Why commercials processing claims under speciality visit ?

United Healthcare are denying claims at POS 02 and 10 . Do we need to bill at POS 11 ? Few Medicare states have paid to us at POS 10, is that needs to be corrected ?

Do we suppose to bill 99454 in a date range with one unit in a month for example
DOS 4/1/22 to 4/20/22 unit 1
Or
DOS 1/4/22 to 20/4/22 unit 20
And also CMS reimbursed $62 for each month or for each unit billed in date range ?? Your help will be really appreciated.
