dumb down implant surgery and implant prosthodontics. It is the patients who suffer buy putting their trust in these poor doctors who buy into the procedure without having training or experience and end up with a practice filled with failing Prosthodontics and or failing implants.
I don't care how good you are or how long you've been doing this, no one can fix my mouth!! They could have, at one time but for some reason chose not to. No doctor or dentist is ever having access to my mouth and thanks to my last experience I won't even go to a doctor anymore. Because of the last dentist I had.

I am really sorry that you didn't have good experience with the dentist. But if you are having a lot if trouble. Like I am you could have infection in your teeth. That you may need antibiotics and having them pulled. I don't want you to get sick.
$46,000 seems to be the going cost in FL. That includes pulling ALL teeth, temp set of implants for 6 months while healing, then permanent implants after that. I have 8 titanium implants which supports a full upper porcelain bridge for the last 15 years however the bridge is chipping and needs to be replaced. I have a great dentist and he will redo the bridge for me for a good price but I want a zirconia bridge rather than the porcelain. My dentist used the old technology of cement rather than screws. He is also installing 6 implants for the bottom bridge (currently using a partial on the bottom). I am wondering if ziconia is too heavy for the cement install. I use a HydroFloss, flossing and frequent office cleaning to keep them clean. Please let me know your thoughts about the weight or should I do ceramic or porcelain this time. I like the zirconia.

Hello great vid just found your channel. I yoo have 4 or 5 implants with my problem being unable to place implants to my upper back part of my mouth. I am told I need sinus lifts. I notice all the xrays I seem in the vid none of the pts had or needed sinus lifts? Were they all blessed with plenty of bone in that part of their mouth? They are even older then me?
Sinus graft works great when properly done
I had a root canal fail at tooth 12 and had an implant put in. I asked the oral surgeon at the time if there was a certain type or way to do the implant so that in the future when I will need dentures this implant post could be used. He said they would just work with this implant. I know in the next 10 years or so I will probably need a full upper denture implant and want to plan future implants to work.

With the use of zirconia upper and lower prosthetics,does it cause problems with the compression of the bite on the implants?
Definitely not but the occlusion and design is key, it’s critical that there be sufficient implants to support the prosthesis. All on 4 is a bad idea with zirconia prosthetics because cantilever is too big on the posterior or back of the prosthesis (where the molars and most load is) need at least 6 implants and it works great 👍

My grandmother actually died because of a couple of bad teeth that kept giving her infection.

Oral health is probably the most important part of overall health. Unfortunately insurance doesn't pay for much if anything. It really should. It would save a ton on secondary problems that result in poor dental health.

I am devastated after selling everything i had to get all on 4 done in Turkey by medlife group. 16 /24 teeth broke and the company are just ignoring me. I now also have TMJ problems. They quoted me for zirconium and gave me ceramic and increased 'worse case scenario' by 1000 GBP.
That’s horrible all on 4 is not for anyone as far as I’m concerned, it’s used only by untrained get rich quick dentists, period requires no surgical or prosthodontic skills.
I am really sorry. When you go to another country to have the implants placed. They don't have the same standards that we do in the US. If you run into complications you won't have any help. My suggestion is to try and find someone who can fix it or remove it. Hopefully they left enough bone for either dentures or implants in the future. I hope you get some help.
I work for a prosthodontist and this is why we hate The All in 4 approach, patients come in asking for it and we have to use this very example to talk them down and some still want them
Prosthodontist does removable dentures also right ??? The reason for my question is when. I am ready for my permanent dentures. The size of my mouth is extremely small. So I know that it will be hard to fit me. That using general dentist might be bad idea for me.

Zirconia does not have nickel and commercially pure titanium does not have nickel

Zirconia prosthesis needs good support and four implants especially as Paolo Malo the developer of all on 4, teaches to place those implants, is not enough to provide proper support. There is also a large cantilever or lack of posterior support one of the two, but actually usually both that’s why I am not a fan of all on 4
Hello Doctor I'm having fixed bridges with 4 implants or 6 implants. If I don't like the temporary fixed bridges can I have snap on dentures with the same implants ?
Of course you can but would never recommend snap in dentures a properly made zirconia fixed prosthesis is the best way to go, especially if you already spent the money on the implants. Now if the implants are in a less than ideal position for this type of prosthesis that is a whole other issue. Good luck
Snap in dentures are a maintenance headache, the attachments constantly need to be changed and it’s very old technology we have not done attachment prosthetics for 15+ years

I am in NE Oregon and will need a full mouth, I have dentures but can not keep them in my mouth due to an autoimmune condition. Is there a way to get scans here where I am by sending an order to hospital for the scans? What time frame would need to be done? All in a day, or would there be several visits? Medicare does not pay for implants....or dental work..the ads for Med Advantage ar very misleading meaning any high expense work like crowns or root canals can be 2500,crowns 1200.....etc. just ask the rep. on the phone I suspect Medicare only covers cleanings .....most dentists will not accept Medicare there has been no coverage for dental for seniors for years and is a problem......unless you have dental insurance....some Dentists will not even take the Delta Dental for an 8000 root canal and pulling of a molar......

With severe bone loss of upper jaw, I was told that quad zygomatic implants were my only alternative. From what I understand, this is a form of all the on 4 process, So, are we talking the same thing? Somewhat confused.

I’d love to see a ct scan to properly guide you, zygomatic implants are just ok, but for those who have no bone, and are being treated by a surgeon not versed in proper guided bone regeneration, that is the only tool they have to offer you a fixed solution. Hey bob, i am also a dentist. I don’t want to put words in his mouth, but although both “all-on-four” and “quad zygoma” both include 4 implants they are very different protocols. Bone grafting is sometimes a better alternative to quads
I just had the top teeth pulled with having the 4 implants put in, and my dentist did put 1 on each side where the canine teeth were, then put 1 on each side towards the back where the second to last tooth was in the back, do you think I could be okay with that or should I ask my dentist to add 2 more (one on each side)?

$30.000-$70.000 would be the ballpark figure according to Google. That increases if you need bone grafting. Additional $10.000 for that. You also could have complications including implant failure or infection. In which case you would be charged the full price if that happens. Unless you are extremely wealthy. I would be looking at the best dentures you can afford. This procedure is not for middle class people period.

I was told that it is £250 GBH for 1 hour to have conscious sedation.
I have 3 teeth and 1 root to be extracted . How long will the procedure take to have 2 fixed bridges and will it hurt in the procedure ?
Would it be okay to have conscious sedation for the hour to have the worst of it over ?

I was about to get implants & just watched the Doc on millions complaining with the FDA & it's basically being covered up.
Implants are life changing when done correctly and using highest quality products
One thing you should've mention too, is that all types of implant retain prosthesis should have a minimal thickness to avoid fractures,(4:44) of course, with the help of NG.
Also should've mention about flanges is this types of prosthesis should be avoided, (12:51)that helps the ptt. to have a better hygiene. Maybe the waterpick was not used by the ptt like it was recommended. That bar used in that prosthesis...looks like it casted not milled. I'm just a tech....thanks for ur video.
Thanks for the comment, properly fabricated all on 6 zirconia prosthesis with perfect occlusion will not fracture, and alveoloplasty is unnecessary. Smooth and contour ridge but no need to reduce 5-10mm of bone to ensure proper thickness for the plastic in a hybrid. All on 4 concept has so many flaws.

Still think having these are still bad designed. What can they not put these under the natural gum? Just have to make the tooth longer and add a very fine nurl , so the gum grips the new tooth.
HELP Question for whoever sees this..I'm a disabled veteran. My rating is 90% and in order to qualify for dental you must be 100%..After a recent car accident that left me with about 11 teeth altogether, my mouth health is affecting EVERYTHING now. I know your question was few years ago. Did you contact any Veterans organizations??? They may be able to get you the dentures you need. The only other one I have is try. Googling low income dental care the town. You live in. There should be dental clinics to call. They charge based on your income. They can pull the teeth. But they don't do dentures. That is the best f can can do.
