I think one of the worst things would be the fact that you are the boss of a transport company and you have to pave connecting highways for your transport and the peak time runs out very quickly
an enormously annoying one would be the time that passes very quickly even if you try to slow it down or stop it, finding out that you can't build anything while the game is paused 😅
Additional: Road vehicle can't be collided each other. Realistic Semi trailer truck can't overtake, but Unrealistic Semi Trailer Truck (like from newgrf Long vehicle set) can be overtake, because they use rigid truck base. High Speed Power Unit Train set can bring unique freight car. Plane can't collide each other.
For me it was always a bit stupid that you can have a two way road (with dedicated lanes) on one tile, but to make a two way raiload you need two tiles width. This makes all the train stations, railoads etc. to be rather not to scale with cities and other infrastructure. I mean how ridiculous anything with more than 4 platforms looks. In real life stations can have way more without an issue, but here even 2 platform station is already somewhat big.
Towns in OpenTTD want you to spontaneously grow the entire Amazon Rainforest in their borders so that you have to pay an extra when its time to build on top of them
basically you download for nothing anything except the graphics it won't work and they will have missing parts and the downloaded package, the best thing is to make your own map and then select "load scenario" it won't work
don't play on mobile because it will be total failure trying to make your settings and desired downloadable map with area topography 😏 where you can select cities in your country badly placed

One of the settings allows you to build while paused, I always enable it

The most stupid thing in openttd is the train route system, why it doesn't have a "round trip" option for avoid the train going to a wrong way
