Take the question, the challenge. measure something which predicts Alzheimers disease 20 years hence using an algorithm, maybe AI-like. Think about how that could be validated? Would need to screen say 100 000 to find 10 000 theoretically positives who along with the negatives would need to be followed for 10-20 years. Maybe it works, maybe it doesn't. If it works and is predictive, a double blind randomized trial of intervention versus placebo will be needed to prove efficacy. We just have to accept it will take time and there will be a lot of failures. It can be done, it is just so expensive and long to undertakes it worth it?
