I have so much I want to say on this topic, but I'll keep it short. ....
Please don't settle for the diagnosis of "epilepsy", but rather try to learn the root cause for your seizures.
Seizures could indicate that you're in a severe stage of another medical issue! For example, a food allergy, auto immune disorder, or taking medication for too long, just to name a few.

Love your videos Dr. Have had seizures for 53 years and take meds, but also have arrhythmias. Was put on sodium channel blocker and out of the blue had a seizure after being on it for 1 month. They changed me med to Propranolol, and I started getting depressed so I quit taking it after 3 to 4 weeks and had a seizure. I am taking my seizure meds, but still having focal seizures almost 1 every day. This is not normal for me. I read a post from someone who had never had a seizure until they stopped taking their propranolol. The patient started back taking the propranolol and no more seizures. Beginning to wonder if it is my heart that has caused these issues.

Hello. You mentioned that if blood sugar gets too low, the patient can pass out and even have some shaking, but then they will wake up and be OK. What if the blood sugar continues to drop though - would they actually wake up, or would they just continue having a seizure, and possibly go into a coma? For example, a person who is malnourished, and the low blood sugar is not due to insulin dosing - without treating the blood sugar, I imagine it would continue dropping.

I wonder if my auras are because of blood sugar issues? I was diagnosed back in 2002 with epilepsy after having some seizures. Medication did not help. Had all the testing which came back negative. Then didn't have any seizures during a very stressful time in my life from 2016 to 2020 when they started to come back once in awhile. They always happen early morning within an hour of waking when I get the aura. Now when I get the aura, I sit and for a minute or less I close my eyes and let it pass. No shaking, no falling to the ground or couch. I just black out for less than a minute. My fear is what is happening to my brain when these happen.

Hi have had 2 seizures in my life
1st 1was when i was 34 years old and was immediately told to take epilepsy medication that was before and form of scanning
I had not been eating properly at the time such as no breaky lunch or dinner most days in the week and remember having lots of juice before my seizure and so i did not take medication
    2nd seizure was 6mths ago
(15 years later) after having an episode and released from hospital i was told to go see my gp
3 day later i went to see gp where she discovered my heart rate was 30bpm. Oh i also have hole in my heart anyways i got referals to see
Specialist and was told to have medication for epilepsy b4 any testing
    Which i didnt
4mths after being told they did eeg and told why idid not take medication and that i should
I am still waiting on heart Specialist appointment
Before siezure my son said i was like i was really tired before we left home
Like i hadnt had any sleep for days
And about 10min after that i had an episode
So my question is, is it possible to get diagnose epilepsy wrong with misfires in brain

You can switch medications! I've been having them for over a year and am trying yet another combination of drugs. The short version is, I've tried three main pills, only take 2, and have been trying different combinations. My current two do NOT have side effects like others, just maybe sleepy sometimes. The seizures haven't quite stopped but I'm feeling close. I recently began a keto diet as well.
I decided it is okay to be on meds forever but the cure can't be worse than seizures!
There are different drugs with different chemicals. Pester your doctor to try another.

Hello, im very happy to found your videos! I have a question, if i have focal epilepsy, can i take piracetam with alpha gpc (acetyl choline)? I have a serious problem after focal seizures, that im most of the time feel forgetful, slow, always non alerted and cant concentrate sometimes, most of time my motivation are fluqtuating, also sleeping really bad most of the time. I found myself that piracetam and alpha gpc given me seriously mood and motivation boost, sleeping well like a child (like never before) can concentrate normally and most important my memory, alertness, reaction and thinking speed are so good now again. But im afraid a bit due side effects of this medicines. I just play myself on these :)

Hello, I've been watching UA-cam videos about temporal lobe epilepsy and you came up on my feed. I watched your temporal lobe epilepsy video and your frontal lobe epilepsy video. I'm going to post a question in this one because I have a 20 year old son who's had an epilepsy diagnosis for 17 years.
Problem is; that in all those 17 years, no doctor has been able to definitively pinpoint what is a seizure and what isn't. Even Cleveland Clinic could not identify what his seizures actually were. (February 2022)
He has a lot of epileptiform activity in the EEG. Back ground slowing in the left temporal lobe. Spike wave discharge activity in both temporal lobes, some discharges appear in the frontal lobes; along with random spike wave discharges that appear with no pattern in other parts of the brain. I don't think they recorded any spike wave discharge that was more than 6 seconds. The current epilepsy classification is "generalized"; although he obviously does not have generalized seizures. (Or at least generalized seizures that anyone has ever identified on an EEG.)
Thus my question is; can someone have obvious epileptiform activity on an EEG and not have epilepsy?
I've dug through several research papers over the years, trying to figure out what kind of epilepsy he has and I have never found a paper that said one could have notable presence of epileptiform activity on an EEG and not have epilepsy. So, I would conclude that answer to be "no". Though I've had more than one doctor try to tell me "maybe he doesn't have seizures?" (Or doesn't have them any more.) I don't believe that for a second!
Not that he doesn't have manifestations of some sort of neurological problem. He has nearly constant tremors of the extremities; balance issues, can't "stand in one place" because he states he feels like he's going to fall over, has difficulty with stairs (feels like he's going to fall down them = seems to be a "visual input" perception problem? Can go up stairs easier than down them.) Noticeable memory problems. He gets confused. He has some fear of getting lost; more fear of disorientation than "I don't know where I am" type of "lost"?) He exhibited quite a serious speech developmental delay both in receptive and expressive speech. He was in speech therapy until 6th grade. He has some obvious "crossing the midline" motor planning problems (seems to be communication issues) between the two sides of the brain. He also sleeps between 10 and 12 hours a day.
Reasoning capacity full scale IQ score 94
Memory subsets only score in the high 50's to mid 60's though.
    He's clearly not intellectually disabled.
He's had all the genetic and mitochondrial testing that can be done. (Nothing). At least 6 LTM EEG's. 3 MRI's at this point. Vascular malformation posterior left temporal lobe of the brain. (Me thinks that's probably the cause of the epilepsy.) They found the malformation on a standard brain MRI that was done when he was 4. Just recently had "epilepsy weighted MRI". Yet to get the results; so I don't know if they are seeing atrophy in the hippocampus, or other scaring in the temporal lobes. I think they'd see it most in the left side; if they find it.
Currently doctors' best guess is "genetic epilepsy yet to be determined". He's currently classified as "intractable" and is on depekote and vimpat. Noted "no EEG change" on or off meds. While off of meds at Cleveland though; his confusion got so bad that he could not answer questions and the hand tremors got so bad that he could not put together Lego sets. At that point he was rather agitated too; (particularly with one specific doctor). So how much of the agitation was neurological as opposed to emotional reaction to environmental factors; is unclear. He's not generally an angry hostile person; but does appear to have some increase in sensory input problems over the past 5 years. It also took about 12 hours back on meds to attain some degree of steady balance.
It doesn't seem likely to me that he doesn't have epilepsy! Seems to me he has cognitive temporal lobe epilepsy and possibly even a combination of that and frontal lobe epilepsy?
After 17 years; we are both pretty irritated and don't understand why it's taken this long and we still don't have a specific diagnosis?
We'll see where current round of testing takes us? We may be contacting your office in the future though for another opinion.
Thanks for these videos though. I've found them useful.

@@kingrex593 - I took my son to Dr. Danoun at Henry Ford in Detroit between December 2022 and April 2023. They did a MEG scan, neuro-psyc testing in evaluation for surgery. He'd had a T-3 MRI done at another facility in November of 2022 so the MRI was recent enough that they could use those images. Plus I had about 19 years of discharge paperwork and school evaluations; I'd brought with us.
The team did identify his seizures. (Eyelid myoclonus) And he was diagnosed with something called "Jeavon's syndrome". Jeavon's syndrome is usually believed to be genetic; but they've yet to identify a genetic link in my son. The clinical presentation of the diagnosis does fit though.
My son also has a cluster of malformed blood vessels in the left temporal lobe of the brain which they did conclude is involved in his seizures. Now whether or not that is a "temporal lobe origin point" or just part of the entire presentation; I don't think anyone is 100% sure of. If he does have a focal epilepsy; they can't tell from the current and most recent EEGs.
And the MEG scan didn't collect enough data to know for sure if the origin point was temporal lobe or deeper in the brain. But what little activity the MEG did pick up was left temporal lobe and did not appear to be a generalized discharge. Unfortunately, they only caught one discharge; which isn't enough data to plot. They need 5 or more.
Now, he was on medication for the MEG scan; but they wouldn't have been able to do the language mapping if he wasn't on meds for the scan; so - six of one and half a dozen of the other. They would have gotten more data off of meds; but the other testing they needed to do; they wouldn't have been able to off meds. The MEG scan did show that he has language on both sides of the brain. Which positively identifies some form of neurological dysfunction in the left temporal lobe. Again though, if that represents an origin point of a focal epilepsy; no one is sure, because clearly identifiable focal discharges are not currently showing up on the EEG. (Although they had in the past. the very first LTM EEG identified a clear focal discharge in the left temporal lobe.) The very first diagnosis was suspected complex partial seizures. But the next LTM EEG had generalized discharges and the focal discharges were not identifiable from that point on.
Also the nero-psych eval showed cognitive deficits coming from the left side. These deficits do affect his short term memory as well as language processing. Multiple nero-psych evals have showed that. (He's had about 4 of them over his life time now.)
So does he have more than one form of epilepsy? That's possible.
Now because the form of epilepsy that best fits the clinical seizure presentation is a generalized epilepsy; and he's not having tonic clonic seizures. Plus where the location of the malformation is; (smack dab in the middle of his language / memory center) his epilepsy was deemed inoperable; though it likely is still classified as refractory epilepsy. He went through a med increase; so until we do another EEG; we don't know what will show up on the EEG. Currently, most short term EEG's are picking up slowing in the left temporal lobe and some sporadic generalized discharges.
They did note that Vimpat was helpful. It is controlling most of the discharges. They did note that on the LTM EEG in Detroit. When they took him off the Vimpat the epileptiform discharges got much more pronounced and he got far less functional (and more agitated). So prior to discharge they did increase the Vimpat dosage.
They wanted to try a different med; but they had problems with the insurance company and timing of authorization. It would take them 72 hours once they got the justification from the doctor for them to decide whether or not they were going to give him the med; and at that point, the patient did not want to stay in the hospital another 3 to 4 days. But that med is still on the "back burner" as an option. It would just require a little more planning if in the future they wanted to do a strait out med swap. (They'd have to put in the request for the authorization upon admission.)
And it is now known that the Depekote was likely contributing to the tremor problem in some way. He went off the Depekote in December of 2022 because after having been on it for 16 years; he was found to have severe osteoporosis of the hips. (Got someone on Depekote; make sure they do DXA scans! His prior neurologist didn't do them until I told him to do them.) Endocrinology says they will repeat the DXA scan in two years. It will take at least that long to get bone density back; but they suspect it will come back at least to some degree on account of his age.
The tremors though did improve notably; but he does still have them. So thus we know they have something to do with his neurology. He is more functional in the increased vimpat and is sleeping better. (He's not sleeping as much and is more "with it" and interactive when he is awake.)
So yes! If you have (or can get) insurance that will cover out of state care (Unless your in MI?) And you want a thorough evaluation; go to Detroit. You will get a thorough evaluation from Dr. Danoun!
So if the patient has Medicare (Not Medicaid - no state will take another state's Medicaid)! Make sure they are on "Classic Medicare" (or switch when your open enrollment starts) and get a Medi-gap plan if you can. That's what we did; although my son also has VA insurance from me. He gets state Medicaid too from our home state; but we're not in MI. In state, Medicaid will pick up what Medicare doesn't cover; but they won't do that for care that's out of state. (Thus you will need a secondary insurance to Medicare if that's the insurance the patient has.)
Now we had to wait for my son's two years on SSDI to qualify for Medicare. But he collects SSDI on his dad's record because his dad is deceased. So yes, dealing with insurance is a mess. But if you can get to Detroit; the team up there will take the time to do the testing to find what ever they can. It may not be wholly conclusive; but they will do their due diligence.
I will pray that you guys find an answer.
      I know - it's frustrating!

@@centaurora913 hmm. All of my EEGS have been normal, even right after a seizure. I only started having seizures in my early twenties when I was going through severe OCD. I was stressed to the point of exhaustion. I’ve probably had more than ten seizures but less than twenty in my life; however, I had auras as a child, and I never knew what they were. They went away for years before returning when I was stressed due to OCD. Now that I’ve been recovering from OCD, and can manage my stress, I haven’t had an aura or seizure in almost 3 years. However, I’m still on Aptiom once a day, and I hate the medication side effects. I tried to wean off of my dose last year, and started having bad withdrawals. I didn’t have seizures, but I could feel one coming on due to the withdrawals. I want to try and wean off again, but I’m not sure if it’s a good idea. I’m not convinced I need the medication, but I’m also not sure due to the fact I had auras as a young child. My neurologist didn’t diagnose me as epileptic but she did diagnose a “seizure disorder.” She’s hesitant to call it epilepsy due to the EEGS being normal.

The problem is if you don't have an event while having an EEG you can come back with a negative EEG. Also keep in mind at least 50% of people they have no idea why. Meaning your MRI can be clear as a bell but you still have seizures even though everything structurally looks ok. That's the thing that irks me. It's like a little phantom just haunting our brains.

Hey Omar, is it possible for Accutane to cause seizures? I used to take a prescribed 40mg dose of Accutane alongside 50mg of Sertraline every evening (I still take Sertraline) before I had my first seizure almost 3 weeks ago so I was wondering if the Accutane might of triggered a side effect that caused the seizure to happen.

Seizures are a rare side effect... Discuss with your neurologist

Hi, Dr. Omar. My son was recently diagnosed with epilepsy and now taking Depakene as his medication. He only seizure everytime he has a fever. But his neuro pedia cannot explain to us why it only occurs everytime he gets sick with fever. Once he gets a fever it spikes to 39 degrees. A fellow commented on my post on a FB page and told me that my son was misdiagnosed. Should I seek other neuro advice?

I can't provide medical advice.. If you are not convinced sure go ahead and ask a pediatric neurologist

I'm waiting on a neurology appointment. My gp keeps sending me to a&e and a&e keep sending me home because my blood tests and ecg are normal. My seizures started about 2 weeks ago. At first it was just one but in the last few days I have been having up to 10 a day. I'm not aware of any specific triggers. I thought they were lasting around a minute but the ambulance person said I had 3 seizures in less than 10 minutes, lasting around 15-20 seconds each. He described them as clonic tonic. I've been watching the videos to try to learn more information until i get the appointments with neurology and the seizure clinic. Can you be awake during clonic tonic seizures? When I have the seizures normally my leg starts stiffening and i know it's starting. If I'm talking I will suddenly forget what I'm saying and be unable to speak. The ambulance man described it as being 'like a light switch' when it starts. I then start convulsing. Normally my eyes are open. I can see and hear what is happening but cannot speak. After they stop I am very tired and it takes me a few minutes to have the energy to get up and speak but generally recover quickly (apart from feeling tired and spaced out). I'm wondering if this sounds like any particular kind of seizure and whether it is epileptic or non epileptic. Thank you

They diagnosed me with non epileptic attack disorder. I've since been diagnosed with autism and suspected adhd (waiting to be assessed) not had any treatment though. It's all been self management stuff. I think NEAD is a form of FND so I found some resources through fnd action and fnd hope
